On June 30, 2026, Licensing Program Analysts (LPAs), Jarred Torres and Venus Mixson, arrived at the facility unannounced to conduct an annual inspection, and met with Administrator (AR), Anhtuan Dang. A facility file review was conducted at the regional office and additional records were requested and reviewed at the facility. Their files were stored in a secure location. The facility is licensed for six residents and was operating at a census of one resident. During this inspection, the resident was at their day program. So, they could not be interviewed during this inspection. LPAs toured the facility along with AR, and made observations pertaining to the annual inspection. LPAs inspected the facility inside and outside. There were no obstructions to the indoor and outdoor passageways at the time of this visit. Additionally, there was an empty pool which was barricaded with a locked gate. During the tour of the premises, LPAs observed the facility's phone to be operational. The facility's phone number is 951-243-8179. The LPAs observed the resident's bedrooms which was equipped with the required furniture as stated in Tittle 22 of the California Code of Regulations (CCR). The furniture in all areas of the facility was observed to be in good condition. The facility's appliances were observed to be operational at the time of this visit. Required postings regarding safety, personal rights, and emergency exits were posted in the home. Sharp items were kept locked and inaccessible to the resident in care. The facility's cooling and heating system is operational and the air temperature was at 73 degrees Fahrenheit (F) and the hot water was measured at 109 degrees F. Continued on LIC 809-C... LPAs observed the medications to be locked and inaccessible to the resident in care. The medication supply was sufficient for their current census. There were no discrepancies with the centrally stored medications. In the kitchen, the food supply of non-perishable and perishable foods consisted of, but not limited to, bread, eggs, frozen meats, vegetables, water, soups, and fruits. The facility goes grocery shopping once every week to maintain an appropriate food supply for the resident. The facility has a designated area for storing activity supplies, and activities are conducted inside and outdoors. Telephone numbers and floor plans were posted in the facility. Client files and staff files were reviewed and no deficiencies were observed. The files were securely stored. The administrator had a valid administrator's certificate on file. LPAs reviewed the emergency disaster plan and fire clearance. The last emergency drill was on June 17, 2026. The fire alarms and carbon monoxide detectors were tested and operational. The fire extinguishers were in good condition and were inspected on May 12, 2026. The emergency disaster plan meets the department's standards. LPAs inspected the bathrooms and observed the hand washing stations to be stocked with soaps, bath tissue, and single-use hand towels. Furthermore, the bathrooms are free of dust and debris. Additionally, the facility has an approved infection control plan in their files. During this annual inspection, the LPAs did not observe any health, safety, or personal rights risks to clients in care. Type of visit: Case Management - Annual Continuation

Licensing Program Analyst (LPA) Janira Arreola conducted an unannounced visit for the a continuation of the required annual. LPA met with Licensee, Anhtuan Dang who was informed of the purpose of the visit. During the time of the visit there was (1) staff, (1) volunteer, and (0) residents present. Resident was at their day program. (1) staff interview was conducted during the time of the visit. LPA checked the carbon monoxide detector during the time of the visit and the hot water temperature was read at 116.9F. LPA reviewed the facility's COVID-19 plan which met the department requirements. LPA reviewed the liability insurance which is current, and the staff and client records. LPA checked (3) staff files and (1) resident file. The files had all required paperwork, criminal record clearances, and training. LPA observed the current administrator's has a valid administrator's certificate. LPA reviewed the staff schedule and observed adequate staff coverage. The client medication and MARS were reviewed. The medication administration was accounted for (1) resident. No health and safety issues were noted at the time of the visit. No deficiencies were cited at the time of the visit. Licensing Program Analyst (LPA) Janira Arreola conducted an unannounced visit for the required annual. LPA met with Licensee, Antuan Dang who was informed of the purpose of the visit. LPA was informed by licensee that client would be retuning from their day program and would be attending an appointment at 3:00pm. LPA conducted a walk through of the home. LPA observed the physical plant. LPA observed the client and staff room. No firearms or pool is being kept at the facility. LPA observed the locked medication, sharps and cleaning supplies were locked and inaccessible to clients. LPA check the food supply which met the department requirements. LPA observed the outdoor space was free of hazards. LPA checked the emergency supply and first aide kit. LPA reviewed the facilities last fire drill April 16, 2024 was conducted on and reviewed emergency and disaster plan. No health and safety issues were noted at the time of the visit. Due to time constraints LPA will return on another date to complete the inspection. No deficiencies were cited at the time of the visit.
