Licensing Program Analyst (LPA) Arian Golbakhsh conducted an unannounced, required Annual Inspection. The facility file and personnel report was reviewed prior to the visit. LPA was welcomed by, identified themselves to, and discussed the purpose of the visit to Administrator Luka Subotic. The facility's license shows a maximum capacity of six (6) non-ambulatory residents, four (4) of which may be bedridden. Bedridden may only reside in bedrooms #1-4. Additionally the facility is approved for a hospice waiver for six (6). During today’s inspection there were six (6) residents in care with three (3) currently receiving hospice services. LPA and Administrator Subotic toured the interior and exterior of the facility and inspected each room. The facility was clean, sanitary, and in good repair. Pathways were free of obstruction and slip hazards. Client bedrooms contained the required furnishings. Doors, windows, screens, toilets, and showers were in working order. Hot water temperature at taps accessible to clients were all compliant: Common bathroom sink adjacent to the kitchen was 110.3F. Extra linens and hygiene supplies were present, as well as Personal Protective Equipment. The facility had sufficient space and equipment to facilitate dining, laundry, visitation, meetings, and client activities. The facility contained at least two (2) days of perishable food, and at least seven (7) days non-perishable food, all safely stored. Cooking, dining equipment, and utensils were present. Knives and other similar sharp objects were locked and inaccessible to residents in care. [Continued on LIC 809-C] [Continued from LIC 809] No toxic chemicals or poisons were accessible to clients. Medications were labeled, as required, and stored in locked areas. No pools or bodies of water exist on the premises. Per Administrator Subotic, no firearms or ammunition are kept at the facility. Smoke and carbon monoxide detectors, emergency lighting, and facility telephone were all in working order. Fire extinguishers were serviced within the last 12 months, dated for February 2026. Last staff emergency drill was conducted on 5/29/26 for the topic of fire. First aid kit was complete and readily accessible. Required licensing postings were observed in visible areas of the facility. LPA interviewed one (1) staff and one (1) client, and interviews did not reveal any licensing or regulatory concerns. LPA reviewed facility records. The files reviewed by LPA contained required documents. Confidential records were stored in locked areas. No deficiencies were cited during the inspection. Type of visit: Post Licensing

Licensing Program Analyst (LPA) Nacole Patterson made an unannounced visit to the facility to conduct a post-licensing inspection. LPA was met by Licensee Luka Subotic and granted entry into the facility and discussed the purpose of the visit. A tour of the facility was conducted inside and out. LPA, accompanied by Licensee Luka Subotic, conducted a general overall inspection, which included, but was not limited to the following: Facility physical plant, food service, care and supervision, medication, record review and facility administration. Administrator Certificate for Luka Subotic expires on 12/24/25. During today's inspection, LPA observed the following: All indoor and outdoor passageways were free from obstructions. No pools or bodies of water were observed. Per Licensee Luka Subotic, there are no firearms or other dangerous weapons in the facility. Poisons and cleaning agents were observed to be inaccessible to clients in care. LPA toured each room in the facility. Rooms designated as client bedrooms had the required furnishings and sufficient lighting available for clients. Licensee provided each client with clean linen in good repair and sufficient hygiene products for personal use. The hot water temperature measured at 105-120 degrees F. The facility had multiple functioning carbon monoxide detectors and multiple smoke detectors. There was an operable fire extinguisher present in the facility. The facility was stocked with a two (2) day supply of perishable and seven (7) day supply of nonperishable food items. Medications were stored in a locked medication cart and were labeled and maintained in compliance with label instructions. Licensee has been approved for liability insurance and is awaiting the physical copy via mail. Licensee will send proof of liability insurance to LPA once the physical copy is received. Based on today's visit, no deficiencies were observed. Type of visit: Prelicensing

Licensing Program Analyst (LPA) Nacole Patterson conducted an announced Pre-Licensing visit. LPA was met by Applicant Luka Subotic and was granted entry into the facility. The purpose of today's visit was to inspect the facility to ensure that the facility is in compliance with California Code of Regulations, Title 22, Division 6. The fire inspection was completed on 5/2/24 and the facility is approved for 6 non-ambulatory residents, 4 of which may be bedridden in rooms 1-4. The facility has an approved Hospice waiver for 6 residents. During today's visit, LPA toured the facility and inspected all common areas, outside spaces, and a sampling of resident rooms across the facility. The facility was found to be clean, safe, and in good repair with no pathway obstructions. Private and common resident bathrooms were observed to be clean and the toilets and showers were found to be in working order. The facility's water temperature in a sampling of resident bathrooms and kitchen were measured at 117.2 and 118.5 degrees Fahrenheit. LPA observed locked storage areas where all hazardous and/or toxic chemicals were stored and secured. LPA observed locked storage for resident medications and files. Fire extinguishers were observed throughout the facility and found to be in compliance. A functioning carbon monoxide detector and smoke detectors were observed in the facility. No bodies of water were observed near or on the premises. LPA observed 7-day supply of non-perishable food. Required postings were observed in a common area of the facility. LPA reviewed the applicant's Infection Control Plan and Emergency Disaster Plan. LPA conducted Component III with the applicant. The topics discussed were continuing operation requirements, record keeping/reporting, and physical plant compliance. Pre-licensing is complete, and this facility has no deficiencies. It is recommended that this facility be licensed pending final review and approval.
