Licensing Program Analyst (LPA) Carmen Lopez conducted an unannounced case management visit to the facility and conducted a health and safety check for resident in care. LPA Lopez identified herself and was granted entry by Kate Sherlock, Receptionist. LPA stated the purpose of the visit and reviewed the basic elements of the visit with Kahina Hadibi, Assisted Living Director. The facility self-reported an Unusual Incident/Injury Report regarding resident #1 (R1) (See LIC 811 Confidential Names List) to Community Care Licensing on February 6, 2026, and the SOC 341 was submitted to the Department on February 5, 2026. According to the reports, a resident had reported to an outside agency that an unknown male staff person wanted to purchase them and also touched them inappropriately. During today’s visit, LPA spoke with staff and R1, and requested and obtained resident records. LPA informed Assisted Living Director Hadibi that possible follow-up visits or calls may be required for this incident. No deficiencies were cited during this visit. Licensing Program Analyst (LPA) Nacole Patterson conducted an unannounced Required Annual Inspection. The facility file was reviewed prior to the visit. LPA was welcomed by and discussed the purpose of the visit to Executive Director Sadie Harness and Assisted Living Director (ALD) Kahina Hadibi. The facility's license shows a maximum capacity of ninety-five (95) non-ambulatory residents, ages 60 and above with a hospice waiver for seven (7). During today’s inspection there were 67 residents in care. LPA and ALD toured the interior and exterior of the facility and inspected each room. The facility was clean, sanitary, and in good repair. Pathways were free of obstruction and slip hazards. Client bedrooms contained the required furnishings. Doors, windows, screens, toilets, and showers were in working order. Extra linens and hygiene supplies were present, as well as Personal Protective Equipment. The facility had sufficient space and equipment to facilitate dining, laundry, visitation, meetings, and client activities. The facility contained at least 2 days of perishable food, and at least 7 days non-perishable food, all safely stored. Cooking, dining equipment, and utensils were present. No toxic chemicals or poisons were accessible to clients. Medications were labeled, as required, and stored in locked areas. No pools or bodies of water exist on the premises. Per ALD, no firearms or ammunition are kept at the facility. Carbon monoxide detectors, emergency lighting, and facility telephone were all in working order. Fire extinguisher(s) were serviced within the last 12 months. First aid kit(s) were complete and readily accessible. Required licensing postings were observed in visible areas of the facility. LPA interviewed staff and clients, and reviewed facility records. The files reviewed by LPA contained required documents. Confidential records were stored in locked areas. No deficiencies were cited during the inspection. LPA, Executive Director Sadie Harness, Wellness Coordinator Snezana Skrbic, and Operations Manager David Sharp toured the interior and exterior of the facility and inspected each room. The facility was clean, sanitary, and in good repair. Pathways were free of obstruction and slip hazards. Client bedrooms contained the required furnishings. Licensing Program Analyst (LPA) Nacole Patterson conducted an unannounced Case Management Visit. LPA was greeted by and met with Administrator Sadie Harness, to discuss the purpose of the visit. Today's visit is in response to the self reported incident of Resident 1 (R1 - see LIC811 Confidential Names List) who left the facility unassisted. R1 was located and assisted back into the facility by staff within 7 minutes with no injuries. LPA interviewed staff and residents and collected records. A wellness check was completed; no health or safety issues were identified. No deficiencies were cited or observed on this date. Licensing Program Analyst (LPA) Amy Rodgers, made an unannounced visit to conduct the required One-Year Inspection. LPA Rodgers was granted entry into the facility by Administrator Sadie Harness, after identifying herself and stating the purpose of the inspection. The facility serves ninety five non-ambulatory elderly residents: ages 60 and above. Hospice care waiver approved for seven. This is a two-story building with approved delayed egress in part of the first floor. LPA was accompanied by Assistant Living Director Kahina Hadibi, during a tour of the facility, which was conducted inside and out and included a sample of resident units, the dining area, recreation rooms, and food storage areas. There is a fire system in place and the carbon monoxide detectors were operational. The last disaster drill was conducted in December 2023. Exterior and interior passageways were free from obstructions. According to Administrator Harness there are no weapons and/or ammunition stored on the premises. Resident’s room temperatures were within a comfortable range. Each resident had clean and sufficient bed linens, towels, and washcloths. All residents’ rooms were equipped with required furnishings. Lighting was present in the bedrooms. Residents’ bathrooms were observed to be sanitary and operational. Toilets and showers were equipped with grab bars. Hot water temperature in residents’ bathrooms were compliant. [Continued on 809-C] [Continued on 809] Facility has a two-day supply of perishable and a seven-day supply of nonperishable food items. Food supply is replenished frequently by outside vendors. Food was observed to be properly stored and labeled. Food menus and activities schedule were posted. Chemicals and cleaning supplies were stored in a locked room. Medication carts are locked and stored in the wellness medication room, not accessible to residents. Medications were labeled and kept in compliance with label instructions. LPA interviewed multiple staff and clients. LPA reviewed multiple staff and client records/files. The interviews did not raise any significant licensing concerns. The reviewed files contained all required documents. LPA Rodgers interviewed several staff also conducted a review of In-service training procedures. Confidential records were stored in locked areas. Licensee's staff also presented proof of current/active business liability insurance. There are several areas used for activities such daily concerts, exercises, game activities, and arts/crafts. At the time of visit, LPA observed a small group activity, in which some residents were participating. LPA observed that residents were being treated with dignity by staff, and there were sufficient staff on duty to meet resident’s needs. No deficiencies were observed or cited at the time of visit. Allegation investigated: Neglect/Lack of Supervision resulted in resident injury.

Licensing Program Analyst (LPA) Iby Strong conducted an unannounced complaint visit to initiate a complaint investigation in the above-mentioned allegation. LPA met with Administrator Sadie Harness and discussed the purpose of the visit. On October 18, 2023, Community Care Licensing (CCL) received a complaint alleging neglect/lack of supervision resulted in Resident 1 (R1) injury. During the investigation, LPA Strong collected pertinent resident records as well as facility documentation and conducted interviews. According to R1’s Physician Report dated May 23, 2023, R1 is diagnosed with a major neurocognitive disorder, is confused, and disoriented and has sundowning behaviors. Records collected also show R1 has occasional hallucinations and delusions. R1 care plan dated May 4, 2023, does not reveal any additional status checks necessary for R1. According to the allegation, on October 18, 2023, R1 was observed to have a large bruise on the left arm. Interviews with multiple staff did not reveal what the direct source of the bruise was. Unsubstantiated Outside source interview revealed that medical provider could not verify that this injury was caused by another person. Interviews with residents did not reveal any information to corroborate the allegation. Based on LPA's interviews, and record reviews there is not a preponderance of evidence to prove alleged violations occurred, therefore the allegation is unsubstantiated.
