Licensing Program Analyst (LPA), Robert Campbell arrived at the facility unannounced to conduct the Required Annual Inspection and met with the Angelique Richardson - Licensee. The facility file review was conducted in the Regional Office and additional forms were reviewed and requested on site. The facility is licensed for six (6) Elderly Adults and is currently operating with zero (0) Residents. The facility is currently wanting to accept Residents at this time. No Staff were present at time of visit. Licensee will contact Community Care Licensing upon accepting Residents to live in the facility. LPA toured the facility along with Licensee, and inspected the facility inside and outside, and there were no obstructions or debris to the indoor or outdoor passageways at the time of this visit. The facility is a single-story home with five (5) bedrooms, and three (3) bathrooms. Physical Plant: Facility phone number is (909) 645-4998 and it is operable. The LPA observed the residents’ bedrooms, and each was equipped with required furniture as per Title 22. LPA inspected facility bathrooms to be clean and in good repair. The bathrooms were clean, and appliances were operating appropriately currently at the time of this visit. The facility is equipped with operating smoke detectors, carbon monoxide alarms, and fire extinguishers. Medications: No medications were available to review at visit. Food Service: Perishable food supply is sufficient per regulations, and there are a variety of food types available for residents. Dishes and utensils were in sufficient supply and stored properly. On 8/5/25, Licensing Program Analyst (LPA) Valerie Flores conducted an announced visit to the facility for the purpose of conducting a pre-licensing inspection. LPA Flores met with facility representative, Henri Chazaud, who accompanied LPA for the tour of the facility. The Applicant has submitted an application to attain a capacity of (6) six residents. On 05/06/25 the City of Escondido Fire Department approved a fire clearance for the six (6) non-ambulatory and (0) zero bedridden residents. LPA observed the following during today's inspection: The facility is a single-story structure with (5) five resident bedrooms, (2) two bathrooms, formal dining room, kitchen, living room, great room, garage, and backyard area. The bedrooms were observed to have met the required bedding, lighting, and furniture. There are plenty of extra linen (sheets, blankets, towels) that were observed to be in good repair. LPA observed a fully charged fire extinguisher. LPA observed a fully stocked first-aid kit located in the kitchen area. A locked cabinet in the kitchen pantry was observed to store knives and other sharp objects. The carbon monoxide and smoke detector were tested and were observed to be operable and in good working condition. The facility has more than a two (2) day supply of perishable foods and seven (7) day supply of non-perishable foods. Resident showers are equipped with slip-resistant strips and grab bars. The patio area is shaded with adequate seating. LPA observed emergency food and water located in the garage. (Continue to LIC809C...) (Continuation from LIC809) Centrally stored medication is kept in a locked cabinet that is inaccessible to residents in care. All indoor and outdoor passageways were free of obstruction. The hot water temperature was measured at 116.0 degrees Fahrenheit meeting the required limits. The facility has an emergency disaster plan and approved infection control training plan are maintained at the facility. Per Henri, there are no firearms or ammunition on the premises. LPA observed the required postings of the emergency disaster plan, resident personal rights, employee rights, emergency contacts, facility sketch, CCL Complaint poster and the Long-Term Care Ombudsman poster posted on the walls throughout the facility. During today's visit, LPA Flores did not observe any issues or concerns. Final approval of licensure will be determined by Centralized Application Bureau (CAB). On 07/29/25 Licensing Program Analyst (LPA) Valerie Flores made an announced visit to the facility to conduct a pre-licensing inspection. LPA conducted a tour of the interior and exterior areas of the facility, and was accompanied by the Applicant, Henri Shazaud. The applicant has applied for a Residential Care Facility for the Elderly (RCFE) for a capacity of (6) six non-ambulatory residents. On 5/6/25, The city of Escondido Fire Department granted a fire clearance for (6) non-ambulatory residents. During the tour, LPA observed the following: The facility is a single-story structure with (5) five resident bedrooms, (2) two bathrooms, formal dining room, kitchen, living room, garage, and backyard area. LPA observed a (2) two-day supply of perishable foods and a (7) seven-day supply of non-perishable foods. Located on the kitchen refrigerator is an anticipated meal calendar that promotes healthy eating. The kitchen area was kept clean and free of litter, vermin, rodents, and insects. The combined smoke and carbon monoxide detectors were tested and found to be operable. The facility maintains sufficient space to promote and facilitate the program of activities. The resident bedrooms were observed to be missing a chest of drawers, bedspread, chair, and nightstand. Resident bathrooms were observed to missing the required grab bars and slip-resistant strips in the showers. LPA did not observe extra linen available for the anticipated residents in care. Based on today's observations the facility is not ready for licensure at this time. LPA confirmed that the applicant was in possession of the self-certification checklist. The applicant was informed of what is needed to be corrected and will call to schedule another inspection date.
