Licensing Program Analysts (LPAs) Ivashia Wright and Mia Lankford conducted an unannounced annual required visit. Upon entry, LPAs was greeted by Loren Mc Elvain, Administrator, and informed them of the purpose of the visit. At the time of the visit, there were two (2) staff member and four (4) residents present. Facility Overview: The facility is a one-story home that consists of five (5) resident bedrooms, one staff bedrooms, three (3) bathrooms, a kitchen and dinning area, a living room area, laundry room, and a patio and yard with sufficient seating and space for activities. There are no pools or known firearms on the premises. Physical Plant: The physical plant, including floors, windows, and doors, was clean and well maintained. Fixtures and furniture were in good repair. The outdoor area was free of hazards. Laundry equipment was in good working condition. Sharp and dangerous objects were securely locked and inaccessible to residents. Both the smoke detector and carbon monoxide detector were operational, and the hot water temperature was 105.1°F. Fire extinguishers located at kitchen and hallway expire 1/16/2027. Food Service: The facility’s kitchen was clean and equipped to prepare food. The facility maintained the required two-day supply of perishable foods and a seven-day supply of non-perishable foods Continued on LIC809-C.... Care & Supervision/Administration: Adequate staff were present to supervise clients during the visit. The administrator holds a current administrator’s certificate. Record Review and Resident/Staff Files: LPAs reviewed files for two (2) staff members, confirming criminal clearances, updated training, and CPR/First Aid certification. Four (4) residents files were reviewed and contained all required documentation. Health-Related Services/Incidental Medical Services: All resident medications were securely locked. LPAs reviewed medications for four residents, confirming that all medications were listed on the Medication Administration Record (MAR) and accounted for. Disaster Preparedness: LPAs reviewed the facility’s emergency and disaster plan, including documentation of the last fire/earthquake drill conducted on 2/15/2026, which met department requirements. All facility exits were clear of obstructions. No deficiencies were cited during the visit. Licensing Program Analyst (LPA), Armando Perez made an unannounced visit to the facility for the purpose of conducting a required annual inspection. The LPA was granted entry by staff to conduct the inspection and met with administrator, Loren McElvain. The LPA informed the Administrator of the purpose for the visit. The inspection included the following. LPA toured the facility inside and outside. LPA observed the facility to be clean and in good repair. The facility consists of five (5) resident bedrooms, one staff bedrooms, three (3) bathrooms, a kitchen and dinning area, a living room area, laundry room, and a patio and yard with sufficient seating and space for activities. The home is maintained at a comfortable temperature for the clients. Lighting is sufficient for safety and comfort. Water temperature measured within regulation. Laundry facilities and a locked cabinet is present for storing laundry soap and other chemicals in the laundry room. All outdoor and indoor passageways are free of obstruction. Emergency lighting is available. There is a telephone working at this location. The LIC 610, emergency disaster plan is maintained. There are no firearms at this home and no bodies of water observed. LPA began review of client records. six (6) records were reviewed. LPA reviewed for identification and emergency information, admission agreement, medical assessment, and TB test results, needs and service plans, placement, functional assessment, centrally stored medication/destruction records, safeguard for personal property/valuables, and personal rights notification. LPA observed client records to be available and complete. LPA reviewed employee records- four (4) records were reviewed. LPA reviewed employee records for first aid certification, criminal record clearance or an exemption, health screening and TB test results, employee rights, training verification, and current administrator certification; expiration date 12/02/2025. LPA observed personnel records to be available and complete. LPA observed facility kitchen had the ability to prepare food in clean environment and possessed equipment in good working condition. Food supply meets the requirement of one (1) week supply of nonperishable and two (2) day supply of perishables. Emergency food and water supply is present. There is a locked location for sharps in the kitchen. Medications are centrally stored. There is a locked hallway room allocated for medication storage. Centrally stored medication and destruction logs are maintained. Medications reviewed appear to have been dispensed accurately. LPA made observation throughout the inspection process to assess if the facility remains in conformity with the State Fire Marshall regulations. Smoke detectors and carbon monoxide detectors were tested and found to be operational. Fire extinguishers was serviced on 07/26/2024. LPA observed fire drills are conducted yearly at the facility with the last drill on 02/20/2025. Administrator stated it was difficult to include the clients so it was conducted yearly. LPA informed Administrator that the drill is for staff and does not need to include the clients. Administrator will begin to conduct emergency drills quarterly. A deficiency was issued. Based on the information received during this visit today in the areas reviewed, there is one deficiency that is being cited per Title 22, Division 6 of The California Code of Regulations. Licensing Program Analyst (LPA) Javina George conducted an unannounced visit to the facility to for the purpose of a 1 year required visit. LPA was greeted and granted entry by Caregiver Jocelyn Esurena, where LPA explained the purpose of today's visit. At the time of the visit was (1) staff and (4) residents present. All staff were observed to have obtained proper fingerprint clearance and were associated to the facility. The facility has an approved hospice waiver for three (3) and there are currently two (2) residents receiving hospice services. Physical plant: LPA conducted a tour of the interior and exterior of the facility. The home is a single story home with five (5) bedrooms and (3) Jack and Jill style bathrooms. LPA observed for the fence in the backyard to be broken with missing slats. Per the Administrator, the fence is in process of being repaired, this was verified by reviewing receipts for materials purchased. The estimated completion date for the fence within the next two weeks. LPA observed inside the kitchen there were missing drawers, exposing the pots and pans in the cabinet below. Per the administrator Loren the drawer is included in the construction and will be completed within the next two weeks. Proof of repairs are to be submitted to the department by 5pm on 4/23/24, as it is contingent upon the weather and wind conditions. The medications are locked cabinets in a secured room/entryway between the facility and the Administrator's home. Additional hazardous chemicals and sharps objects are inaccessible to residents in care. The utilities (gas, water, electric) were observed to be operable. The facility's carbon monoxide and smoke detectors were tested and were observed to be operable. The hot water was tested and measured to be within regulatory limits of 105-107 degrees Fahrenheit. The facility has a mitigation plan on file that was submitted to the department on 4/28/21. The facility has all the required postings, posted on the wall inside the hallway, upon entry to the facility. The facility does have a live in caregiver that resides in room #3. LPA reviewed two (2) staff and two (2) resident files, the required documentation was present. However for Resident #1 (R1) the physician's report has the last page missing verifying the medical assessment was completed. The administrator contacted R1's responsible party for the complete document. Based on today's visit no deficiencies were observed, as the needed repairs are already in process.
