Licensing Program Analyst (LPA) Robert Campbell conducted an unannounced annual required visit. Upon entry, LPA was greeted by Mary-Jane Estiller/Manager, and informed them of the purpose of the visit. Mari Dee Sandra Cid/Administrator, arrived later during the visit. Facility Overview: The facility is a one-story home with ten (9) bedrooms and six (6) bathrooms. There are no pools or known firearms on the premises. Infection Control: LPA observed that hygiene and cleaning supplies were available for regular facility maintenance. The facility has infection control plan in file. Physical Plant: The physical plant, including floors, windows, and doors, was clean and well maintained. Fixtures and furniture were in good repair. The outdoor area was free of hazards. Laundry equipment was in good working condition. Sharp and dangerous objects were securely locked and inaccessible to residents. Both the smoke detector and carbon monoxide detector were operational, and the hot water temperature was 105°F. Fire extinguisher located at kitchen had current inspection tag. Food Service: The facility’s kitchen was clean and equipped to prepare food. The facility maintained the required two-day supply of perishable foods and a seven-day supply of non-perishable foods. Continued on LIC809-C.... Care & Supervision/Administration: Adequate staff were present to supervise clients during the visit. The administrator holds a current administrator’s certificate. Record Review and Resident/Staff Files: LPA reviewed files for four (4) staff members, confirming criminal clearances, updated training, and CPR/First Aid certification. Five (5) resident files were reviewed and contained all the required documents. Health-Related Services/Incidental Medical Services: All resident medications were securely locked in medication cabinet. LPA reviewed medications for five (5) residents, confirming that all medications were listed on the Medication Administration Record (MAR) and accounted for. Personal & Incidental Funds: All residents monies are not handle by the facility. Disaster Preparedness: LPA reviewed the facility’s emergency and disaster plan, which met department requirements. All facility exits were clear of obstructions. LPA gave technical assistance with the Manager and Administrator on completing quarterly emergency drills and submitting a drill for 7/2026 by email to the LPA by 7/13/2026. On 7/31/2025, Licensing Program Analyst (LPA) Valerie Flores arrived unannounced to the facility for the purpose of conducted the 1-year required visit. LPA met with caregiver, Fe Dimzon, and explained the purpose of the visit. LPA was granted entry, and a tour of the facility was conducted. During today’s visit, LPA observed the following: The facility is a single-story structuring consisting of (8) eight resident bedrooms, (1) one staff bedroom, (5) five bathrooms, formal dining room, kitchen, garage, living room, and backyard. Resident bedrooms were equipped with the required bedding, furniture, and functional lighting. Additional linen and towels are available for residents and appear to be in good repair. LPA did not observe no pools or bodies of water. LPA observed the kitchen to have a clean environment to prepare food and possessed equipment in good working condition. There was a locked cabinet located in the kitchen that stored knives and other sharp objects. LPA observed the facility met the required (2) two-day supply of perishable and (7) seven-day supply of non-perishable foods. LPA observed a locked cabinet located in the dining room that contained centrally stored medication. A locked cabinet located in the hallway stored disinfectants, and poisonous substance. The facility maintains telephone services on the premises. Indoor and outdoor pathways were free of obstruction. Outside shaded seating is available for the residents in care. The living room contained activities such as books, puzzles, and board games. Fire extinguishers were fully charged and inspected. Smoke alarms and carbon monoxide detectors were deem fully operable. Per Administrator, Sandra Cid, there are no firearms or ammunition on the premises. (Continue to LIC809...) Resident record review included but not limited to physician report, functional capability, appraisal, admission, and hospice care plan. Caregiver record review included but not limited personnel record, criminal background clearance, health screening, CPR/First-aid certification, and training's to support the care needs of the residents. LPA Flores did not observe any health and safety concerns, and no deficiencies were issued during today's visit. Licensing Program Analyst (LPA) Sara Martinez conducted an unannounced annual required visit. LPA was granted entry and met with Administrator Mary Jane Laygo, who was informed of the purpose of the visit. At the time of the visit there was three (3) staff and nine (9) residents present. LPA conducted a tour of the interior and exterior, reviewed facility documents and conducted interviews. LPA observed the following: Physical plant, floors, windows, and doors were observed to be clean. Fixtures and furniture were in good repair were present. The outdoor area was observed to be free of hazards. Detergents, cleaning solutions, and sharp and dangerous objects were observed to be locked and inaccessible. The smoke detector and carbon monoxide was inspected by the fire department 11/20/2023 and was operational, and the hot water temperature met department requirements. Facility kitchen had the ability to prepare food in clean environment and possessed equipment in good working condition. LPA observed the facility met the required 2-day supply of perishable and 7-day supply of non-perishable foods. LPA reviewed two (2) staff files and training. All staff have the required personnel records on file and criminal record clearance and updated training along with CPR/First Aid. Nine (9) resident files were reviewed and possessed all required paperwork which included Admissions Agreement, Pre-Placement Appraisals, and Physician's Report. The listed administrator possesses a current administrator's certificate. Resident medication was centrally stored and locked in a cabinet located in the dining room. A refrigerator to centrally store medication was locked and located in the kitchen. LPA reviewed medications prescribed to the residents and found all medication with required labeling found to be in place. Facility contains infection control plan and emergency and disaster plan. All facility exits were clear from obstructions. LPA observed emergency supplies and first aid kit with all required items. Fire extinguishers were fully charged and inspected. No deficiencies were cited at the time of the visit.
