May 14, 2026Facility evaluation reportReport on file
Type of visit: Required - 1 Year
On May 14, 2026, Licensing Program Analyst (LPA) Kayla Adkison, arrived at the facility unannounced to conduct a Required Annual Inspection. LPA met with Chong Xiong, Caregiver, and explained the purpose of the visit. During the inspection, there were 2 (two) residents and 2 (two) staff providing direct care. Licensee, Tamra Leak, arrived approximately 20 minutes later and joined the inspection. LPA and staff toured the facility together to ensure the health and safety of residents in care. Areas toured include but are not limited to: common areas, four (4) resident bedrooms, two (2) bathrooms, kitchen, storage areas, and backyard. All areas observed were found to be clean and in good repair. All walkways were clear of obstructions. LPA observed several furniture items in the backyard that the licensee will be disposing of by the end of the month. LPA observed each bedroom to have the required furnishings and working lights. LPA observed the facility to be at a comfortable temperature. There were various activities available for client recreation including games and crafts. Facility has a 2-day perishable and a 7-day non-perishable amount of food. Clients participate in the creation of the weekly menu. LPA observed all medications, sharps, and cleaning supplies to be locked away and inaccessible to clients in care. LPA reviewed Medication Administration Records (MARs) and found them to be current and accurate. LPA observed (2) two fire extinguishers which were last inspected in January 2026. Smoke detectors and carbon monoxide detectors were observed and found to be in working condition. The facility is conducting emergency disaster drills monthly with the last drill documented in April 2026. LPA observed a complete first aid kit ready for use. Continued on LIC 809-C In the areas toured no immediate health, safety, or personal rights violations were observed. LPA reviewed a total of four (4) resident files and four (4) staff files which contained all required documentation. All staff are fingerprint cleared and properly associated to the facility. Administrator certificate is current. No deficiencies are being cited as a result of this inspection. Exit interview conducted. Type of visit: Prelicensing
On June 11, 2025, at approximately 10:00 AM, Licensing Program Analyst (LPA) Kayla Adkison arrived at the facility to conduct a scheduled pre-licensing inspection. LPA met with licensee, Tamra Leak, and explained the purpose of the visit. Licensee has waived Comp 3 due to running multiple facilities. LPA toured the facility inside and out. The inside of the facility was observed to be clean and in good repair. The facility has received fire clearance for four (4) residents, all of which may be non-ambulatory. The facility has four (4) client bedrooms, and two (2) bathrooms. LPA observed a kitchen table with seating for four in the dining room. LPA observed the living room to have ample seating for clients. Food storage meets Title 22 regulation requirements. Plates, utensils, pots, and pans were in place during the inspection. Dishwasher, stove, microwave, and refrigerator were all present and working. LPA observed (2) two fully charged fire extinguishers to be present and operating. LPA observed smoke alarms and carbon monoxide detectors fully functioning. First aid kit is stocked and available for use. Continued on LIC809-C Bedrooms were observed to have furniture as required by Title 22 Regulations. All beds were made up with linens and bedspreads. The facility has ample amount of extra bedding and linens. Bathrooms were observed to be in good repair with sturdy grab bars available for use. Storage and lighting are adequate in the home. Medications are to be stored in a locked cabinet. Cleaning supplies and toxins are to be locked in a spare room attached to the laundry room. Knives are locked up in the kitchen. Washer and dryer observed in place and ready for use. There is an office space where client and staff files will be stored. The backyard has a covered area for client use. There are no pools or bodies of water on the premises. The applicant has passed the pre-licensing portion of the application process. LPA will contact the Central Application Bureau.
