Licensing Program Analyst (LPA) Janira Arreola conducted an unannounced annual required visit. LPA met with Administrator, Bashar Myyas, who was informed of the purpose of the visit. At the time of the visit there were (2) staff and (5) residents present. The facility is a one story home with a detached garage and laundry room. (7) bedrooms and (5) bathrooms. No pools or firearms are housed at the facility. The facility is approved for (6) non ambulatory clients. Physical plant, floors, windows, and doors were observed. At the time of the visit (1) resident room's floor required cleaning. At the end of the visit LPA ensured the floor was cleaned for the resident. LPA also observed in the same resident room linens were being launder, by the end of the visit LPA ensured the bed had clean linens and absorption pads for the resident. Technical notes were documented to ensure resident rooms have clean floors and clean linens. Fixtures and furniture were in good repair were present. The outdoor area was free of hazards. LPA observed hygiene supplies for residents, PPE, clean linens. There are designated locked areas for sharps, dangerous objects, cleaning supplies, and medications. During the time of the visit LPA observed unlocked detached garage where the laundry detergent is being stored. A deficiency was documented and a plan of correction was created. The smoke detector and carbon monoxide are operational, and the hot water temperature 115.5F. There are emergency supplies such as emergency complete first aid kit. During the time of the visit the facility did not have flashlights or emergency lighting for residents. A technical note was documented and the administrator agreed to have flashlights at the facility. The emergency exits paths are clear and free of obstructions. LPA observed facility met the required 2-day supply of perishable and 7-day supply of non-perishable foods. During the time of the visit (1) staff interviewed who prepares resident meals stated no residents have a special diet. However, the records review revealed (4) of (5) current residents have dietary restrictions documented on their LIC602 Physician's report. A technical note was documented for the administrator to provide training to staff preparing meals. LPA reviewed the facility's emergency and disaster plan and infectious control plan. Required postings were found in the facility. The listed administrator has a current administrator's certificate. LPAs checked (3) staff files and (5) resident files. (1) resident file did not have the rental amount listed on the admission agreement, a technical note was documented and administrator ensured a signed copy of the admission agreement was on file with the rental amount. All resident records did not have a LIC621 Resident Personal Property and Valuables List. A technical note was documented for all residents to have the form completed and on file. No Personal and incidental funds are being handled by the facility. Central Stored Medications lists were on file and reviewed for all residents. Licensing Program Analyst (LPA) Abdoulaye Zerbo conducted an unannounced visit for a required annual inspection. The LPA was greeted by Caregiver Elvia Nazares, notified them of the purpose for the visit and was allowed to enter the facility to conduct the inspection. The Administrators Ebraheem Hamed and Bashar Myyas joined at a later time. Facility Overview: The facility is a single story building with five(5) residents bedrooms, 2 staff bedrooms, (5) bathrooms, a dinning room, an office, a kitchen and a detached garage. There is no gated pool and there are no firearms on the premises. Deficiencies were issued regarding a separate unit with 16 residents, using the same address as the licensed facility. Infection Control: LPA observed that hygiene and cleaning supplies were available for regular facility maintenance. The facility’s infection control plan was reviewed and found to meet department's requirements. Physical Plant: The physical plant, including floors, windows, and doors, was clean and well maintained. Fixtures and furniture were in good repair. Laundry equipment was in good working condition. Sharp and dangerous objects were securely locked in the kitchen and inaccessible to residents. The smoke detector and carbon monoxide detector were in good working condition. LPA observed fire extinguishers to be in compliance with the department's requirements. The water temperature was tested within regulations measuring at 116.4 F Continued 809-C...... Care & Supervision/Administration: Adequate staff were present to supervise residents during the visit. The administrator holds a current administrator’s certificate with expiration date of April 24, 2026 and a CPR certification with the expiration date of February 19th, 2025 Record Review and Resident/Staff