Type of visit: Required - 1 Year
On 5/19/2026 at approximately 12:40 PM, Licensing Program Analyst (LPA), Angelica Segovia conducted an unannounced annual visit to the facility. LPA was greeted by the staff and stated the reason for their visit. The Administrator, James Durando arrived shortly after to assist with today’s visit. LPA asked for the census, Staff/Resident Roster, and Liability Insurance. LPA conducted a physical plant tour at approximately 01:00 PM and the following was noted: The facility is a single-story building with five (5) bedrooms and two (2) bathrooms. The facility is currently occupying four (4) residents. There is no designated staff room. The facility has an approved fire clearance for two (2) ambulatory and two (2) non-ambulatory residents. Due to time constraints, the annual inspection will be done at a later date. There were no immediate health and safety hazards observed during the day of inspection. Type of visit: Case Management - Annual Continuation
On 4/16/2025 at approximately 10:00 AM, Licensing Program Analysts (LPAs), Angelica Segovia and Jose Gary Tan conducted an unannounced CASE MANAGEMENT-ANNUAL CONTINUATION visit to this facility to continue with the Annual/Required visit initiated on 03/12/2025. LPAs were greeted by the caregiver and stated the reason for their visit. Administrator (AD) James Durando arrived shortly after to assist with today’s visit. LPAs asked for the census and Staff/Resident Roster. LPAs conducted a physical plant tour at approximately 11:00 AM and the following was noted: Kitchen: The kitchen was observed to be clean and free from pests. Sufficient supplies of seven (7) day nonperishable foods and two (2) day perishable foods were observed. Knives/sharps were observed to be kept in a locked kitchen cabinet. Kitchen appliances were observed to be working and in proper condition. Bedrooms: The residents’ rooms are adequately furnished with appropriate furniture and lighting system. Hallways/passageways are lighted appropriately. Extra linens/covers were observed stored in a storage cabinet located within the hallway. Staff Room: LPAs observed staff room to be clean, neat, and organized. LPAs observed staff room to be free of any toxins or hazards accessible to residents. Bathrooms: The bathrooms were checked for cleanliness and proper operation. The hot water temperature was measured within regulations at 105.6 °F. (continued on LIC 809-C) Laundry Room: The laundry room is located within the hallway’s passageway leading towards the bedrooms. LPAs observed laundry room to be kept locked and cleaning solutions properly stored within laundry room and inaccessible to residents. The laundry appliances were observed to be working and in proper condition. Garage: The garage can be accessed from inside of the facility and was observed to be kept locked. The garage was observed to be used as a staff office and storage. Backyard: The backyard of the facility is equipped with a designated shaded area with outdoor furniture for residents. LPAs observed a locked storage unit. There is no body of water located at the facility. Medications: The medications were observed stored in a locked filing cabinet located aside the dining room. First-aid kit observed to be equipped with but not limited to bandages, scissors, digital thermometer, tweezer, and First Aid manual. Smoke detectors and carbon monoxide observed to be working properly and were tested. Residents/Staff Records: LPAs conducted a complete file review of resident records. Resident records appeared to be complete and updated. Staff records: LPAs conducted a complete file review of staff records. Staff records appeared to be complete and updated. There were no immediate health and safety hazards observed during the day of inspection. Type of visit: Required - 1 Year
On 3/12/2025 at approximately 09:40 AM, Licensing Program Analyst (LPA), Angelica Segovia conducted an unannounced annual visit to the facility. LPA was greeted by the caregiver and LPA stated the reason for their visit. Administrator James Durando arrived shortly after to assist with today’s visit. LPA asked for the census and Staff/Resident Roster. LPA conducted a physical plant tour at approximately 11:00 AM and the following was noted: The facility is a single unit building with five (5) bedrooms and two (2) bathrooms currently occupying three (3) residents. The facility has an approved fire clearance for two (2) ambulatory residents and two (2) non-ambulatory residents. The facility is vendor through North Los Angeles Regional Center (NLARC) and is being operated at a Level