Allegation investigated: Facility is abusing residents

On April 1, 2026, the Department of Social Services staff conducted an unannounced visit to this facility to continue investigation of the above allegation and to deliver findings. The Department was met by Val Paraiso and the purpose of the visit was explained. Investigation consisted of the following: On October 6, 2023 the Department conducted an unannounced initial visit to the facility to investigate the complaint allegation mentioned above, in addition to a subsequent visit on October 2023 to continue investigation. During those visits, it was determined that the complaint required further investigation. On April 1, 2026, the Department conducted a facility tour with Administrator, obtained staff roster (dated: 2/14/26), client roster (dated 3/31/26), staff training: Resident Rights (dated ), copy of signed Suspected Elder Abuse policy (various dates),The department interviewed Administrator (A1), and 2 staff (S1-S2), and 5 Residents (R1-R5) Page 1 of 2 Unsubstantiated The investigation revealed the following: Allegation: Facility is abusing residents The complaint indicates an allegation that residents are being abused; however, no information was provided regarding the type of alleged abuse or the date of the incident. On, April 1, 2026, at 1:27pm, the Department interviewed Administrator (A1) , who denied the allegation stating that there have been no reports of staff abusing resident—physical or otherwise. A1 further stated that his staff has had resident rights training and are aware of the facility’s policy on suspected elder abuse. The training is refreshed during in-service meetings. On April 1, 2026, between 1:00pm and 2:39pm, the Department interviewed 2 [available] staff regarding the allegation. 2 out of 2 staff denied the allegation stating that they have never abused any residents nor have they witnessed any other staff abusing a resident in care. Additionally, 2 out of 2 staff state that she has had residents right training. On April 1, 2026, between 2:00pm and 4:15pm the Department interviewed 5 resident (R1-R5) regarding the allegation. Of those interviewed, 5 out of 5 Residents states that staff treat them well and has never hit, kicked or pushed them nor have they witness staff hit, kick or pushed another client in care. Additionally, 5 out of 5 state that they feel safe and well cared for in the facility. On April 1, 2026 the Department reviewed and evaluated the following documents: staff roster (dated:2/14/26), client roster (dated 3/31/26), staff training: Resident Rights, copy of signed Suspected Elder Abuse policy (various dates ranging from 3/25/16-10/31/25), On April 1, 2026, the Department made the following observations during the facility tour: The department observed the facility to be clean, sanitary and free of odors, Additionally, the department observed the Residents to be well groomed. Lastly, the Department observed the staff to be attentive to the residents in care. Based on the information gathered, there is insufficient evidence to support the allegation mentioned above; Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED. Licensing Program Analyst (LPA) Jacqueline Shaw-Ross conducted an unannounced annual required visit to the facility. Upon entry, LPA was greeted by Janet Moster, caregiver, and informed them of the purpose of the visit. Administrator, Val Paraiso, arrived shortly. At the time of the visit, there were (1) staff member and (4) residents present. LPA was advised that (2) residents were out in the community. Facility Overview: The facility is a one--story home with (6) bedrooms and (5) bathrooms. There are no pools or known firearms on the premises. Infection Control: LPA observed that hygiene and cleaning supplies were available for regular facility maintenance. The facility’s infection control plan was reviewed and found to meet department requirements. Physical Plant: The physical plant, including floors, windows, and doors, was clean and well maintained. Fixtures and furniture were in good repair. The outdoor area was free of hazards. Laundry equipment was in good working condition. Sharp and dangerous objects were securely locked and inaccessible to residents. Both the smoke detector and carbon monoxide detector were operational, and the hot water temperature was 118°F. There are four fire extinguishers located throughout the facility and all have current inspection tags. Food Service: The facility’s kitchen was clean and equipped to prepare food. The facility maintained the required two-day supply of perishable foods and a seven-day supply of non-perishable foods. LPA observed one gallon of milk that showed an expiration date of 01/18/2026, however upon further inspection, LPA observed that the facility kept frozen gallons of milk in the freezer. The gallon of milk in question was not "technically" expired and did not appear to be altered in consistency and smell. LPA advised Administrator to use milk within adequate time frame to the ensure health and safety of residents. Continued on LIC809-C.... Care & Supervision/Administration: Adequate staff were present to supervise residents during the visit. The administrator holds a current administrator’s certificate. Record Review and Resident/Staff Files: LPA reviewed files for (3) staff members, confirming criminal clearances, updated training, and CPR/First Aid certification. (4) resident files were reviewed and contained all required documentation. Health-Related Services/Incidental Medical Services: All resident medications were securely locked. LPA reviewed medications for (3) residents, confirming that all medications were listed on the Medication Administration Record (MAR) and accounted for. Disaster Preparedness: LPA reviewed the facility’s emergency and disaster plan, including documentation of the last disaster drill conducted on 11-15-2025, which met department requirements. All facility exits were clear of obstructions. No citations were issued today. Licensing Program Analyst (LPA) Kathleen Banrasavong