On 05/08/2026 at 8:30AM, Licensing Program Analyst (LPA) Andrew Christy arrived unannounced to conduct the 1-Year Annual Required inspection. LPA met with Administrator, Christine Soriano, and explained the purpose of the visit. The facility is currently at capacity with six (6) residents. LPA toured facility including but not limited to bedrooms, bathrooms, kitchen, common area and backyard. All outdoor and indoor passageways are kept free of obstruction. No bodies of water were observed. A comfortable indoor temperature is maintained at 69.0 degrees Fahrenheit. The hot water temperature in the residents’ shared bathroom was measured at 106.9 degrees Fahrenheit. LPA observed lighting in all rooms are adequate for the comfort and safety of the residents. Residents’ bathrooms are equipped with grab bars and non-skid mats. There is a minimum of one week supply of non-perishable and 2 day of perishable foods. Centrally stored medication and sharps were locked and inaccessible to residents. Smoke detectors and carbon monoxide detectors were in operating condition during visit. Fire extinguisher was last serviced on 05/06/2026. At 10:00AM, LPA reviewed five (5) resident files and five (5) staff files, all found to be complete. The emergency disaster plan was last reviewed 04/04/2026. Quarterly emergency drills were last conducted 04/04/2026. A review of resident medications and the Medication Administration Record (MAR) found no outstanding errors. No deficiencies cited during visit. On 05/20/2025 at 1:25 PM, Licensing Program Analyst (LPA) P. Manalo arrived unannounced to conduct 1-Year Annual Required inspection. LPA met with Direct Care Staff, Josephine Holgado, and explained the purpose of the visit. Administrator was unable to come and gave authorization on the phone for staff to sign the report. Administrator certificate is current. LPA toured facility inside and out including but not limited to bedrooms, bathrooms, kitchen, common area and backyard. The facility consists of 5 total bedrooms which all bedrooms are occupied by residents and 1 bathroom. There are no bodies of water observed. A comfortable temperature is maintained at 72 degrees Fahrenheit. LPA observed lighting in all rooms are adequate for the comfort and safety of the residents. The hot water temperature in the residents’ shared bathroom was measured at 109.8 degrees Fahrenheit. Residents’ bathrooms are equipped with grab bars. Smoke detectors and carbon monoxide detector were in operating condition during visit. Fire extinguisher was last serviced on 05/09/2025. Emergency Disaster Plan was last posted on 05/20/2025. First aid kit was observed to be complete. Emergency disaster drill was last conducted on 04/26/2025. At 2:16 PM, LPA reviewed 6 residents records. At 2:52 PM, LPA reviewed 4 staff records and all are associated to the facility. At 4:30 PM, LPA reviewed two samples of resident’s medications. Continue to LIC809-C... Continue from LIC809... At 3:33 PM, LPA observed that staff did not have sufficient training hours. At 3:45 PM, LPA observed that S3 and S4 did not have a LIC503 and TB test on file. The Facility was cited from the California Code of Regulations, Title 22 and/or Health and Safety Code Failure to correct deficiencies by POC date may result in additional Civil Penalties. On 4/25/2024 at 2:45pm, Licensing Program Analyst (LPA) L. Hall conducted an unannounced 1-Year Required inspection. LPA met with Caregiver, Aurose Ratilla, and explained the purpose of the visit and explained the purpose of the visit. Administrator, Christine Soriano, arrived at 5:05pm. The Administrator currently holds a certificate (#6023135740) that expires on 12/14/2024. The facility’s fire clearance was approved for five (5) non-ambulatory and one (1) bedridden residents. LPA toured the facility with Caregiver including but not limited to bedrooms, bathrooms, kitchen, garage, common area, and back yard. The facility consists of four (5) total bedrooms and one (1) bathrooms. All outdoor and indoor passageways are kept free of obstruction. A comfortable temperature is maintained at 68 degrees Fahrenheit. LPA observed lighting in all rooms are adequate for the comfort and safety of the residents. The hot water temperature in the residents’ shared bathroom was measured at 103.5 degrees Fahrenheit. Residents’ bathrooms are equipped with grab bars and non-skid mats. There is a minimum of 7-day supply of non-perishable and 2-day of perishable foods. Smoke detectors/ carbon monoxide were in operating condition during visit. Fire extinguisher was last services on 15/5/2023. First aid kit was observed to be complete. Fire drill was last conducted on 11/8/2023. Continued on LIC809. Continued from LIC809. Five (5) staff records were reviewed. All five (5) resident records were reviewed and complete. LPA requested the following documents to be submitted to CCLD by 5/2/2024. At 4:05pm, LPA observed office walls had be torn down to make open area.
