On 2/18/2026 at approximately 09:45 AM, Licensing Program Analyst (LPA), Angelica Segovia conducted an unannounced annual visit to the facility. LPA was greeted by staff and stated the reason for their visit. The Co-Administrator, Alvin Manlapaz arrived shortly after to assist with today’s visit. LPA asked for the census, Staff/Resident Roster, and Liability Insurance. LPA conducted a physical plant tour at approximately 12:00 PM and the following was noted: The facility is a single-story building with four (4) bedrooms and two (2) bathrooms. The facility is currently occupying four (4) residents. There is a designated staff room. The facility has an approved fire clearance for six (6) non-ambulatory residents of which one (1) may be bedridden. Hospice waiver approved for three (3). Common areas: The living rooms and dining room were observed to be neat, clean, and organized. The rooms were observed to be properly furnished and in good repair. The facility maintains a comfortable temperature at 70°F. LPA observed a fire extinguisher to be located near the kitchen and dated 11/04/2025. LPA observed required postings such as Long-Term Care Ombudsman, Emergency Disaster Plan, and Personal Rights to be located alongside the common ares. A working telephone was observed. LPA observed the fireplace to be covered and inaccessible to residents. Kitchen: The kitchen was observed to be clean and free from pests. Sufficient supplies of seven (7) day nonperishable foods and two (2) day perishable foods were observed. The cleaning solutions/toxins and knives/sharps were observed to be kept locked underneath the kitchen sink. Kitchen appliances were observed to be working and in proper condition. (continued on LIC 809-C) Bedrooms: The residents’ rooms are adequately furnished with appropriate furniture and lighting system. Hallways/passageways are lighted appropriately. Extra linens/covers were observed to be stored in storage closet located within the hallway’s passageway. Bathrooms: The bathrooms were checked for cleanliness and proper operation. The hot water temperature was measured within regulations. LPA observed appropriate grab rails and slip-resistant mats to be in proper condition. Backyard: The backyard of the facility is equipped with various designated shaded areas with outdoor furniture for residents. LPA observed there to be four (4) locked sheds. There is no body of water located at the facility. Staff room: LPA observed the staff room to be located in the backyard and to be kept locked. Garage: The garage can be accessed from inside of the facility. The garage was observed to be kept locked and used for storage purposes. Extra refrigerator and freezer with additional food for residents was observed. Laundry Room: The laundry appliances were observed to located within the garage. LPA observed cleaning solutions and toxins stored appropriately within the laundry room and inaccessible to residents. The laundry appliances were observed to be in proper condition. Medications: The medications along with staff and residents’ files were observed to be kept in locked filing cabinets located near the kitchen. First-aid kit observed to be equipped with but not limited to bandages, scissors, digital thermometer, tweezer and First-Aid Manuel. Smoke detectors and carbon monoxide observed to be working properly and were tested. Residents/Staff Records: LPA conducted a complete file review of resident records. Resident records appeared to be complete and updated. Staff records: LPA conducted a complete file review of five (5) staff records. Staff records appeared to be complete and updated. There were no immediate health and safety hazards observed during the day of inspection. On 12/04/2024 at approximately 09:44 am, Licensing Program Analyst (LPA), Angelica Segovia and Licensing Program Manager (LPM) Troy Agard conducted an unannounced annual visit to the facility. Upon arrival LPA was greeted by care giver Ruby Musni. LPA disclosed the reason for today’s visit. Caregiver Musni then called Administrator Eileen Maniapez who could not attend today’s visit but designated Assistant Administrator Alvin Maniapez to oversee today’s visit and sign any documentation's needed. LPA asked for census, staff, and resident file. LPA conducted a physical plant tour at approximately 10:30 am and the following was noted: There is only one entrance being utilized at the facility. The facility is a single unit building with four (4) bedrooms and two (2) bathrooms currently occupying five (5) residents. There is one (1) room located outside near room four (4) that is designated for staff use. The facility is fire cleared for six (6) non-ambulatory residents, one of which may be bedridden in room #4. Hospice waiver for three (3) residents. Required postings such as Facility License, Facility Sketch, and Ombudsman were located at the main entrance. Screening area is located immediately upon entrance. Sign in sheet, hand sanitizer, gloves and masks are available. Common areas are neat, clean, and organized. Rooms are properly furnished and in good repair. Fireplace observed to be covered inaccessible to residents. The facility maintains a comfortable temperature at 69°F. Fire extinguisher located in the kitchen and was purchased on 10/15/24. Additional required postings were observed aside the common areas such as: Yes poster, Emergency/Disaster Plan, and Personal Rights. Along side the common areas heading towards the kitchen medication, resident, and staff files are kept locked and inaccessible to residents. (continued on LIC 809-C) The kitchen observed to be fully stocked with two (2) days perishable and seven (7) days non-perishable food. Kitchen observed to be clean and inaccessible to pests. Knives, sharps, and