Type of visit: Collateral

On July 16, 2026, at 8:30AM, Licensing Program Analyst (LPA) Edward Kim conducted an unannounced Collateral visit. LPA Kim met with Caregiver (CG) Brandonle Mora Sandoval. CG Mora called Administrator (ADMIN) Daniel Datcu and LPA Kim explained the purpose of the visit. The purpose of this Collateral visit was in reference to a complaint with Complaint Control Number: 22-AS-20250516082515. ADMIN Datcu could not attend today’s visit and stated CG Mora could sign on behalf of the facility. LPA interviewed resident #1 in private room due to complaint allegations that occurred at another licensed facility. No deficiencies were cited during the inspection visit. Type of visit: Required - 1 Year

On December 23, 2025, Licensing Program Analyst (LPA) Jenifer Tirre conducted an unannounced required annual visit. LPA was greeted by staff and granted entry after stating the purpose of the visit. Administrator Daniel Dactu was available to assist with the facility inspection. The facility is licensed for six (6) Non ambulatory of which one (1) may be bed ridden with approved Hospice waiver of four (4). There are five (5) Residents present during today’s visit of which one (1) is on hospice. This is a two story home with attached garage facility. The facility has seven rooms located on first floor (six private resident rooms and one (1) staff room) and four (4) bathrooms. Second floor is three (3) bedrooms for live in staff. Residents do not have access to second floor. LPA conducted a tour of the physical plant accompanied by Administrator Daniel Dactu, and the following was observed: There were no bodies of water on the premises. All rooms were inspected. Beds and bedding supplies were in operational condition, lighting was provided, and storage for the residents’ personal belongings was observed. Bathrooms were operational with water temperature measured between 106.5 to 107.9 degrees F. A comfortable temperature of 71 degrees F. was maintained in the facility. LPA observed the facility to be furnished at the time of the visit. Storage areas for personal hygiene, toxins and sharps objects were stored and not accessible to residents. The kitchen was inspected, and sufficient perishable and non-perishable food was maintained adequately. Facility has emergency food and water supply. Two fire extinguishers was mounted and fully charged. During the visit, LPA observed the facility has a supply of Personal Protective Equipment (PPE). All mandated inspection control posters were posted. LPA observed First Aid Kit was maintained. The last fire drill was conducted on December 01, 2025. The facility had operational smoke and carbon monoxide detectors in bedrooms and common areas. The facility has current liability insurance on file effective 12/18/2025- 12/18/2026. A review of five residents (R1-R5) service files and three staff (S1-S3) personnel files was revealed to be complete. The facility has the current administrator's certification on file for Daniel Dactu Expiration 3/12/2026. No deficiencies during this inspection visit. Type of visit: Required - 1 Year

Licensing Program Analysts (LPA) Samer Haddadin made an unannounced visit for the purpose of conducting a Required/Annual Inspection. LPA was greeted by Staff (S1) Brandon Mora and granted entry into the facility and explained the purpose of the visit. During the inspection LPAs and S1 conducted a tour of the inside and outside of the facility, common areas, resident rooms, kitchen, garage and observed the following: This is a two-story house with seven residents’ bedrooms and three restrooms located on the first level, The second story is for staff and had three staff bedrooms and one restroom. During the inspection LPAs observed two staff on duty and five residents in care. LPA and S1 tested smoke detectors/carbon monoxide in common areas and bedrooms; all were operational. Resident bedrooms had the required furniture, bed linens and closet/drawer space to accommodate each resident comfortably. LPAs observed fire extinguisher mounted in kitchen area with last inspection date of on July 18th, 2024. Upon review, fire drill log not conducted nor maintained per regulation . During inspection of the kitchen area, LPAs observed sharps, knives in kitchen drawer to be unlocked, due to a broken lock and accessible to residents in care. LPA observed 2-day supply of perishable and 7-day supply of non-perishable foods and water was observed during today’s visit. Kitchen appliances were observed to be operational during today's visit. Restrooms toilets and water faucets were also observed to be operational. Grab bars were secure, and showers were observed to be free of mold/mildew. Water temperature measured between at 112.6 degrees Fahrenheit and 113. degrees Fahrenheit. LPAs and S1 toured the backyard of the facility and observed a shaded seating area for residents’ enjoyment. LPAs reviewed three residents’ and three staff files and observed they all had the required documentation. Based on the observations made during today’s inspection, deficiencies are being cited per Title 22 Division 6 of the California Code of Regulations.