arrived unannounced to conduct an annual inspection. Upon arrival LPA was greeted by facility staff and granted entry. LPA began inspection with introduction and visit purpose. Upon arrival LPA learned that six (6) residents live at this facility. There was one (1) staff member was present. The Administrator, Val Paraiso was advised of the annual and came to conduct and completed the facility tour. Client Records/Incident Reports/Clients Rights Information: LPA reviewed client records. Six (6) records were reviewed. LPA reviewed for identification and emergency information, admission agreement, medical assessment, and TB test results, needs and service plans, placement, functional assessment, centrally stored medication/destruction records, safeguard for personal property/valuables, and personal rights notification. Personnel Records/Training/ Staffing/ Administration: LPA reviewed employee records. Two (2) records were reviewed. LPA reviewed employee records for first aid certification, criminal record clearance or an exemption, health screening and TB test results, employee rights, training verification, and current administrative organization. Val Paraiso, Administrator’s certificate expiration date was 05/02/2025. Food Service: Food prep areas are clean and organized. Food supply meets the requirement of one (1) week supply of nonperishable and two (2) day supply of perishables. Emergency food and water supply is present. There is a location for sharps in the kitchen. Physical Plant and Safety of Environment/Operational Requirements: LPA toured the facility inside and outside. LPA observed the facility to be clean and in good repair. The facility is maintained at 75 degrees for the client’s comfort. Lighting is sufficient for safety. Water temperature measured 108.0 degrees F. Laundry is done in the laundry room. There is a locked location for storing laundry soap, cleaning supplies and chemicals in the closet in the laundry room. All outdoor and indoor passageways are free of obstruction. Emergency lighting is available. There is a telephone working at this location. LPA dialed the facility’s landline number, which rang and was operable. The LIC 610, emergency disaster plan is maintained. There are no firearms at this facility. There is one (1) secured fireplace at this facility. There are zero (0) pools at the facility. There are two (2) secured gates that have a self-latching lock located on the front and northwest side of the house. LPA observed emergency supplies and one (1) first aid kit. The last emergency fire drill was conducted on 11/05/2024. Infection Control: The LPA observed the hand washing stations in the facility restrooms and kitchen had hand hygiene supplies and hand washing signs. LPA observed PPE equipment and cleaning supplies to do regular cleaning of the facility. LPA reviewed the facility's infection control plan which met department requirements. LPA reviewed staff records and found that staff had infection control training. Medications/Health Related Services/Incidental Medical Services: The medications are centrally stored. There is a locked cabinet allocated for medication storage. Centrally stored medication and destruction logs are maintained separately. LPA reviewed medication logs and observed that they were dispensed accurately. LPA made observation throughout the inspection process to assess if the facility remains in conformity with the State Fire Marshall regulations. LPA observed ten (10) smoke detectors and two (10) carbon monoxide detectors throughout the facility. The facility is also monitored 24 hours by ADT. Pursuant to Title 22 of The California Code of Regulations Division 6, there are zero (0) deficiencies observed. On January 16, 2024, Licensing Program Analyst (LPA), Venus Mixson arrived at the facility unannounced to conduct the required annual inspection and met with the Administrator, Val Paraiso. The facility file review was conducted in the office and additional records were requested and reviewed on site. The facility is licensed for six elderly and operating at full capacity. LPA Mixson toured the facility along with the Administrator and inspected the facility inside and outside, and there were no obstructions to the indoor or outdoor passageways at the time of this visit. The facility is a single-story home, located at 2644 Canyon Rd. Escondido, CA 92025. Physical Plant: The facility phone number is (760) 739-0311 and is operable. The LPA observed the residents bedrooms, and they are equipped with required furniture as per Title 22. The LPA inspected facility bathrooms, and the hot water temperature was tested and tested within regulations. The bathrooms were clean and appliances were operating appropriately currently at the time of this visit. The facility is equipped with operating smoke detectors, carbon monoxide alarms, and fire extinguishers. The LPA observed required postings such as "If you See Something, Say Something" the "Personal Rights" and the Ombudsman postings were posted in a common area. The cleaning supplies and sharp items were kept locked and inaccessible to the residents in care. There was a designated storage space for the resident and staff files and it was locked. Medications: were reviewed, locked and inaccessible to residents, and there was a 30 day supply. The overall facility is clean, the furniture is in good condition. The facility heating system and other appliances were operable currently at the time of this visit. Food Service: Non-perishable and perishable food supply is sufficient per regulations, and there are a variety of food types available for residents. Dishes and utensils were in sufficient supply and stored properly. Care & Supervision Facility has sufficient staff, two staff at the time of this visit and the staff were engaging the residents. Records Review: The LPA reviewed files and conducted staff interviews, and resident interviews. Previous Community Care Licensing forms were reviewed. There were no Title 22, Division 6 Regulation violations observed or cited during today’s visit.
