Type of visit: Required - 1 Year

On 07/31/2025 at 12:00 PM, Licensing Program Analyst (LPA) Jose Calderon conducted an unannounced annual inspection visit at the Magical Touch Care Home Facility. LPA Calderon was allowed entry into the facility by Administrator Maria Matias. The facility is licensed to operate for six (6) non-ambulatory residents with disabilities. Currently there are 5 residents in care living at the facility. LPA Calderon explained to Administrator Maria Matias, the purpose of the one-year Annual Inspection visit, and escorted LPA Calderon on a tour of the entire inside and outside facility grounds. As part of the inspection, LPA Calderon reviewed: Five (5) client service records, five (5) client medication records, two (2) staff records, and inspected the inside facility and outside grounds. The facilities’ last fire drill was conducted on 07/16/2025. The one-story residential home consists of four (4) client bedrooms, two (2) client bathrooms, living room, dining room, kitchen, staff room, office area, garage with washer and dryer/ storage area, backyard with table and chairs. No weapons are stored on the premises. The kitchen was inspected and observed to be clean and operational. A two-day supply perishable and seven-day supply of non-perishable foods are present in the facility. Emergency Water Storage is in the garage and kitchen area. LPA Calderon observed that all facility rooms are clean and in good repair. A comfortable temperature was observed, and the facility has central air and heating. LPA Calderon observed the following during inspection of client’s rooms: mattresses are in good condition, adequate lighting present, plenty of dresser/closet space is present, and all bed linens present. All bedrooms contain furniture, lighting fixtures and personal storage space as required, all beds have the required amount of linen and mattress covers, LPA Calderon observed fully stocked closet with bedding, towels, and toiletries supplies. Bathroom fixtures are clean, in good repair, and working properly and contain the required nonskid mats and grab bars. LPA Calderon observed bathrooms were found to be within Title 22 regulation. Bathroom #1 hot water temperature measured at 108 degrees Fahrenheit, and bathroom #2 hot water temperature measured at 110 degrees Fahrenheit. Kitchen hot water temperature is properly measured at 115 degrees Fahrenheit. Facility (1) Carbon Monoxide and (5) Smoke Detectors are hard-wired and connected were tested and are working properly. The facility (1) Fire Extinguishers were checked and found to be fully charged and accessible. All exit doors in the facility have alarm systems. All toxins and knives are locked/secured and inaccessible to clients. Medications are centrally stored and in a locked storage cabinet. Facility first aid kit is fully stocked with manual was checked and in order. Outside grounds were toured and no bodies of water were observed. All Exits/ Walkways around the home were free of debris and hazards. Outside patio accessible to clients. Five (5) client files were reviewed and found to be complete. LPA Calderon reviewed five (5) resident medications, and they were all found to be administered according to doctor's orders. Two (2) staff files were checked and have the required documents. LPA Calderon noted the Administrator Maria Matias Certification # 7000353740 expiration date of 10/03/2025 was valid at time of visit. Commercial General Liability Policy #001561111 policy period from 05/25/2024 to 06/26/2026 underwritten by James River Insurance Company, coverage 1,000,000/3,000,000 is valid at time of inspection. Administrator Maria Matias emailed LPA Calderon a full copy of the commercial insurance policy including all endorsements no later than 08/10/2025. All the required documents are posted in the facility in a clearly visible area. According to the California Code of Regulations (Title 22, Division 6, Chapter 8), LPA Calderon did not observe deficiencies therefore no citations were issued at this time. Annual Licensing Fee is CURRENT. Type of visit: Prelicensing

On 06/06/2024 around 9am Licensing Program Analysts (LPA) Jose Calderon conducted an announced face to face visit with Licensee Maria Matias for purpose of a pre-licensing evaluation for Magical Touch Care Home. The requested capacity is for 6 adult residents of which there is 1 bedridden/ 5 non-ambulatory Adult Residents. Facility is a 4 bedroom, 2 bathrooms, one-story house. The client bedrooms are spacious and will easily accommodate the client's furnishings. There is a backyard with a covered patio for shade. The patio contained 1 small table and 3 chairs. Outdoor passageways, walkways, driveways, steps, and patios are free from obstructions. LPA Calderon did not observe hazards, such as ladders, gardening tools and/or motorized equipment in the front, back and/or side areas of the facility. Residents Bedrooms: All 4 Bedrooms are for ambulatory/non-ambulatory clients. Bedroom’s #1 and #2, #3 and #3 has one bed each, one chair, one-night stand, one lamp. There are dressers within the closet for each resident. Bedrooms #1, #2 #3 and #4 all comply with the requirement of 8 cubic feet of space. Bathroom 2: Have a working toilet, wash basins, and walk-in shower. Bathroom #1 shower is missing grab bars, but licensee states bathroom #1 is for ambulatory only residents. LPA Calderon observed adequate lighting in hallway leading to bathrooms. Linens & Hygiene Supplies: Beds have the required linen/supplies which include, pillowcase, mattress pads, fitted sheet, blanket and bedspreads. Adequate supply of linen stored in hall closet. Emergency Phone Numbers, Exit Plan & Menu: The telephone system is a land line and operable. Emergency Disaster Plan and "See something, say something Let Us Know" was noted. Fire Extinguisher 1 mounted on the wall in the kitchen. Food Service: Dishes, cups and flat ware are stored in the kitchen cupboards, inspected and in good repair. Knives, cutlery, and other sharp kitchen utensils are stored in a locked cabinet located in the kitchen area. Food supply is adequate; 7 days of dry food was found in the kitchen pantry which consisted of dry pasta, canned foods. Facility did not have 7 days of emergency water. Smoke Detectors: 5 smoke detectors are battery operated & working. Carbon monoxide detector 1 located and mounted in the hallway is operational. Appliances: Gas Stove, oven, microwave, washer, and dryer working. Refrigerator in the kitchen has a measured temperature of at least 41 degrees Fahrenheit for appropriate food storage. Freezer is at 19 degrees Fahrenheit. The residence is equipped with central air and heat and each client bedroom is individually climate controlled. Toxins: Locked/stored in the storage room located in the kitchen. Water Temperature: Bathrooms water temperature tested in #1 at 109 F. and #2 110 F. degrees, kitchen sink temperature tested at 115 F degrees. Medications, First-Aid Kit & Book: Medication administration records storage area, and first aid kit has been inspected, which are stored in locked kitchen cabinet adjacent the refrigerator, available for staff use but inaccessible to clients. Reading Material, Games, Equipment & Materials: The facility has board games, books, and other recreational materials for the client's use. LPA Calderon did not observe any pet or bodies of water at the facility. LPA Calderon observe delayed egress, no chain locks, or dead bolts on exits. LPA Calderon did not observe pad locks or other mechanisms which may be obstructions for safe and quick egress during an emergency on side gates and front exits. Pool/Jacuzzi & Pets: LPA Calderon did not observe any pet or bodies of water at the facility. Fire clearance: Fire Clearance was approved on 02/16/2024 for 1 bedridden/5 non-ambulatory clients with no special instructions. LPA Calderon did observe delayed egress, no chain locks, or dead bolts on exits. LPA Calderon did not observe pad locks or other mechanisms which may be obstructions for safe and quick egress during an emergency on side gates and front exits. Component III: (05/30/2024) about how to operate the facility within substantial compliance was reviewed by the licensee at this time. An exit interview was conducted, and a copy of this report has been furnished to the applicant Maria Matias by hand. Accordingly, LPA Jose Calderon will submit a copy of this facility evaluation report to the Central Applications Bureau (CAB) for review.
