Type of visit: Required - 1 Year

On June 29, 2026, Licensing Program Analyst (LPA) Eboni Bentley conducted an unannounced required 1-Year annual visit using the CARE Inspection Tool. Upon arrival at the facility, LPA Bentley introduced self and was granted entry into the facility by staff, after stating the purpose of the visit. Administrator (AD) Ronald Wilkes was present and assisted with the inspection. Ronald Wilkes has an administrator certificate that expires on September 7, 2027. The facility is licensed to operate for six (6) non-ambulatory residents, with a Hospice waiver for four (4) residents. The building is a single-story home located in a residential neighborhood. It consists of the following: six (6) resident bedrooms, one (1) staff bedroom, three (3) full bathrooms, living room area, dining area, kitchen, an outdoor covered seating area, and an attached two car garage. There are currently five (5) resident on census. During the visit, LPA toured inside and outside of the physical plant with AD and the following was observed. There were no bodies of water or obstructions on the premises. All rooms were inspected. Beds and bedding supplies were in good condition, storage for each resident’s personal belongings was observed. Bed linens, comforters, and bath towels were adequately stocked at the time of visit. A comfortable temperature of 70 degrees F was maintained in the facility. Bathrooms were found to be clean and operational with water temperatures measured between 114 degrees F and 114.8 degrees F. The kitchen was observed clean with all appliances in working order. CONTINUE TO LIC 809-C.... The smoke alarms and carbon monoxide detectors were operable. First aid kit is maintained and contains all the necessary elements. Emergency safety drills were last conducted on April 22, 2026. The facility has one (1) fire extinguisher that was charged and last serviced on September 9, 2025. Emergency food, emergency water, or emergency supplies were observed. Current Liability Insurance was provided during the visit with a policy start date of July 11, 2025, through July 11, 2026. A working telephone (714)916-0529 remains available and the facility has a device that can be used for video teleconference purposes. LPA conducted an audit of five (5) resident files (R1-R5), four (4) staff files (S1-S4), and medication and medication administration review. Resident and staff interviews were conducted. Based on today's observations, no deficiencies are being cited. Type of visit: Required - 1 Year

On this day Licensing Program Analyst (LPA) Fred Arias made an unannounced visit to conduct a required annual inspection. LPA was greeted and granted entry into the facility by staff and explained the reason for the visit. Facility is licensed for 6 non-ambulatory residents. Facility has an approved hospice waiver for 4 residents and the home currently has 6 residents, with no residents on hospice. Administrator (AD) Ron Wilkes arrived shortly to assist with the inspection. Facility provided updated liability insurance that expires on 7/11/2026. LPA along with staff toured the facility at 9:00 AM. LPA toured the physical plant, checked food service, and facility documentation. The home consists of 6 resident bedrooms, staff room, living room, dining room, and kitchen as well as 3 bathrooms. Resident bedrooms had the required furniture, bed linens and closet/drawer space to accommodate each resident comfortably. Resident bathrooms were checked. Toilets and water faucets worked properly, grab bars were secure and shower was free of mold/mildew. Water temperature measured between 105.2 degrees F and 113.9 degrees F in all bathrooms. Resident bath towels, toiletries and personal hygiene supplies were adequately stocked. Common areas were clean and clear of hazards. Auditory exit alarms were operational during today's visit. LPA toured the kitchen and observed sharps locked in a drawer during today's visit. Perishable and non-perishable food supply was checked and adequately stocked at time of visit. Kitchen appliances were operational during today's visit. Smoke detectors tested operational during today's visit. Fire extinguishers were fully charged. LPA reviewed the infection control and emergency disaster plans and plans are complete and thorough. Facility conducts quarterly emergency drills with the last drill conducted on 10/12/2025. Outside grounds were toured. Walkways around the home were clear of hazards. There is shaded outdoor seating for residents. Exit gate is unlocked and operational. First aid kit contained all required items including tweezers, scissors and thermometer. Facility conducts activities in the form of exercise and games. LPA observed the emergency food and water supply. LPA reviewed six resident files and two staff files. All resident files contained required documentation including admission agreements, physician reports, resident appraisals, and physician orders for bed rails as indicated. Staff files reviewed contained required documentation including required annual training, medical assessment/ TB, criminal record clearance and proof of CPR training. LPA reviewed medication storage and administration. Medications are stored in a locked closet. Medications are being administered per physician order. Based on the observations made during today’s visit, no deficiencies are being cited per Title 22 Division 6 of the California Code of Regulations. Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Michael Tea conducted an unannounced visit. The purpose of today’s visit was to conduct the Annual Required inspection. At around 8:03 AM, LPA Tea was greeted and granted entry into the facility by caregiver Maria Jalbuena and explained the reason for the visit. The administrator (AD) Ronald Wilkes arrived shortly to assist with the visit. The facility is licensed for 6 non-ambulatory residents with a hospice waiver for four. Currently there are six residents, of which two residents are on hospice during today's visit. Around 8:25 AM, LPA Tea reviewed six resident files and two staff files. Resident files and staff files contained all required documentation. Upon review of records, the facility is up to date with required quarterly fire drill, which was last conducted in June 10, 2024. The administrator certificate expires on September 07, 2025. LPA Tea along with AD Wilkes toured the facility at 9:17 AM. LPA toured the physical plant, checked food service, and the first aid kit. The facility is a single-story home that consists of 6 resident bedrooms, 1 caregiver room, 3 bathrooms, living room, dining room, kitchen and a garage. LPA observed smoke detectors/carbon monoxide in common areas and bedrooms are operational. Resident bedrooms had the required furniture, bed linens and closet/drawer space to accommodate each resident comfortably. Resident bathrooms were checked. Toilets and water faucets worked properly, grab bars were secure, and shower was free of mold/mildew. Water temperature measured between 113.1 F degrees and 113.3 F degrees. Resident bath towels, toiletries and personal hygiene supplies were adequately stocked at time of visit. Common areas were clean and clear of hazards, doorways were free of obstructions. First aid kit had all the required elements including bandages, tweezers, thermometer, and scissors. Kitchen was inspected. Perishable and non-perishable food supply was checked and adequately stocked at time of visit. Annual continuation on LIC809-C LPA observed sharps locked in the dishwasher after being used in the morning, normally they are locked in the storage closet where medication and files are stored. LPA also observed toxin substances to be locked and inaccessible to clients in care underneath the kitchen sink. The fire extinguisher was fully charged in the dining area. Kitchen appliances are operational during today's visit. LPA toured the outside grounds and there is ample seating underneath a shaded patio area in the backyard. There is one exit gate that is self-latching and operational on the left side of the house. LPA observed PPE supplies, emergency food and water stored in storage closet by the front entry way. Facility provides activities based on their personal choice. Residents walk around the neighborhood and facility grounds. Musician comes plays during special occasions such as birthdays. There is a fitness instructor that comes weekly to do chair exercises and dancing. Occasionally students would bring their pets for pet therapy for residents to play with. The residents also color and play games and puzzles. At the time of the visit, LPA observed residents watching game shows on television. LPA reviewed medication storage and administration. Medications are stored in a locked storage closet by the front entryway. Medications are being administered per physician order. LPA interviewed residents regarding their quality of care and spoke to staff present regarding care provided. Based on the observations made during today’s visit, no deficiencies were noted today in the areas inspected per Title 22 Division 6 of the California Code of Regulations. This report was reviewed with Administrator Ronald Wilkes and a copy of this report LIC809, along with the 809-C, LIC858, LIC858-C, LIC859, and LIC9102TV were read and provided to the facility.the state’s words, verbatim · CDSS document, Sep 9, 2024