Files: LPA reviewed files for three(3) staff members, confirming criminal clearance, updated training, and CPR/First Aid certification. Three (3) residents' files were reviewed and contained all required documentation. LPA observed first kit to be locked and inaccessible to the residents in care. The residents and staff files were kept locked and inaccessible to unauthorized individuals Health-Related Services/Incidental Medical Services: All residents' medications were securely locked in a cabinet and located in the office area. LPA reviewed medications for three(3) residents, confirming that all medications were listed and accounted for. Disaster Preparedness: LPA reviewed the facility’s emergency and disaster plan, including documentation of the last emergency drill conducted on May-01-2025, which met the department's requirements. All facility exits were clear of obstructions. No deficiencies were cited during the visit. Type of visit: Case Management - Deficiencies

On 05-20-25, Licensing Program Analysts (LPAs) Abdoulaye Zerbo and Janira Arreola conducted a visit to the facility to conduct a case management . LPAs were greeted and granted entrance by Caregiver Sable Smith. LPAs identified themselves and discussed the purpose of the visit. Licensee Ebraheem Hamed joined the visit at a later time. LPAs conducted a health and safety check by conducting tour of the facility. The facility is a single story building with five (5) residents bedroom, two (2) staff rooms, two(2) living rooms, an office, a dining room and a detached garage. During the visit, LPAs observed an additional unit on the property, which is a single story building with eight(8) bedrooms, four (4) bathrooms, a living room, a dining room and an outdoor area, with 16 residents residing on the premise. LPA conducted interviews with Licensee Ebraheem Hamed who stated the additional unit is an independent living facility that does not have any relationship with the licensed facility. LPAs advised licensee that since the other unit is at the same address as the licensed facility with 16 residents, the facility is over capacity. A citation will be issued. During the visit, LPAs interviewed residents and staff, and information obtained revealed the Administrator is having a romantic relationship with resident (R1) since last year. The licensee stated they were not aware of any relationship happening between the administrator and the resident. The licensee terminated the Administrator with an immediate effect of 05-20-25 and the Administrator vacated the facility at the time of the visit. Type of visit: Prelicensing

Licensing Program Analyst (LPA) Janette Romero made an announced visit to the facility to conduct a pre-licensing evaluation. The facility submitted a change of ownership application to the Centralized Applications Bureau (CAB). LPA met with Licensee, Ebraheem Hamed. On 4/22/2024, the facility was granted a fire clearance for a total capacity of six (6) non-ambulatory elderly residents. LPA toured the facility's interior and exterior with Licensee Hamed. The facility is made up on a one-story home with six resident (6) bedrooms, five (5) bathrooms, two (2) living rooms, a kitchen, dining room, activity room, and detached car garage. Indoor and outdoor passageways are free of obstruction. There are no bodies of water on the premises. Licensee Hamed tested one of the smoke alarm/carbon monoxide detectors and LPA found it to be operational. The facility has a central heating and air conditioning system installed with a central panel located in the living room to control entire house. All resident bedrooms are adequately furnished with a bed, chair, closet, appropriate sized linens, adequate lighting, and a smoke alarm/carbon monoxide detector. All bathrooms have a working toilet, wash basin, and have toilet paper, paper towels, and soap readily available. Three of the facility's bathrooms have showers. Bathrooms with showers have grab bars near the toilet and in the showers. LPA tested three (3) of five (5) bathroom's water temperatures, which measured at 104.9, 106.1 and 108.3 degrees Fahrenheit. LPA toured the kitchen and observed the facility has a sufficient supply of dishes, glasses, utensils, pots and pans for the approved capacity. Knives/sharp instruments are secured in a kitchen drawer. Medications are centrally stored in the administrator's office. Cleaning solutions and disinfectants are secured in a locked kitchen cabinet. The facility has a confidential storage area designated for staff and resident records. Emergency phone numbers, personal rights and Long Term Care Ombudsman information are posted near the living rooms. During today's visit, LPA did not observe any issues or concerns, and there are no corrections required. Final approval of licensure will be granted by the CAB analyst.
