Allegation investigated: Facility has a locked gate that was not approved by the fire department/CCLD

At 10:00am, Licensing Program Analyst (LPA), Angela Panushkina conducted an unannounced visit in response to the above-mentioned allegation. LPA met with the Administrator and explained the reason for the visit. At 10:10am, LPA requested resident and staff roster. At approximately 10:15am, LPA conducted a physical plant tour. Between 10:20am - 11:00pm, LPA conducted an interview with the Administrator and two (2) staff members. It was alleged that the “Facility has a locked gate that was not approved by the fire department/CCLD”. On 10/13/25 two (2) credible witnesses conducted a visit to this facility and observed that the gate around the front yard of the facility, where visitors coming in need to be let in. Upon leaving the facility, they noticed a keypad to exit the front gate of the facility, and the door would not open without a code. Continue on LIC9099-C Substantiated On 11/10/25, Witness 1 (W1) contacted the Administrator (after several attempts) to address the concerns of the locked facility as a restraint and fire hazard for the residents that in case of emergency, they may not be able to exit the facility. Administrator informed W1 that the gate is battery powered and is not affected by electricity. W1 was also informed that licensing had previously raised concern, and the Administrator contacted the fire department to assist them with getting a new lock and security for the facility gate that fits guidelines. Furthermore, on 12/01/25, W1 conducted a subsequent visit to the facility, and the keypad was still present with no ability to leave the facility without being buzzed out by staff or inputting the code. W2 contacted the Los Angeles Fire Department (LAFD) Inspector, Linsay Pelligrini (on 12/12/25), who confirmed that when she approved this location, there was not a keypad lock and that this keypad is a violation of the state Fire Marshall code. Lastly, LPA Panushkina conducted review of facility file and observed that on 10/20/25, the Administrator emailed a request to Community Care Licensing Division (CCLD) for an approval to keep the front door locked. However, LPA Rahimi, replied to the email (on 10/21/25 at 11:10am) by informing the Administrator that a Fire Department must be contacted for such approvals. During today’s visit, LPA Panushkina also observed that the front gate is locked, and the Fire Clearance remains the same (last approved on 03/29/24). Therefore, based on the information obtained from the credible witnesses, facility file review and LPAs observation, this allegation is Substantiated. Fire Safety: All facilities shall be maintained in conformity with the regulations adopted by the State Fire Marshal for the protection of life and property against fire and panic. This requirement is not met as evidenced by: Based on LPA observaton, Licensee did not comply with the section cited above by keeping the exterior gate locked. Facility does not have a fire clearance or documentation from the fire department permitting for the use of this type of lock. Plan of correction: Licensee will remove or replace the lock to insure gate has easy access to enter or exit during emergencies. Proof via photo will be submitted to CCL by POC date. This is a zero-tolerance citation and immediate civil penalty of $500 shall be assessed this day. Additional 100 dollars will be assessed until POC is received.

At 9:45 AM, Licensing Program Analyst (LPA) Huma Rahimi, conducted an unannounced annual inspection at the facility mentioned above. LPA met with the staff Dinara Kundakpayeva and the staff contacted the Administrator Vahagn Petrosyan. LPA explained the reason for the visit. Physical tour was conducted with the Administrator LPA observed the following: KITCHEN: The facility has a kitchen area that is equipped with a refrigerator, microwave oven and sink. LPA observed enough supplies of staple non-perishable for minimum 1 week and perishable for 2 days at the facility. Knives and sharps were observed to be locked in a box in the kitchen drawer and inaccessible. MEDICATION: LPA observed that the medication and facility staff/resident files are locked in a cabinet located in the living room. During the medication review, LPA did not observe any discrepancies. BEDROOMS: There are six (6) bedrooms designated for residents’ private use. All bedrooms are furnished and well equipped with beds, night-stand, chair, dresser, bedding, and extra linen. Rooms were observed to have sufficient lighting and closet space. Auditory alarms were tested and observed to be operational at 10:15 AM. Facility has an awake staff. BATHROOMS: The facility has six (6) bathrooms, bathrooms #3 and #4 are designated for staff and residents’ family members use. All bathrooms were observed to have the proper fixtures, grab bars, and non-skid mats. The hot water delivered in the bathrooms measured at 10:00 AM to be at 118.4°F degrees. Continue on LIC809-C SMOKE DETECTORS/CARBON MONOXIDE. The smoke detectors and carbon monoxide are hard wired, inter-connected and were located throughout the facility. At 10:20 AM they were tested and observed to be operational. The facility has a fire extinguisher that was purchased on 03/25/2025. The fire extinguisher is located in the kitchen area. COMMON AREAS: The facility maintains a comfortable temperature at 75°F. The living room and dining appeared clean and were properly furnished. No obstructions and or tripping hazards throughout the facility. LPA observed two supplements in a drawer in the living room. Laundry: LPA observed laundry room located in a closet in the hallway by the kitchen. LPA observed an additional storage area for laundry supplies and other chemicals which was observed locked and inaccessible to residents in care. SURROUNDING GROUNDS: In the back of the facility has sufficient yard space. LPA observed appropriate outdoor furniture, with covered shaded area for residents. The backyard is fenced. There are no bodies of water. GARAGE: The garage is currently being used for storage and additional supplies for residents. LPA observed the garage locked and not accessible to residents in care. Between 10:45 AM to 11:45 AM, LPA reviewed records of five (5) residents and two (2) staff. Residents and staff records appeared to be complete and updated. Administrative: LPA collected Certificate of Liability Insurance, and LIC500. No deficiencies cited during today's visit. Type of visit: Prelicensing

