Licensing Program Analyst (LPA) Nancy Guillen made an unannounced visit for the purpose of conducting a required annual Inspection. LPA was greeted and granted entry by care provider after explaining the purpose of the visit. Administrator (AD) Cotila Dahabreh was notified via telephone and later arrived to assist with the inspection. LPA observed the Administrator certificate was current and expires January 27, 2027. This is a Residential Care Facility for the Elderly (RCFE) licensed to six non-ambulatory residents with a hospice waiver for three. The facility is a one-story home with six resident bedrooms, one staff bedroom, four bathrooms, and an attached garage. During the inspection, LPA and AD conducted a tour of the inside and outside of the facility, common areas, resident rooms, kitchen, garage and observed the following: LPA observed residents washing dishes and watching television in the living room and respective bedrooms. LPA observed three residents in care and two staff present. LPA observed the See Something Say Something Poster (PUB 475) was not posted at the facility, the framed poster was brought from storage and placed back on the wall during visit. All resident bedrooms had the required furnishings. LPA observed all resident beds had linens and blankets with additional linens stored in the hallway storage closet. LPA observed bathrooms were clean and equipped with grab bars and non skid floor mats. LPA observed all windows were appropriately screened. Bathrooms were observed to be free of debris and mildew. Faucets and toilets were operational. Water temperature tested between 125.7 and 130.6 degrees Fahrenheit. LPA toured the outside of the facility and observed spray paint, paint, sealer and pruning shears in unlocked back patio storage; items were removed and placed in garage during visit. LPA observed the backyard had a shaded sitting area with furniture for resident use. Continued on LIC 809C LPA observed the facility had a 2-day supply of perishables and a 7-day supply of non-perishable food as required by regulations. Smoke detectors and carbon monoxide detectors tested operational. Fire extinguisher was observed to be fully charged with a purchase date of February 07, 2026 and located by the kitchen. Medication cabinet was observed to be locked and centrally stored in living room closet however, LPA observed medication Tussin DM Max, Ferrous Sulf, Milk of Magnesia, Ferrous Sulfate Elixir, Dayquil, and Mucinex in kitchen pantry accessible to residents in care. LPA observed the First Aid Kit had all the required components. LPA observed the facility conducted their last emergency disaster drill on March 23, 2026. LPA reviewed two resident records. All the required documentation were present and current in the residents’ files reviewed. LPA reviewed two employee records. All employee’s present have a criminal record clearance and were associated to the facility. LPA observed records reviewed have a current First Aid certificate. Based on the observations made during today’s inspection, deficiencies are being cited per Title 22 Division 6 of the California Code of Regulations. Allegation investigated: Toxins and medications are accessible to residents in care.

On this day, Licensing Program Analyst (LPA) Fred Arias made an unannounced visit to the facility for the purpose of continuing the investigation of the allegation listed above. LPA was greeted and granted entry into the facility by staff and explained the reason for the visit. An initial investigation visit was conducted on August 24, 2022 by LPA Jenifer Tirre. During the visit, LPA Tirre accompanied by staff conducted a tour of the physical plant. LPA Tirre conducted interviews, reviewed files and requested pertinent documents. The investigation determined as follows: regarding the allegation toxins and medications are accessible to residents in care, it was reported that toxins and disinfectants are located under the kitchen sink and unlocked, accessible to residents in care. It was revealed during the initial investigation on August 24, 2022, LPA Tirre observed bleach and other toxins under the kitchen sink unlocked and accessible to residents. LPA Tirre also observed medication in an unlocked cabinet in the kitchen. During the visit, four out of six residents were diagnosed with dementia. Continued on 9099-C dated 5/28/2025 Substantiated On this day, LPA Arias toured the physical plant and did not observe any toxins under the kitchen sink. LPA Arias observed medications locked in a cabinet in the hallway. LPA Arias did not observe medications anywhere outside the medication cabinet. LPA observed cleaning supplies stored in the garage. Based on interviews conducted, observation, and record review, the preponderance of evidence standard has been met. Therefore, the above allegation is found to be SUBSTANTIATED. California Code of Regulations, (Title 22, Division 6, Chapter 8), are being cited on the attached LIC 9099D. An exit interview was conducted with Colitia Dahabreh and a copy of the report was left with the facility representative along with appeal rights. Regarding the allegation facility is not in good repair, it was reported the kitchen sink leaks and not fixed. During the initial investigation by LPA Tirre, LPA Tirre did not observe any leaks under the kitchen sink. On this day, during the physical plant tour, LPA Arias did not observe any leaks under the kitchen sink. LPA found the facility to be in good repair. Regarding the allegation facility did not ensure that food is of good quality, it was reported facility does not cook for the residents and are served leftover meals. During the initial investigation by LPA Tirre, LPA Tirre observed adequate perishable and non-perishable food of good quality. On this day, LPA Arias observed adequate perishable and non-perishable