On 1/17/2026 at approximately 10:00 AM, Licensing Program Analyst (LPA), Angelica Segovia conducted an unannounced annual visit to the facility. LPA was greeted by the House Manager, Richard Garcia and stated the reason for their visit. LPA asked for the census, Staff/Resident Roster, and Liability Insurance. LPA conducted a physical plant tour at approximately 12:00 PM and the following was noted: The facility is a single-story building with seven (7) bedrooms and three (3) bathrooms. The facility is currently occupying five (5) residents. There are two (2) designated staff rooms. The facility has an approved fire clearance for six (6) non-ambulatory residents of which two (2) may be bedridden. Hospice waiver approved for six (6). Common areas: The living room and dining room were observed to be neat, clean, and organized. The rooms were observed to be properly furnished and in good repair. The facility maintains a comfortable temperature at 74°F. LPA observed a fire extinguisher to be located near the kitchen and dated 11/14/2025. LPA observed required postings such as Long-Term Care Ombudsman, Facility License, and Personal Rights to be located throughout the common areas. A working telephone was observed. LPA observed the fireplace to be covered and inaccessible to residents. Kitchen: The kitchen was observed to be clean and free from pests. Sufficient supplies of seven (7) day nonperishable foods and two (2) day perishable foods were observed. Knives/sharps were observed to be kept in a locked kitchen drawer. The cleaning solutions/toxins were observed to be kept locked underneath the kitchen sink. Kitchen appliances were observed to be working and in proper condition. (continued on LIC 809-C) Bedrooms: The residents’ rooms are adequately furnished with appropriate furniture and lighting system. Hallways/passageways are lighted appropriately. Extra linens/covers were observed to be stored in cabinets located within the hallway’s passageway. Staff Room: The staff rooms were observed to be locked. Bathrooms: The bathrooms were checked for cleanliness and proper operation. The hot water temperature was measured within regulations. LPA observed appropriate grab rails and slip-resistant mats to be in proper condition. Backyard: The backyard of the facility is equipped with a designated shaded area with outdoor furniture for residents. There is no body of water located at the facility. Garage: The garage can be accessed from inside of the facility. The garage was observed to be used for storage purposes. Extra refrigerator/freezer with additional food for residents was observed. Laundry Room: The laundry room was observed to be kept locked. LPA observed cleaning solutions and toxins stored appropriately within the laundry room and inaccessible to residents. The laundry appliances were observed to be working and in proper condition. Medications: The medications were observed to be kept in locked cabinets located near the kitchen. First-aid kit observed to be equipped with but not limited to bandages, scissors, digital thermometer, tweezer and First-Aid Manuel. Smoke detectors and carbon monoxide observed to be working properly and were tested. Residents/Staff Records: LPA conducted a complete file review of resident records. Resident records appeared to be complete and updated. Staff records: LPA conducted a complete file review of four (4) staff records. Staff records appeared to be complete and updated. There were no immediate health and safety hazards observed during the day of inspection. Licensing Program Analyst (LPA) Melissa Spaeth conducted an unannounced visit on 12/23/2024 and was greeted three caregiver at 10:00 am. LPA stated the purpose of the visit was to conduct an annual inspection. The caregiver confirmed there are five residents. The facility is licensed for a total of six non-ambulatory residents of which two can be bedridden. The Administrator, Stephanie Domingo arrived at 11:00 am. LPA Spaeth and the Administrator toured the location at 10:00 am until 10:40 am. Common Areas – LPA observed the family room, kitchen, and dining room are combined. The family room contained comfortable seating and a television. The dining room section contained a dining room table and chairs. Kitchen – LPA observed a two-day supply of perishable food and a seven-day supply of non-perishable food items. The medications are locked in a kitchen cabinet. The knives were locked in a kitchen drawer. A fire extinguisher was located in the kitchen area. LPA observed a resident's medication is stored in the refrigerator but is not locked in a safe location. Staff Room - LPA observed the staff was not locked but there were no safety hazards located in the room. Laundry Room – The laundry room was locked and contained washer/dryer, laundry soap, and cleaning solutions. Hallway – Clean linens are stored in cabinets. Continued on 809-C Garage - LPA observed the garage was locked. An additional freezer is located in the garage and contained additional frozen food items. Resident Bedrooms - There are four resident bedrooms which are furnished with a bed, linens, night stand, chest of drawers and a closet. Two residents are currently residing in the master bedroom (Bedroom #3) . Bathrooms – There are two bathrooms in the facility which contained hand soap, paper towels, slip resistant mat, and a trash can. Water Temperature - LPA tested the water temperature at 10:30 am which was 112.0 degrees F. Backyard - Comfortable outdoor seating is provided for the residents. The side gate leading from the backyard to the front yard was not locked. Smoke/Carbon Monoxide Detectors – The detectors were tested at 10:50 am and were operable. Medications - LPA observed the medications are locked in a kitchen cabinet. Auditory Device - At 10:25 am, LPA observed there was not an auditory device when exiting the master bedroom (#3) to the backyard. LPA reviewed residents' records at 11:00 am until 11:45 am. LPA reviewed staff records at 12:15 pm until 12:40 pm LPA reviewed resident medications at 1:00 pm. There were no issues when reviewing the records. Based upon the Title 22 regulations, the following deficiencies have been issued. LPA stated the purpose of the visit was to conduct an annual inspection. The Administrator confirmed there are five residents.. The facility is licensed for a total of six non-ambulatory residents of which two can be bedridden. LPA Spaeth and the Administrator toured the location at 10:50 am until 11:24 am. Common Areas – LPA observed the family room, kitchen, and dining room are combined. The family room contained comfortable seating and a television. The dining room section contained a dining room table and chairs. Kitchen – LPA observed a two-day supply of perishable food and a seven-day supply of non-perishable food items. The medications are locked in a kitchen cabinet. The knives were locked in a kitchen drawer. A fire extinguisher was located in the kitchen area. Laundry Room – The laundry room was located and contained washer/dryer, laundry soap, and cleaning solutions. Hallway – The hallway contained cabinets and the clean linens are stored in the cabinet.. Two residents are currently in the master bedroom (Bedroom #3) along with the master bath. Bathrooms – There are two bathrooms in the facility which contained hand soap, paper towels, slip resistant mat, and a trash can. LPA tested the water temperature at 11:00 am which was 117.0 degrees F. Backyard - Comfortable outdoor seating is provided for the residents. The side gate leading from the backyard to the front yard was not locked. Smoke/Carbon Monoxide Detectors – The detectors were tested at 11:10 am and were operable. LPA reviewed residents' records at 11:51 am until 12:15 pm. LPA reviewed staff records at 12:15 pm until 12:40 pm LPA reviewed resident medications at 1:00 pm until 1:15 pm.
