Type of visit: Required - 1 Year

On 8/10/2026 at approximately 09:30 AM, Licensing Program Analyst (LPA), Angelica Segovia conducted an unannounced annual visit to the facility. LPA was greeted by the Administrator, Alma Guinto and stated the reason for their visit. LPA asked for the census, Staff/Resident Roster, and Liability Insurance. LPA conducted a physical plant tour at approximately 11:15 AM and the following was noted: The facility is a single-story building with four (4) bedrooms and two (2) bathrooms currently occupying two (2) residents. There is a designated staff room. The facility has an approved fire clearance for three (3) non-ambulatory residents and two (2) ambulatory residents. Common areas: The living room and dining room were observed to be neat, clean, and organized. The rooms were observed to be properly furnished and in good repair. The facility maintains a comfortable temperature at 76°F. LPA observed a fire extinguisher to be located near the kitchen and dated 03/09/2026. LPA observed required postings such as Long-Term Care Ombudsman, Emergency Disaster Plan, and Personal Rights to be located alongside the dining area. A working telephone was observed. LPA observed the fireplace to be covered and inaccessible to residents. Kitchen: The kitchen was observed to be clean and free from pests. Sufficient supplies of seven (7) day nonperishable foods and two (2) day perishable foods were observed. Knives/sharps were observed to be kept in a locked kitchen drawer. Kitchen appliances were observed to be working and in proper condition. (continued on LIC 809-C) Bedrooms: The residents’ rooms are adequately furnished with appropriate furniture and lighting system. Hallways/passageways are lighted appropriately. Extra linens/covers were observed to be stored in cabinets located within the hallway’s passageway. LPA observed operational auditory alarm signals to be located in each exists of the facility. Bathrooms: The bathrooms were checked for cleanliness and proper operation. The hot water temperature was measured. LPA observed appropriate grab rails and slip-resistant mats to be in proper condition. Backyard: The backyard of the facility is equipped with a designated shaded area with outdoor furniture for residents. There is no body of water located at the facility. Garage: The garage can be accessed from inside of the facility. The garage was observed to be used for storage purposes with an operational auditory alarm observed. An extra refrigerator/freezer with additional food for residents was observed. Laundry Room: The laundry appliances were observed to located within the garage. LPA observed cleaning solutions and toxins stored appropriately within the laundry room and inaccessible to residents. The laundry appliances were observed to be in proper condition. Medications: The medications were observed to be kept in a locked cabinet located in the kitchen. First-aid kit observed to be equipped with but not limited to bandages, scissors, digital thermometer, tweezer and First-Aid Manuel. Smoke detectors and carbon monoxide observed to be working properly and were tested. Residents/Staff Records: LPA conducted a complete file review of resident records. Resident records appeared to be complete and updated. Staff records: LPA conducted a complete file review of three (3) staff records. Staff records appeared to be complete and updated. There were no immediate health and safety hazards observed during the day of inspection. On 08/04/2025 at approximately 10:00 AM, Licensing Program Analyst (LPA), Angelica Segovia conducted an unannounced annual visit to the facility. LPA was greeted by the caregiver and stated the reason for their visit. The Administrator, Alma Guinto arrived shortly after to assist with today’s visit. LPA asked for the census, Staff/Resident Roster, and Liability Insurance. LPA conducted a physical plant tour at approximately 12:00 PM and the following was noted: The facility is a single-story building with four (4) bedrooms and two (2) bathrooms currently occupying two (2) residents. There is a designated staff room. The facility has an approved fire clearance for three (3) non-ambulatory residents and two (2) ambulatory residents. Common areas: The living rooms and dining room were observed to be neat, clean, and organized. The rooms were observed to be properly furnished and in good repair. The facility maintains a comfortable temperature at 76°F. LPA observed a fire extinguisher to be located near the kitchen and dated 03/07/2025. LPA observed required postings such as Long-Term Care Ombudsman, Emergency Disaster Plan, and Personal Rights to be located throughout the common areas. A working telephone was observed. Kitchen: The kitchen was observed to be clean and free from pests. Sufficient supplies of seven (7) day nonperishable foods and two (2) day perishable foods were