On May 26, 2026, at 10:00 AM, Licensing Program Analyst (LPA) Edward Kim conducted an unannounced required 1-Year annual visit using the CARE Inspection Tool. Upon arrival at the facility, LPA Kim met with Administrator (ADMIN) Hany Fawaz and explained the purpose of today's visit. The facility is licensed to operate for six (6) nonambulatory residents and has a hospice waiver for six (6) residents. The facility is a single-story structure located in a residential neighborhood. It consists of the following: four (4) resident bedrooms, two (2) bathrooms, living area, dining area, kitchen, outdoor shed in the backyard, outdoor covered patio, and an attached two car garage. LPA Kim toured inside and outside of the physical plant with ADMIN Fawaz. There were no bodies of water or obstructions in the facility. All rooms were inspected. Beds and bedding supplies were in good condition, adequate lighting was provided, storage for each resident's personal belongings was observed. Bed linens, comforters, and bath towels were adequately stocked at the time of visit. The Resident’s rooms were inspected: Resident Room 1, Resident Room 2, Resident Room 3, and Resident Room 4. Bathrooms were found to be within Title 22 regulations and were clean and operational. The water temperature measured at 105.8 degrees F to 111.0 degrees F. A comfortable temperature of 76 degrees F was maintained in the facility. LPA Kim observed the facility to be appropriately furnished at the time of visit. Storage areas for personal hygiene, cleaning supplies, toxins, and sharps objects were stored and not accessible to residents. The kitchen was inspected and there is a two-day supply of perishable and seven-day supply of non-perishable food both were available and maintained properly. Emergency food, emergency water, and emergency supplies are stored in garage. Evaluation Report Continues on LIC 809-C During the visit, LPA Kim observed the facility's infection control practices, plan of operation, and screening protocols for visitors, staff, and residents. LPA observed the facility has a 30-day supply of Personal Protective Equipment (PPE). All mandated inspection control posters were posted. A working telephone (714-602-7072) and internet accessible device dedicated to residents both remain available. Emergency drill logs are conducted quarterly and last conducted on March 19, 2026. Certificate of Liability Insurance is effective on July 3, 2025, and expires on July 3, 2026. Fire Extinguisher is mounted in the kitchen. LPA Kim conducted an audit of resident files (R1-R4), staff files (S1-S7), and medication and medication administration record were all in order and complete. LPA Kim conducted interviews for one staff. No deficiencies were cited during this visit. Allegation investigated: - Facility staff made false or misleading statement regarding the facility in their advertisement

On this day, Licensing Program Analyst (LPA) Michael Tea made an unannounced visit to conduct a complaint investigation. LPA Tea was greeted and granted entry by facility staff and explained the reason for the visit. Administrator (AD) Hany Fawaz arrived shortly to assist with the visit. The department received a complaint on Feburary 25, 2026 and LPA Tea conducted the initial 10-day visit the following week on March 3, 2026. It was alleged that facility staff made false or misleading statement regarding the facility in their advertisement. LPA Tea toured the facility and interviewed facility staff and other witnesses and collected pertinent documents. The investigation determined the following: LPA Tea spoke to Administrator (AD) Hany Fawaz. He stated he earned a Doctor of Pharmacy degree in Egypt. He confirmed that he does not have a medical license or pharmacy license in the United States (Complaint Investigation continued on LIC9099C) Unfounded and does not practice medicine or pharmacy. He stated he understands it would be illegal to practice without a license. AD Fawaz explained that members of his Egyptian Christian Coptic church sometimes call him “Doctor” because of his past education and title in Egypt. He stated he has never told residents or families that he is a practicing doctor in the United States and has never advertised medical services at the facility. LPA spoke two staff members present at the time of the visit. Two staff members confirmed that AD Fawaz has a pharmacy degree from Egypt but does not practice in the United States. Both staff stated they have never heard him tell anyone that he is a practicing medical doctor or pharmacist. They also stated they are not aware of any advertisement saying this. A visiting individual who was caring for a resident stated she knew the Administrator was formerly a doctor in Egypt. However, they confirmed they have never heard him say he provides medical services at the facility and has never seen any advertisement claiming this. LPA Tea reviewed the personnel file and confirmed documentation showing AD Fawaz completed his pharmacy degree at Cairo University in Egypt. LPA also reviewed the facility’s pamphlet and advertisement. There was no mention of AD Fawaz’s medical background and no advertisement of medical doctor services. Based on interviews and records reviewed, the investigation found no evidence that the facility or Administrator made false or misleading statements in advertisements. Interviews consistently confirmed that AD Fawaz does not present himself as a practicing medical doctor or pharmacist in the United States. Review of marketing materials substantiated that no medical services or physician credentials are advertised. There is no corroborating evidence to support the allegation. Therefore the allegation mention above has been determined as UNFOUNDED, meaning the allegation is false, could not have happened and/or is without a reasonable basis. No deficiencies cited at this time and an exit interview was conducted with Administrator (AD) Hany Fawaz. Type of visit: Case Management - Annual Continuation

