On 08/03/2026, at 09:37 AM, Licensing Program Analyst (LPA) James Sampair arrived at the community unannounced. During the investigation of complaint 15-AS-20260713130153, the LPA observed deficiencies and added this case management. Upon entry into the facility, the LPA identified himself and stated the purpose of the visit to Caregiver Asterio Quiblat. Administrator (ADM) Lucas Argueza arrived at approximately 10:45 AM. The facility was cited for two Type-B deficiencies: no license posted and physical plant repairs needed for the front door lock, bathroom sink handles, and sliding glass door. On 10/13/2025, at 1:00 PM, Licensing Program Analysts James Sampair and Andrew Christy arrived unannounced for this Case Management - Deficiencies visit. Upon entry, the LPAs explained the purpose of the visit to Caregivers Elena Ilagan and Asterio Quiblat, who were later joined by Managers Lucas Argueza and Vincente De Peralta. The Licensee was cited for fingerprinting violations for 3 staff members. 2 Type-A citations were issued during this inspection (for details refer to LIC 809-D). On 10/13/2025 at 1:00PM, Licensing Program Analysts (LPAs) Andrew Christy and James Sampair arrived unannounced to conduct the Required Annual Inspection. Upon entry, LPA stated the purpose of the visit to Staff Members Alena Ilagan and Asterio Quiblat. Manager Vicente Deperalta arrived at approximately 2:00 PM. The LPAs inspected the interior and exterior of the facility. The inspection of the physical plant included the kitchen, dining area, restrooms, community living spaces, resident rooms, storage areas, garage, and the grounds of the facility. More than the required minimum of 7 days of nonperishable and 2 days of perishable foods were appropriately stored. The hot water temperature in the resident's bathroom was measured to 114.8 degrees Fahrenheit and the living room temperature was 71.4 degrees Fahrenheit. The fire extinguisher was last serviced on 09/16/2024. The carbon monoxide and smoke detectors were fully operational. The LPAs observed required postings in the facility, including the Residential Care Facility for the Elderly Complaint Poster, Ombudsman and Personal Rights posters, and the Theft and Loss Policy. LPAs reviewed the facility records, records of 5 residents, and records of 5 staff members. The Emergency Disaster Plan was last reviewed 10/13/2025. The following deficiencies were cited during the inspection: At 1:20PM, LPAs found an unlocked cabinet in the garage with cleaning chemicals like bleach and Comet. As well, there were screws in the drawers. At 1:45PM, there was a bathroom discovered to have an unlocked cabinet under the sink with various cleaning chemicals, as well as more screws in the drawers. At 1:05PM, the fire extinguisher was found to be expired. Deficiencies are cited per Title 22 California Code of Regulations and listed on LIC809D. On 11/08/2024 at 8:30 AM, Licensing Program Analyst (LPA) James Sampair arrived unannounced to conduct the Required Annual Inspection. Upon entry, LPA stated the purpose of the visit to Staff Members CONNIE VILLARETE and BONAFE QUIBLAT. Administrator Alberto Bernardino arrived at approximately 9:55 AM. The LPA inspected the interior and exterior of the facility. The inspection of the physical plant included the kitchen, dining area, restrooms, community living spaces, resident rooms, storage areas, garage, and the grounds of the facility. More than the required minimum of 7 days of nonperishable and 2 days of perishable foods were appropriately stored. At approximately 10:55 AM, the kitchen hot water temperature was 124.3 degrees and the living room temperature was 71.1 degrees Fahrenheit. The fire extinguisher was last serviced on 9/16/2024. The carbon monoxide and smoke detectors were fully operational. The LPA observed required postings in the facility, including the Residential Care Facility for the Elderly Complaint Poster, Ombudsman and Personal Rights posters, and the Theft and Loss Policy. An administrator is on site more than the minimum of 20 hours a week to oversee the proper business operations. The LPA reviewed the facility records, records of 5 residents, and records of 5 staff members. 3 Type-A and 3 Type-B citations were issued during the inspection. On 11/15/2023 at 3:45 PM, Licensing Program Analyst (LPA) J. Sampair arrived unannounced to conduct an annual inspection. Upon arrival, LPA explained the purpose of the visit to Caregiver Edelita Pine. Administrator (ADM) Raquel Coyle arrived at approximately 4:30 PM. During the Inspection, the LPA inspected the facility inside and outside. All outdoor and indoor passageways are kept free of obstruction. A comfortable temperature is maintained at 68 degrees Fahrenheit. The LPA observed that the lighting in all rooms is adequate for the comfort and safety of the residents. Residents’ bathrooms are equipped with grab bars and non-skid mats. There is a minimum of one week supply of nonperishable and 2-day of perishable foods. Sharps were locked and inaccessible to residents. Fire extinguishers observed to be fully charged. Annual inspection is incomplete. The LPA will return unannounced at a later date to complete the inspection. 1 Type-A citation was issued during the visit. Exit interview conducted with ADM.