cleaning solutions are kept locked in a cabinet under the sink and inaccessible to residents. Stove observed to be working and in proper condition. The backyard of the facility is equipped with a designated shaded area with outdoor furniture for residents. There is no body of water in this facility. Smoke detectors and carbon monoxide observed to be working properly and were tested. The Garage can be accessed from the inside of the facility. The garage is equipped with an extra refrigerator and fully stocked with food. Laundry room located inside the garage. Laundry detergents, cleaning agents, and other toxins are stored in the garage which is kept locked and inaccessible to residents. The Residents' rooms are adequately furnished with appropriate furniture and lighting system. Hallways/passageways are lighted appropriately. Residents have sufficient personal hygiene product which is provided by the licensee. The bathrooms were checked for cleanliness and proper operation. LPA observed the appropriate grab bars in the toilets and showers. The hot water temperature was measured at a range of 107.6°F. Towels and washcloths are not shared. Sufficient availability of clean lien stored in hallway cabinet. Medications: LPA observed medication stored in locked cabinet and inaccessible to residents. Medication usage recorded and stored properly. LPA along with Assistant Administrator Maniapez conducted a review of the medication to ensure compliance. First-aid kit observed to be equipped with but not limited to: bandages, scissors, digital thermometer, tweezer, and manual. Resident records: LPA conducted a complete file review of resident records. Resident records appeared to be complete and updated. Staff records: LPA conducted a complete file review of staff records. Staff records appeared to be complete and updated. There was no immediate health and safety hazard observed during the day of inspection. Licensing Program Analyst (LPA) Gary Tan, met with staff Maria Prima Dominguez for a One (1) Year Required visit for this facility. LPA arrived and was greeted by Prima who called the administrator and informed the purpose of the visit. Administrator Elieen Manlapaz designated Ms. Dominguez to sign the report. There is only one entrance being utilized at the facility, there are required poster posted at the main door. Screening area is located immediately upon entrance. Sign in sheet, hand sanitizer, gloves and masks are available. The facility had submitted and approved Mitigation and Infection Plan. Signs to wear a mask and other Covid 19 prevention protocol signs were posted outside the doors. Hand washing, coughing etiquette, physical distancing and other necessary signs were posted in the bathroom and all over the facility. The facility has a designated visitors' area at the backyard. The facility has sufficient stock of PPE in the storage room. A tour of the physical plant was conducted with Mr. Dominguez at 12:59 PM. The facility is a single storey building with four (4) bedrooms and two (2) bathrooms currently occupying four (4) residents. The facility is fire cleared for six (6) non-ambulatory residents, one of which may be bedridden in room #4. Hospice waiver for three (3) residents. Physical environment was checked for cleanliness and condition. Walls, windows, ceilings, floors and floor coverings, and doors were checked, the following was noted: Living and dining room furniture were also checked. The living room is neat and clean along with dining The facility maintains a comfortable temperature at 74°F. Dual smoke/carbon monoxide detectors are hardwired and inter connected and observed to be operational. The fire extinguishers were filled and last bought on 09/29/23. The facility is equipped with sprinkler system. (continued on LIC 809-C) The backyard of the facility has outdoor furniture, with a covered shaded area for clients. The front and backyard passageways were clear of any obstruction. There is no body of water in the facility. The backyard has three (3) tool sheds observed to be locked. The garage is attached to the home and was digitally locked and inaccessible to residents during the visit. The garage is also used as a stock room for emergency foods and as a laundry area. Food Service/Kitchen area was sufficiently stocked with two (2) days perishable and seven (7) days non-perishable food. Frozen foods are properly wrapped and stored appropriately. Food storage and preparation areas are clean and inaccessible to pests. Cleaning supplies including detergents and pesticides and other toxins are stored in garage. Knives and sharps are observed to be kept in a locked drawer in the kitchen. The Clients' rooms are adequately furnished with appropriate furniture and lighting system. Clients have sufficient amounts of personal hygiene product which is provided by the licensee. The bathrooms were checked for cleanliness and proper operation. LPA observed the appropriate grab bars for each toilet, bathtub and shower. The hot water temperature measured at a range of 107.7°F to 109.9°F. Towels and washcloths are not shared. There is enough clean linen available in stock at the linen cabinet. Medications: LPA observed medication in the kitchen cabinet to be locked and inaccessible to residents. Medications are listed on the centrally stored medication and destruction record. First aids kits have complete tools and supplies. Client records: Client records are reviewed and appeared to be complete and updated. Staff records: LPA conducted a complete file review of staff record. Staff records appeared to be complete and updated. Disaster drill was last conducted on 03/30/24. Required posting are observed to be complete and current and displayed properly at the facility.
