On 07/06/2026, at 09:45 AM, Licensing Program Analyst (LPA) James Sampair arrived unannounced to conduct this Required 1 Year inspection. Upon entry, the LPA explained the purpose of the visit to Kathereen Dayap and Rizalino Deperio. The Administrators Norberto “Bing” Geronimo Jr. and Eileen Carreon arrived at approximately 10:30 AM. The LPA toured the community inside and outside. The tour of the community included, but was not limited to, residents’ rooms, bathrooms, the kitchen, common areas, the garage, and the yards. The community was clean, appropriately furnished, and well lit. There are no bodies of water on the grounds of the community. The walkways in both the interior and exterior of the community were free from obstructions. The residents’ bathrooms are equipped with grab bars and slip-resistant mats. At 10:06 AM, the inside temperature was measured at 71 degrees Fahrenheit in the dining room. At 10:24 AM, the refrigerator temperature was measured at 39 degrees and the freezer at -3 degrees Fahrenheit. At 1:34 PM, the maximum hot water temperature was measured at 116 degrees Fahrenheit in the kitchen. There is more than the required minimum of a 7-day supply of nonperishable foods and a 2-day supply of perishable foods on hand. Disinfectants, cleaning solutions, poisonous substances, knives, matches, tools, sharp objects, and other similar items were in locked storage and stored in areas separate from food supplies. The centrally stored medication cabinets were locked and each resident's medication was stored in its originally received container. Continued on LIC 809-C . . . . . . The Emergency Disaster Plan was reviewed by the ADM on 07/06/2026. Emergency and disaster drills are conducted quarterly; the most recent was conducted on 06/12/2026. The first aid kit was observed to be complete, and staff dispense medications directly from the original container to residents. The liability insurance expires on 07/31/2026. An administrator is on site more than the minimum of 20 hours a week to ensure proper business operations are being conducted. The LPA reviewed community records, 5 resident records, and 5 staff records. No citations were issued during the inspection. Type of visit: Case Management - Other

On 06/17/2026 at 02:00 PM, Licensing Program Analyst (LPA) James Sampair arrived unannounced to conduct a case management visit to deliver an immediate exclusion letter. The LPA met with Caregiver Joy Fernandez and explained the purpose of the visit. The Administrator (ADM), Norberto Geronimo, was out of the facility. During visit, the LPA hand delivered the immediate exclusion letter for Staff S1 to the Caregiver, who stated that S1 had resigned. The LPA communicated with the ADM via voice mail and text messaging. Through text messaging, the ADM confirmed that S1 is not currently employed at the facility and will be immediately removed from the facility's Guardian Roster. On 07/07/2025 at 1:00 PM, Licensing Program Analysts (LPAs) James Sampair and Andrew Christy arrived unannounced to conduct the Required Annual Inspection. Upon entry, LPAs stated the purpose of the visit to Lead Caregiver Elizabeth Joy Fernandez. The LPAs inspected the interior and exterior of the facility. The inspection of the physical plant included the kitchen, dining area, restrooms, community living spaces, resident rooms, storage areas, and the grounds of the facility. More than the required minimum of 7 days of nonperishable and 2 days of perishable foods were appropriately stored. Temperature in the dining room was measured at 74.8 degrees Fahrenheit and the water temperature measured 118.0 degrees Fahrenheit. The fire extinguisher was fully charged and last serviced 12/11/2024. The carbon monoxide and smoke detectors were fully operational. The LPAs observed required postings in the facility, including the Residential Care Facility for the Elderly Complaint Poster, Ombudsman and Personal Rights posters, and the Theft and Loss Policy. An administrator is on site more than the minimum of 20 hours a week to oversee the proper business operations.