On June 25, 2025, at 8:00 AM, Licensing Program Analyst (LPA) Edward Kim conducted a continuation visit for a required 1-year annual visit. LPA Kim was greeted and gained entry to the facility by staff. Administrator Hany Fawaz arrived around 8:20 AM. LPA Kim conducted a record review on six staff files (S1-S6). An internet access device dedicated to the residents was available. The facility has smoke detectors and carbon monoxide detectors that were operable. First Aid Kit contained all the necessary elements. LPA Kim conducted interviews for one resident and two staff. LPA Kim reviewed resident medications and medication administration record. No deficiencies were cited during this inspection visit. Type of visit: Prelicensing

Licensing Program Analyst (LPA) Sean Haddad conducted this announced inspection for the purpose of conducting a pre-licensing inspection. LPA met with Applicant (AP) Hany Fawaz, discussed the purpose of the inspection, and toured the facility. Facility is to operate a Residential Care Facility for the Elderly. Application was submitted to Community Care Licensing on 08/25/2023. This is a change of ownership with persons in care. During the inspection, LPA and AP observed the following: Structure: facility is a 4-bedroom, 2-bathroom, 1-story house with attached garage that is used for storage and an office. Facility telephone number is (714) 602-7072. Resident Bedrooms: the 4 resident bedrooms are spacious and will easily accommodate the residents’ furnishings. Lights, chairs, linens, and storage for each resident bedroom inspected. Staff Bedrooms: there are no staff bedrooms. Bathrooms: were clean, faucets and toilets were operational. Water temperature: tested at 110 degrees F in the common resident bathroom and 105 in the private resident bathroom. Linens & Hygiene Supplies: new linens and fully stocked linen closets were observed. Emergency Phone Numbers, Exit Plan & Menu: reviewed. Food Service: 2 days perishable and 7 days nonperishable food supply reviewed. Carbon Monoxide, Smoke Detectors, Fire Extinguisher: observed and tested. Appliances: stove burners, microwave, washer, and dryer inspected. Knives: observed locked in the kitchen. Toxins: observed locked in the kitchen and garage. Medication cabinet is locked. First-Aid Kit & Activity Supplies: observed and available. Resident & Staff Files: LPA reviewed 4 resident files and 2 staff files. Fire clearance was approved by the Anaheim Fire Department on 10/09/23. Backyard exit gate is operational and unlocked. Backyard has shaded area for outdoor activities and sufficient seating for residents. Component III was completed with AP during today’s inspection. Facility is currently operating under the liability insurance of current facility HARVARD GUEST HOME (306006121). AP will switch liability insurance to new facility once the application is approved. During the inspection, LPA explained the process of this application and about the post licensing inspection once the facility is licensed. AP was informed today that the facility is ready for licensure and final approval will be processed by the CAB supervisor in Sacramento.
