A Non-Compliance Conference (NCC) was conducted today in the Santa Rosa Regional Office. Present in the meeting were: Regional Manager (RM) Carla Nuti-Martinez, Licensing Program Manager (LPM) Victoria Bertozzi, Licensing Program Analysts (LPAs) Christi Coppo, Marisol Cuadra and Licensee Josephine Credo. The purpose of today's office meeting was to discuss areas of concern in the facility operation identified by the department and placing Valley View Care Home, Five Palms Care Home and Chanate Care Home on a Non-Compliance Conference (NCC) plan. Parties present during the meeting agreed to an NCC plan to bring the facilities into compliance. An Office meeting was held 9/15/2025 to discuss a change of ownership for all licensed facilities from a sole proprietorship to an LLC. To date, the Change of Ownership is delayed due to not having a valid LLC. Items addressed during the meeting include, but are not limited to: Significant Delay in bringing LLC into compliance therefore delaying the Change in Ownership Recent findings by the Department of Labor Medication Management Record keeping (Administrator) Parties present during the meeting agreed to an NCC plan for 2 years to bring the facility into compliance. The licensee understand that may not operate under their LLC as they are licensed under sole proprietorship There was a discussion about Technical Support Program (TSP) referral and Licensee agreed to be referred to the TSP program and referral will be submitted. Type of visit: Required - 1 Year

At approximately 12:50 PM, Licensing Program Analyst (LPA) Robert Frank arrived unannounced to conduct a Required 1 Year visit. Licensee Josephine Credo arrived at 2:00 PM. Chanate Care Home is Licensed as a Residential Care Facility for the Elderly (RCFE). The facility is a single story ranch house. The facility has an approved fire clearance for five (5) non-ambulatory residents. The facility has a Hospice Waiver for two (2) residents. Upon arrival, LPA was informed that there were four (4) residents in care and two (2) staff members on-site. LPA reviewed the Facility's Staff Roster and found that all staff on-site were background cleared and associated to the facility per regulation. At approximately 1:15 PM, LPA toured the facility. All exits were clear and unobstructed. All three (3) fire extinguishers were last serviced and tagged on 2/25/2025. The facility was sufficiently lighted. LPA inspected three (3) resident bedrooms and found all to have sufficient lighting and furnishings as required per Title 22 Regulations. There was a sufficient supply of both perishable and nonperishable foods as required by Title 22 Regulations. There was an appropriate supply of cleaning products, linens, hygiene products and paper products available for residents. Toxins were observed to be stored inaccessible to residents. Facility has an infection control plan as required. The facility has a required emergency disaster plan. The facility is conducting fire and emergency drills quarterly. The last disaster drill was conducted on 11/8/2025. The facility does have emergency food and supplies to meet the "72 hour shelter in place" requirements. Hot water temperatures for all sinks in facility were found to be within Title 22 regulations of 105 to 120 degrees Fahrenheit. Facility smoke detectors and carbon monoxide detectors were tested and observed to be operational. Continued on 809-C... ...Continued from 809 At approximately 1:55 PM, LPA reviewed four (4) resident files. Four (4) of four (4) resident files were observed with all required documentation. LPA reviewed three (3) staff files. All staff files were observed with all required documentation including First Aid and CPR certification and proper training documentation. LPA spot checked Medication for three (3) residents. LPA observed all medications to be centrally stored, secure and with proper documentation. The facility does not handle resident’s monies for personal and incidental items. Back Up Administrator Elena Credo's Certification 7007838740 is current with an expiration date of 3/18/2027. LPA requested the following documents be submitted to Community Care Licensing by 12/21/2025: LIC 500 Personnel Report LIC 610E Emergency Disaster Plan Proof of Liability Insurance Updated Lease Copy of Current Admissions Form The facility is in compliance. An informal meeting was conducted today in the Santa Rosa Regional Office. Present at the meeting were Licensing Program Managers Bethany Moellers and Victoria Bertozzi (via Microsoft Teams), Licensing Program Analysts Christi Coppo, Robert Frank, Marisol Cuadra (via Microsoft Teams) and Licensee of the facility, Josephine Credo. On 9/10/25, The Licensee submitted written communication via email to the Department requesting information regarding change of ownership. Currently, LIC309 Administrative Organization form indicates that these facilities are in a sole proprietorship. According to the Licensee, their accountant has advised them to switch it to a limited liability company (LLC) where Josephine Credo will be the Licensee in LLC of these facilities. The purpose of the office meeting was to address concerns and discuss the change of ownership for Five Palms Care Home #496803300, Valley View Care Home #496803362 and Chanate Care Home #496801588 in which Josephine Credo is the identified Licensee to date a new application has not been submitted to the Department. License acknowledges that