On 03/25/2026, Licensing Program Analyst (LPA) V Gorban arrived to the facility unannounced to conduct the required annual inspection visit. LPA was greeted by staff Allys G, stated the purpose of the visit and were allowed entry into the facility. Administrator on record is Lindsey Lamerson, certification number 7009987740 and expiration date 02/05/2027. LPA toured the facility inside and out and observed six residents in the home at the time of visit, three residents are receiving Hospice services or Home Health care services. The facility temperature read at 74 degrees F. Resident bedrooms were observed to have the required lighting/furnishings and are free from odor and passageway obstruction/fire hazards. Bathrooms were observed to have operational lights, running water, and non- slip floors. Hot water temperature tested at 105 degrees F. Cleaning supplies were observed to be locked cabinet located in the laundry room. LPA toured the kitchen observed the required 7-day supply of non-perishable food and 2- day supply of fresh perishables to be properly stored. Carbon monoxide detectors were observed to be operational. A fire extinguisher was observed with a service date of 01/07/2026, disaster drill recorded on 03/02/2026. First aid kits were observed to contain all required items. Medications were observed to be locked cabinet located in the dinning room. Required postings were observed. A sample of resident and staff files were reviewed and documented. Plan of Operations, Disaster Plan and Infection Control plans observed to be complete. On 03/18/2026 Licensing Program Analyst (LPA) Gorban unannounced visited the facility regarding complaint investigation. LPA introduced self and met with house manger Italy Woods. LPA stated purpose of the visit and was allowed entry. Administrator was notified of Licensing visit. During the complaint investigation LPA toured the facility conducting health and safety checks, reviewed records, and conducted interviews. Allegation: Staff do not follow resident's physician's order. Based on interviews and records facility files up to date with staff familiar to provide care to each individual resident. Allegation: Staff are not properly trained. Based on interviews and records reviews staff completed annual training as required. Although the allegations may have happened or are valid, there are not a preponderance of evidence to prove the alleged violations did occur, therefore the allegations are Unsubstantiated. On 03/26/2025, Licensing Program Analyst (LPA) V. Gorban arrived unannounced at the above facility to conduct an Annual Inspection. LPA introduced self, stated the purpose of the visit, and was granted entry to the facility by staff Valerie Ramos. Administrator Lindsey Lamerson was notified of the visit, but was not able to attend the visit. House manager Italy Woods assisted LPA with inspection. During the tour LPA observed facility to be at a comfortable temperature, of 75 degrees F. Facility is free of debris, in good repair, and no passageway obstructions or fire hazards were observed. Common areas were properly furnished and well-lit throughout. LPA observed some residents in common area after lunch, others in their rooms resting. Department phone number and infection prevention information signs were posted thought the facility. Inspecting kitchen LPA observed the required 7-day supply of non-perishable food and 2- day supply of fresh perishables to be properly stored. An emergency disaster supply was observed. Groceries purchased on weekly bases by house manager. Fire extinguisher was observed with a service date of 12/02/2024. Last fire drill recorded on 02/19/2025. All 6 residents’ bedrooms were observed to be with comfortable temperature. Bathroom water temperature was tested and recorded reading of 107 degrees F. No deficiencies were observed or cited during this visit. On 4/23/2024, Licensing Program Analyst(LPA) D. Ayers arrived unannounced to conduct a Required Annual inspection. LPA met with Licensee/Administrator Lindsey Lamerson and announced the purpose of the inspection. Administrator certificate is current with renewal date 2/5/2025. The facility has a hospice waiver for three residents, and there was one resident receiving hospice services at the time of inspection. LPA toured the facility inside and outside. All passageways and exits were clear and free from obstruction. Fire extinguisher was recently serviced. Smoke and carbon monoxide detectors were present and operational. Outdoor area was free from hazard and had enough seating for residents in a covered area. The fence gate had a self-latching mechanism. LPA reviewed emergency disaster plan and record of emergency drills. There was an adequate supply of perishable and nonperishable foodstuffs. There was an adequate supply of emergency food and personal protective equipment. Medications were locked in a cabinet and appeared to be administered properly. Sharp items and detergents were secured and inaccessible to residents. LPA toured bedrooms and bathrooms. Bedrooms were clean, odor free, and had required minimum furnishings. Bathrooms were clean and serviceable, and had required secure garb bars and non-skid mats. Facility had a extra towels, linens, and personal hygiene items for residents. Resident and staff files were reviewed and contained all required records. No deficiencies were cited during the inspection.
