An Annual visit was conducted on the date above by Licensing Program Analyst (LPA). LPA met with Assistant Administrator (AA) Elisa "Lisa" Pua. LPA spoke with Arlene Bautista by telephone. No bodies of water on the premises. Facility has a wood burning fireplace with a gas starter. Key to gas starter secured. Front of fireplace has secure glass enclosure. Physical plant reviewed. Living & dining rooms sufficiently furnished with adequate lighting. Kitchen appeared clean & organized. Sufficient dining wear & utensils. Refrigerator & freezer appeared to be clean & sufficiently stocked. 7 day nonperishable & 2 day perishable food on the premises. Resident rooms sufficiently furnished with adequate lighting. Bathrooms fixtures operational. Grab bars in all toilet & tub/shower areas. Hallways clean & clear of any obstructions. Laundry room has combination door lock. Garage access is through laundry room. Garage contains miscellaneous items, organized with not tripping hazards. Garage has upright freezer containing additional perishable food. Appeared to be clean at sufficient temperature. Backyard has covered patio with additional seat & tables. Facility front & back yards appear to be sufficiently maintained & tidy. Facility & Resident records reviewed including Centrally Stored Medication & Destruction Record (CSMDR) & Medication Administration Record (MARs). Medication stored in locked area & organized. Allegation investigated: Staff did not meet a resident's hygiene needs while in care Staff did not ensure a resident was properly dressed

On 4/3/25 at 2:45 pm Licensing Program Analyst (LPA) J. Leffall conducted a subsequent complaint visit to deliver findings on above allegations. LPA met with Administrator A1 Elisa Pua and stated purpose of visit. The Department reviewed records and conducted interviews with, R1 and facility A1. Based on the records review and interviews that were conducted with staff, residents and Administrator, all staff ensured R1 was properly dressed, groomed and has proper hygeine. Based on observation of R1, R1 was properly dressed, groomed, has proper hygeine, appeared happy and is happy living in the facility. The Resident's and Administrator that were interviewed did not confirm that residents were not treated with dignity and respect. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did occur, therefore the allegation is Unsubstantiated. On 10/28/24 at 11:30am Licensing Program Analyst (LPA) J. Leffall arrived unannounced to conduct an Annual Inspection LPA was greeted by Staff Renaldo Yabut (S1). LPA introduced self, stated the purpose of the visit and Administrator (A1) Elisa Pua was present during inspection. LPA was granted entry. 5 residents were present during inspection. LPA toured facility with A1. The facility was observed to be at a comfortable temperature, clean, in good repair, and no passageway obstructions or fire hazards were observed inside. An adequate supply of perishable and non-perishable food was observed. Freezer temperature was maintained at -10 degrees F and refrigerator temperature was maintained at 44 degrees F. Fire extinguisher was observed with a purchase date of: 5/16/24. Fire drill last completed on 10/13/24. Washer and dryer observed operational during visit. Carbon monoxide and smoke detectors were tested and observed to be operational. Residents' bedrooms were toured and observed to be adequately furnished with bed, dresser, and adequate lighting. All bathrooms are toured and observed to be operational. Hot water temperature was tested at a temperature of 112.6 degrees in bathroom 1 and 110 degrees F. in bathroom 2. Non-skid mat and grab bars observed in bathrooms. Outside of facility toured. Side gate was self-closing and self-latching. Outside was observed with adequate outdoor seatings available for residents. A sample of medications were checked and observed kept locked in the hall closet. Residents’ MARS was reviewed. Based on record review LPA observed 1 out of 1 residents medication count was under the required amount. First aide kit observed with all of the required items. All residents and samples of staff files reviewed to have all the required documents. Deficiencies are being cited on the attached 809D in accordance with California Code of Regulations, Title 22,Division 6. Exit Interview conducted. The following documents requested to be updated and submitted to Fresno CCL by 11/12/24: Lic 308, Lic 500, Lic 610E, Current Liability Insurance and current Administrator’s certificate. Type of visit: Prelicensing

On 11/08/23, Licensing Program Analyst (LPA) M. Yang conducted an announced Pre-licensing / Component III Inspection for Change of Location. LPA introduced self, stated the purpose of the visit, and was allowed entry into the facility. LPA met with Administrator 1(A1) Arlene Bautista and Administrator 2(A2) ELisa “Lisa” Pua. LPA tour facility with A1 and A2. The facility is a 5 bedroom and 2 bathroom home and fire clearance were granted for 6 Non-Ambulatory for a total capacity of 6. Facility was free from ground obstructions and odor free. Common areas were observed to have adequate seating and lighting available. Kitchen observed to have dishes, plates, utensils. Knives was kept locked and secure under kitchen sink. LPA observed a 2 day supply of perishable foods and a 7 day supply of non-perishable foods. Refrigerator temperature observed maintained at 34 degree F and freezer temperature maintained at -7 degree F. Medications will be locked in kitchen shelve and in hall closet. Cleaning supplies and chemicals observed to be locked under kitchen sink and in laundry room. A fire extinguisher was observed and had a service date of 05/12/23. First aid kit was observed and contained all required items. Bedrooms were observed to have the required furnishing and are ready for occupancy. Bathrooms were observed functional and operational during inspection. Hot water temperature 120 degree F in bathroom 1 and range between 118.9 to 118.8 degree F in master bathroom. LPA observed a supply of extra bed linens and personal hygiene and grooming products. Outside of facility toured. Exits were open and free of obstructions. Side gate observed self-closing and self-latching. LPA observed adequate outdoor seatings available for residents. Smoke detectors and carbon monoxide were observed to be operational during this inspection. Component III was conducted during today's pre-licensing visit. I have found that the applicant has met all pre-licensing requirements.