they are responsible for the operation until the new application is approved and issued. The Licensee agrees to provide documentation to ensure control of property. The Licensee confirms understanding that the current license is not transferable and an application is required to be submitted to the Department for change of ownership. Also, areas of concern regarding training materials that are not updated and reporting requirements. The current options discussed during today’s office meeting are the following: -Licensee confirmed that facility is operating under new J&J Assisted Living, LLC. The LLC was filed in 2019. Licensee advised LegalZoom was not able to submit 2020 and 2021 Statement of Facts which is the explanation licensee gave for being suspended with the Franchise Tax Board (FTB). LPM Moellers advised this must be cleared up Continued on 809C.... ...Continued from 809 before proceeding with Centralized Application Bureau. LPM Bertozzi advised to update their Plan of Operation. To be included in the new Plan of Operation licensee will need to reference what training will be provided to staff. Submit a change of ownership application with new corporation or LLC to the Centralized Application Bureau (CAB) for the three facilities by not later than 10/13/25. CAB contact information was provided. -LPAs reviewed reporting requirements regulation (87211), and gave copy to LLC. -The Licensee agrees to submit current training materials and a plan of how she will utilize the new vendor for staff training. Licensee to submit course contents from new training vendor, by no later than 9/22/25. LPM Moellers offered TSP, licensee agreed to participate. Deficiencies are cited from the California Code of Regulations (CCRs), Title 22, Division 6, Chapter 8 and the Health and Safety Code. 12/17/2024, Licensing Program Analyst (LPA) Loera conducted an unannounced Annual Required – 1 yr. inspection visit for this facility. Facility has an emergency disaster plan as required. Facility has an infection control plan as required. There are currently five (5) residents in care. Facility approved/cleared for five (5) non-ambulatory and hospice waiver granted for two (2) residents. LPA was greeted by staff member. At approximately 1:50pm, LPA and Licensee toured the building and grounds. The facility was found to be at a comfortable temperature. LPA observed a 2 day supply of perishable and 7 day supply of non-perishable food. Refrigerated food was found to be stored in a safe manner being labeled and dated. Medications were found to be centrally stored. All rooms were equipped with lighting, night stand, and chest of drawers. All rooms were in good repair. Extra hygiene products and linens were available. Water temperature in sinks accessible to residents in care were measured at 111.6 and 111.3 degrees F which is within the range of 105 to 120 degrees F. Fire extinguishers were last inspected 02/2024. Smoke/Carbon Monoxide detectors located throughout the facility were tested and operational. Facility conducts quarterly fire drills with the last one being conducted on 11/22/2024. Toxins, sharps and other items that could pose threat if available to residents were located in the garage and in the kitchen and found to be secured. LPA conducted spot medication count and found all prescription medication to be properly recorded on the Centrally Stored Medication Record. LPA conducted a review of four (4) resident records. All records had the required documentation. LPA conducted review of four (4) staff records/training. Upon a review of staff records, LPA found all staff to have required annual and initial training as well as current 1st Aid & CPR certification on file. No deficiencies cited during today's inspection. Licensing Program Analyst (LPA) Victoria Bertozzi arrived unannounced to conduct an Annual Required inspection and was greeted by caregiver, Ana Caadyang. Licensee/Administrator, Josephine Credo was unavailable but staff were able to assist LPA with visit. LPA initiated a tour of the facility around 9:15am and made the following observations: Facility was a comfortable temperature and passageways were free from obstructions. Resident rooms were furnished per regulation. Water temperature in bathrooms used by residents measured at 107 and 109 degrees F which are within the range of 105 to 120 degrees F allowed per regulation. Extra hygiene products and linens were available. Cabinets containing cleaning supplies were inaccessible. Facility has at least two days of perishable and one week of non-perishable foods which appeared to be of quality and stored per regulation. Medications were centrally stored and locked. Emergency food and water supplies are stored in the garage. Personal Protective Equipment is also stored in the garage. Fire extinguishers were last serviced February 2023. Smoke and Carbon Monoxide detectors located throughout the facility were tested and operational during inspection. Facility did not have the date of their most recent fire/disaster drill but per staff, it was conducted September or October, 2023. LPA directed staff to document all drills in the future. Five staff files and five resident files were reviewed. Staff have required First Aid and CPR certificates. Administrator Certificate for Administrator, Josephine Credo is expired but is on the Department's pending list. Medications and medication records were reviewed. Training records were also reviewed.