On 7/17/2024 at 9:30AM Licensing Program Analyst (LPA), Perchui Milena Khurshudyan conducted an announced Pre-Licensing visit to the above facility and met with facility Licensee Yegeyan Nazar and Administrator Elen Bareghamyan. Fire Clearance was approved on 04/02/2024 for a maximum capacity of six (6) Non-ambulatory residents, of which one (1) can be Bedridden resident. Facility has Dementia Care Program. The purpose of today’s visit is to inspect the facility to ensure that the facility is in compliance with rules and regulations under California Code of Regulations, Title 22, Division 6. The facility is a single-story building. Today's site visit consisted of LPA touring the physical plant inside and outside and observed the following: KITCHEN: The facility has a Kitchen area that is equipped with a new refrigerator, microwave oven and sink. At 9:45AM, LPA observed adequate supplies of nonperishable food properly stored in the cabinets and inside the supply room. Perishable food items are not required at this time as there are no residents in the facility. Licensee will supply sufficient amount of perishable food upon arrival of the residents. LPA observed dining ware to accommodate a maximum capacity of six (6). All knives and sharps are observed to be locked in a kitchen cabinet and inaccessible to residents in care. The facility has one (1) new fire extinguisher, appeared to be full and serviced and were last purchased on 04/11/2024. BEDROOMS: There are six (6) bedrooms designated for residents’ private use. All bedrooms are furnished with beds, dressers, chairs, night stands and required bedding and linen. The bedrooms have sufficient closet space and have sufficient lighting. Auditory alarms were tested and observed to be operational at 10:00AM. There is no bedroom designated for staff use. Facility will have awake staff at night. Emergency call buttons are available and will be provided to residents. LAUNDRY ROOM: The laundry room is located in the separate locked area in the hallway between living room and bedrooms #5 and #6. The washer/dryer appear to be new and in good condition. Laundry supplies along with other chemical items are kept locked inaccessible under supervision when not in use. Continue on LIC809-C BATHROOMS: Facility has six (6) bathrooms. At 10:15AM LPA observed all bathrooms are clean, in good repair and properly supplied with toilet papers, soap and paper towels. LPA observed appropriate grab bars and non-skid mats in all bathrooms. Trash cans had closed tight fitting lids throughout. Water temperature was within regulation, and it was measured by an LPA at10:20AM to be at 110°F degrees. Bathrooms #3 and #4 are designated for Staff/Guests use only. COMMON AREAS: The facility maintains a comfortable temperature at 72°F. The living room and dining area appeared clean and were properly furnished. In the living room cabinet there were puzzles, coloring books, crayons, and board games. No obstructions and or tripping hazards throughout the facility. An emergency exit plan/sketch is posted along the hallway with other posting requirements. Facility has no fireplace. No fire-arms observed or will be maintained on the premises. MEDICATION ROOM: The medication and facility staff/resident files will be kept centrally stored and locked in the office area between the living room and the kitchen. First-aid kit was checked to be complete and has the new manual available. Facility will have four (4) staff members, two (2) for each shift (AM and PM). SMOKE DETECTORS/CARBON MONOXIDE. Dual smoke detectors and carbon monoxide were located in the bedrooms and throughout the facility. At 10:50AM they were tested and observed to be operational. SURROUNDING GROUNDS: The facility has gated entrance and sufficient yard space. LPA observed appropriate outdoor furniture, with a covered shaded area for residents. The backyard is fenced. The facility is sharing the property with another licensed RCFE facility. There is no attached garage. LPA discussed the importance of maintaining the care and supervision to meet the needs of the residents. There are no bodies of water. The facility has land line located in the kitchen area checked by an LPA to be operational. Component III: On 06/19/2024 the Licensee Yegeyan Nazar and Administrator Elen Bareghamyan. completed the Component III during Pre-Licensing visit at facility “Ingomar Assisted Living #197610556” at which time it was explained how to operate the facility within substantial compliance and had an opportunity to ask questions. Component III was waived based on LPA's discussion with the LPM. Based on inspection and observation, the physical plant is in compliance with Title 22 Regulations at this time. This report will be forwarded to the Centralized Application Bureau (CAB) and the applicant will be notified by the CAB Analyst when the license has been approved.
