On June 30, 2026, Licensing Program Analyst (LPA) Eboni Bentley conducted an unannounced required 1-Year annual visit using the CARE Inspection Tool. Upon arrival at the facility, LPA Bentley introduced self and was granted entry into the facility by staff, after stating the purpose of the visit. Administrator (AD) Ronald Wilkes was present and assisted with the inspection. Ronald Wilkes has an administrator certificate that expires on September 7, 2027. The facility is licensed to operate for six (6) non-ambulatory residents, with a Hospice waiver for four (4) residents. The building is a single-story home located in a residential neighborhood. It consists of the following: five (5) resident bedrooms, one (1) staff bedroom, three (3) full bathrooms, living room area, dining area, kitchen, an outdoor covered seating area, and an attached two car garage. There are currently six (6) resident on census. During the visit, LPA toured inside and outside of the physical plant with AD and the following was observed: There were no bodies of water or obstructions on the premises. All rooms were inspected. Beds and bedding supplies were in good condition, storage for each resident’s personal belongings was observed. Bed linens, comforters, and bath towels were adequately stocked at the time of visit. A comfortable temperature of 74 degrees F was maintained in the facility. Bathrooms were found to be clean and operational with water temperatures measured between 112.2 degrees F and 116 degrees F. The kitchen was observed clean with all appliances in working order. CONTINUE TO LIC 809-C.... The smoke alarms and carbon monoxide detectors were operable. First aid kit is maintained and contains all the necessary elements. Emergency safety drills were last conducted on April 11, 2026. The facility has one (1) fire extinguisher that was charged and last serviced on September 9, 2025. Emergency food, emergency water, or emergency supplies were observed. Current Liability Insurance was provided during the visit with a policy start date of July 11, 2025, through July 11, 2026. A working telephone (714)840-6848 remains available and the facility has a device that can be used for video teleconference purposes. LPA conducted an audit of six (6) resident files (R1-R6), four (4) staff files (S1-S4), and medication and medication administration review. Resident and staff interviews were conducted. Based on today's observations, no deficiencies are being cited. On this day Licensing Program Analyst (LPA) Fred Arias made an unannounced visit to conduct a required annual visit. LPA was greeted and granted entry into the facility by staff and explained the reason for the visit. Facility is licensed for 6 non-ambulatory residents. Facility has an approved hospice waiver for 4 residents and the home currently has 6 residents, with 2 residents on hospice. Administrator (AD) Ron Wilkes arrived shortly to assist with the visit. LPA along with staff toured the facility at 10:40 AM. LPA toured the physical plant, checked food service, and facility documentation. The home consists of 5 resident bedrooms, staff room, living room, dining room, kitchen, 4 bathrooms, along with an attached garage. Resident bedrooms had the required furniture, bed linens and closet/drawer space to accommodate each resident comfortably. Resident bathrooms were checked. Toilets and water faucets worked properly, grab bars were secure and shower was free of mold/mildew. Water temperature measured between 115.5 degrees F and 117.8 degrees F in all bathrooms. Resident bath towels, toiletries and personal hygiene supplies were adequately stocked. Common areas were clean and clear of hazards. Auditory exit alarms were operational during today's visit. Perishable and non-perishable food supply was checked and adequately stocked at time of visit. Kitchen appliances were operational during today's visit. Smoke detectors tested operational during today's visit. Fire extinguishers were fully charged. LPA reviewed the infection control and emergency disaster plans and plans are complete and thorough. Facility conducts quarterly emergency drills. Outside grounds were toured. Walkways around the home were clear of hazards. There is shaded outdoor seating for residents. Exit gates are unlocked and operational. LPA observed the emergency food and water supply stored in the garage. LPA observed sharps stored in a locked cabinet in the garage. First aid kit contained all required items including tweezers, scissors and thermometer. Facility conducts activities in the form of exercise. LPA reviewed six resident files and two staff files. Continued on LIC809-C dated 11/25/2025 All resident files contained required documentation including admission agreements, physician reports, and resident appraisals. Staff files reviewed contained required documentation including required annual training, medical assessment, criminal record clearance and proof of CPR training. LPA reviewed medication storage and administration. Medications are stored in a locked cabinet. Medications are being administered per physician order. Based on the observations made during today’s visit, no deficiencies are being cited per Title 22 Division 6 of the California Code of Regulations. Licensing Program Analyst (LPA) Michael Tea conducted an unannounced visit. The purpose of today’s visit was to conduct the Annual Required inspection. At around 1:03 PM, LPA Tea was greeted and granted entry into the facility by caregivers, Mary Elizabeth Parke and Mary Ann Lawson and explained the reason for the visit. The administrator (AD) Ronald Wilkes arrived shortly to assist with the visit. The facility is licensed for six non-ambulatory residents with a hospice waiver for four. Currently there are six residents, of which two are on hospice during today's visit. Around 1:35 PM, LPA Tea reviewed six resident files and two staff files. Resident files and staff files contained all required documentation. Upon review of records, the facility is up to date with required quarterly emergency disaster drills, which was last conducted in June 19, 2024. AD Wilkes administrator certificate is current and expires September 07, 2025. LPA Tea along with House Manager Sharon Pajarillaga and AD Wilkes toured the facility at 2:30 PM. LPA toured the physical plant, checked food service, and the first aid kit. The facility is a single-story home, that consists of 5 resident bedrooms, 1 staff room, 3 bathrooms, living room, dining room, kitchen and an attached garage. LPA observed smoke detectors/carbon monoxide in common areas and bedrooms are operational. Resident bedrooms had the required furniture, bed linens and closet/drawer space to accommodate each resident comfortably. Resident bathrooms were checked. Toilets and water faucets worked properly, grab bars were secure, and showers were free of mold/mildew. Water temperature measured around 113.1 F degrees. Resident bath towels, toiletries and personal hygiene supplies were adequately stocked at time of visit. Common areas were clean and clear of hazards, doorways were free of obstructions. First aid kit had all the required elements including bandages, tweezers, thermometer, and scissors. Kitchen was inspected. Perishable and non-perishable food supply was checked and adequately stocked at time of visit. LPA observed sharps locked and inaccessible to residents in a cabinet in the garage. LPA also observed toxin substances secured in the same cabinet in the garage. Annual continuation on LIC809-C The fire extinguisher in the kitchen is fully charged and was serviced on September 10, 2024. Kitchen appliances are operational during today's visit. LPA toured the outside grounds and there is ample seating underneath a shaded patio area in the backyard. The resident and their visitors sometimes hang out in the backyard. There are two exit gates that are self-latching and operational on both sides of the house. LPA observed PPE supplies, emergency food and water stored in the garage. Facility provides activities based on their personal choice. Residents like to go for walks around the neighborhood and inside the facility. They play bingo, color, read magazines or books. A fitness instructor comes to the facility to have residents do exercises and dance. High school volunteers bring their pets for pet therapy for the residents. At the time of the visit, LPA observed resident eating lunch and lounging in the living room watching television. At 2:47 PM LPA reviewed medication storage and administration. Medications are stored in a locked cabinet in the garage. Medications are being administered per physician order. LPA interviewed residents regarding their quality of care and spoke to staff present regarding care provided. Based on the observation made during today’s visit, no deficiencies were noted today in the areas inspected per Title 22 Division 6 of the California Code of Regulations.