food of good quality stored in two separate refrigerators, kitchen cabinets, and garage. LPA Arias observed staff preparing meals for the residents during the investigation. Regarding the allegation staff do not give residents the correct, prescribed medications, it was reported staff do not follow physician's orders. On this day, LPA Arias audited medications for two out of five residents along with staff. The facility keeps a record of centrally stored medications for every month and every resident. Medications audited were a mix of bottles and bubble packs. All medications were accounted for and bubble pack numbers aligned with the calendar days. LPA Arias interviewed staff with regards to medication administration. Staff revealed a refill order is placed about 5 days prior to depleting a medication. LPA observed discontinued orders are updated directly on the centrally stored medications record. Based on the information gathered during the investigation, interviews and review of documents obtained the Department is unable to ascertain if the allegations occurred as reported. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove or refute the alleged violations occurred; therefore, these allegations are deemed UNSUBSTANTIATED. Type of visit: Required - 1 Year

On this day Licensing Program Analyst (LPA) Fred Arias made an unannounced visit to conduct a required annual visit. LPA was greeted and granted entry into the facility by staff and explained the reason for the visit. Facility is licensed for 6 non-ambulatory residents. Facility has an approved hospice waiver for 3 residents and the home currently has 5 residents, with 3 residents on hospice. Administrator (AD) Cotilia Dahabreh arrived shortly to help conduct facility tour. AD provided updated liability insurance that expires on 3/13/2026. LPA along with staff toured the facility at 9:10 AM. LPA toured the physical plant, checked food service, and facility documentation. The home consists of 6 resident bedrooms, staff room, living room, dining room, and kitchen as well as 4 bathrooms. Resident bedrooms had the required furniture, bed linens and closet/drawer space to accommodate each resident comfortably. Resident bathrooms were checked. Toilets and water faucets worked properly, grab bars were secure and shower was free of mold/mildew. Water temperature measured between 115.8 degrees F and 119.8 degrees F in all bathrooms. Resident bath towels, toiletries and personal hygiene supplies were adequately stocked. Common areas were clean and clear of hazards. Auditory exit alarms were operational during today's visit. LPA toured the kitchen and observed sharps locked in a cabinet during today's visit. Perishable and non-perishable food supply was checked and adequately stocked at time of visit. Kitchen appliances were operational during today's visit. Smoke detectors tested operational during today's visit. Outside grounds were toured. Walkways around the home were clear of hazards. There is shaded outdoor seating for residents. Exit gates are unlocked and operational. LPA observed the emergency food and water supply. Fire extinguisher was fully charged. LPA reviewed the infection control and emergency disaster plans. Facility did not provide a record of disaster drills. Continued on LIC809-C dated 5/28/2025. LPA reviewed five resident files and two staff files. All resident files contained required documentation including admission agreements, physician reports, and resident appraisals. Staff files reviewed contained required documentation including required annual training, medical assessment/ TB, criminal record clearance and proof of CPR training. LPA reviewed medication storage and administration. Medications are stored in a locked cabinet. Based on the observations made during today’s visit, a deficiency is being cited per Title 22 Division 6 of the California Code of Regulations. On 8/29/2024, Licensing Program Analyst (LPA) Janette Romero conducted an unannounced visit to the facility for a required annual inspection. LPA was greeted and granted entry by Caregiver, Floare Caisley who was informed of the purpose of the visit. Administrator, Cotilia Dahabreh arrived during the visit and was also informed of the purpose of LPA's visit. The facility has a fire clearance for six (6) non-ambulatory elderly residents and an approved hospice waiver for three (3). LPA was informed there are three (3) residents currently receiving hospice services at the facility. LPA toured the facility with Administrator Dahabreh and observed the facility is made up of a one-story home with six (6) resident bedrooms, four (4) bathrooms, a staff rooms, kitchen, dining room, living room and attached garage. During the tour, Administrator Dahabreh tested one (1) of the smoke alarms/carbon monoxide detectors and LPA observed it to be operational. LPA observed a charged fire extinguisher mounted near the living room, purchased on 3/28/2024. Indoor and outdoor passageways were free of obstruction. The facility has shaded seating for the residents in care. There were no bodies of water observed on the premises. Medications are secured in a locked closet. Resident bedrooms had the required furniture and lighting. Resident bathrooms had grab bars near the toilets and in the showers. The facility had more than a 2-day supply of perishable foods and 7-day supply of non-perishable food items. Knives are secured in a locked kitchen drawer. Cleaning solutions and disinfectants are secured in the locked garage. LPA reviewed random resident and staff files. Resident files reviewed had signed admission agreements and residents with a dementia diagnosis had updated physician's reports. Staff present has a criminal record clearance and a valid first aid/CPR certification. During today's visit, LPA did not observe any issues or concerns.