III Residential Facility for the Elderly. Sign in sheet, hand sanitizer, gloves and masks are available. Common areas: The living room and dining room were observed to be neat, clean, and organized. The rooms were observed to be properly furnished and in good repair. The facility maintains a comfortable temperature at 73°F. LPA observed a fire extinguisher to be located near the kitchen and dated 05/30/24. LPA observed required postings such as See/Say Something, Facility License, and Personal Rights alongside the dinning area. A fireplace was observed to be covered and inaccessible to residents. A working telephone was observed. Due to time constraints this required annual will be completed at a later time. Aug 6, 2024Complaint investigation reportSubstantiated
Allegation investigated: Facility temperature was not maintained at a comfortable level
Licensing Program Analyst (LPA) Michael Cava conducted a complaint visit to the facility to investigate the above allegation. LPA met with staff, Wendy Rivera and the administrator, Jim Durando, and advised them of the complaint. Today’s investigation consisted of interviews with staff and residents. A physical plant inspection was also made to insure the health and safety of the residents. In regards to the allegation, it was reported by an another agency, that works in conjunction with the Department, and is also a credible source, that on or around 07/31/24, the inside of the facility was warm. The temperature inside the home was at approximately 89 degrees. Exterior temperature, or the weather, outside of the facility, on or around 07/31/24 was at least 100 degrees. Air conditioning was not broken, but staff did not know the combination to the lockbox covering the thermostat to adjust the temperature. At the end of this agency’s visit, temperature was still warm, and measured at approximately 87 degrees. Photos of the thermostat and temperature provided to Community Care Licensing (CCL). Substantiated Based on the information received, the allegation is Substantiated. Citation issued on the 9099D. Maintenance and Operation: The facility shall cool rooms to a comfortable range, between 78 degrees F and 85 degrees, or in areas of extreme heat to 30 degrees F less than the outside temperature. This requirement was not met as evidenced by information recieved by a credible agency submitted to CCL confirming that facility temperature was approximately 89 degrees indoor, while exterior temperature was at least 100 degrees. Plan of correction: Moving forward, the administrator will insure that the facility will maintain a comfortable temperature for the residents at all times. Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Spaeth conducted an unannounced visit and was greeted by the Administrator, Jim Durando. LPA stated the purpose of the visit is to conduct an annual inspection. The facility is licensed for two ambulatory and two non-ambulatory residents. LPA confirmed there are three residents in the facility. LPA and the Administrator toured the facility at 10:00 am until 10:30 am. Kitchen - LPA observed a two day supply of perishable food and a seven day supply of non perishable food items. The knives were safely locked in a kitchen cabinet. Common Areas –The living room contained comfortable seating. The dining room contained table and chairs. The medications are safely locked in a two-drawer cabinet in the dining room. Residents’ Rooms – The rooms contained a bed, linens, night stand, lamp, chest of drawers and a closet. Bathroom – The bathroom contained grab bars, slip resistant mats, paper towels and a covered trash can. The water temperature was tested at 12:35 pm and was 116.0 degrees F. Backyard –The backyard contained comfortable seating. The gate leading from the backyard to the front yard was not locked. Medications - The medications were locked in a two-drawer cabinet in the dining room. The first aid kit was also locked in the cabinet. Cont’d 809-C Laundry Area – The washer and dryer are located in a laundry area and was locked. The laundry detergent and cleaning solutions were locked in the laundry area. Egress System -LPA Spaeth observed the egress system was working when exiting the facility. Smoke/Carbon Monoxide Detectors - The smoke detectors and the carbon monoxide detectors were tested at 10:45 am and were operable. . Residents’ Records -LPA reviewed residents' records at 10:50 am until 11:15 am and LPA did not observe any issues with the records. Staff Records - LPA reviewed five staff records at 11:15 pm until 11:45 am. Based upon LPA's observations, there are no deficiencies.