not observed. LPA observed expired food within the refrigerator and mold growth. Knives/sharps were observed to be kept in a locked kitchen drawer. The cleaning solutions/toxins were observed to be kept underneath the kitchen sink. LPA observed one of the locked latches to be broken. Kitchen appliances were observed to be working and in proper condition. (continued on LIC 809-C) Bedrooms: The residents’ rooms are adequately furnished with appropriate furniture and lighting system. Hallways/passageways are lighted appropriately. Extra linens/covers were observed stored within storage closets located in the hallway’s passageway. Bathrooms: The bathrooms were checked for cleanliness and proper operation. The hot water temperature was measured within regulations at 106.2 °F. Garage: The garage can be accessed from inside of the facility and was observed to be located towards the back of the facility. LPA observed the garage to be used for storage purposes and extra food. Laundry Room: The laundry appliances were observed to be located in the garage. LPA observed cleaning solutions and toxins stored within locked cabinets above the laundry appliances and inaccessible to residents. The laundry appliances were observed to be working and in proper condition. Backyard: The backyard of the facility is equipped with a designated shaded area with outdoor furniture for residents. LPA observed locked shed. There is no body of water located at the facility. Medications: The medications were observed stored in a locked cabinet located in the kitchen. LPA observed one (1) out of the two (2) residents’ medication to be kept in separate containers and not within their original containers. First-aid kit observed. Smoke detectors and carbon monoxide observed to be working properly and were tested. Residents/Staff Records: LPA conducted a complete file review of resident records. Resident records appeared to be missing reappraisals for both residents. Staff records: LPA conducted a complete file review of staff record. Staff records appeared to be missing staff training on Dementia. There were no other immediate health and safety hazards observed during the day of inspection. Citations issued please see 809-D. Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Gary Tan conducted an unannounced visit at this facility for a One (1) year Required visit. LPA met with the administrator Alma Guinto and explained the purpose of the visit. A tour of the physical plant was conducted at 9:18 AM. The facility has four (4) bedrooms and two (2) bathrooms currently occupying two (2) residents on two (2) shared and one (1) private room. One (1) bedroom is designated for staff use. The facility fire cleared for three (3) non-ambulatory and two (2) ambulatory residents. There is only one entrance being utilized at the facility. Screening area is located immediately upon entrance. Sign in sheet, hand sanitizer, gloves and masks are available. The facility had submitted and approved Mitigation plan and Infection plan. Physical environment was checked for cleanliness and condition. Walls, windows, ceilings, floors and floor coverings, and doors were checked, the following was noted: Living and dining room furniture were also checked. The living room is neat and clean along with the family room. The facility maintains a comfortable temperature at 75°F. The carbon monoxide and smoke detector are tested and observed to be operational. Smoke detectors are hardwired and interconnected. Fire extinguisher was observed to be full and last bought on 03/17/24. The backyard There is a tool shed at the backyard that observed to be locked during visit. There is a make shift extension accessible through the kitchen with furniture for the residents and there is a shaded area at the front porch. There is no body of water at the facility. (continued on LIC 809-C) (continued from 809-C) The Garage is currently being used as frozen and emergency food, PPE and old equipment storage. It is also being used as the laundry area and laundry detergents, pesticides and other cleaning agents storage. Food Service/Kitchen area was sufficiently stocked with two (2) days perishable and seven (7) days non-perishable food. Frozen foods are properly wrapped and stored appropriately. Food storage and preparation areas are clean and inaccessible to pests. Dishwashing liquids are kept under the locked sink cabinet. The Clients' rooms are adequately furnished with appropriate furniture and lighting system. Clients have sufficient amounts of personal hygiene products on each bathroom which is provided by the licensee. Staff Rooms: Staff room was locked. No medications are observed in the staff room. The bathrooms were checked for cleanliness and proper operation. LPA observed the appropriate grab bars for each toilet, bathtub and shower. The hot water temperature measured at a range of 105.2°F to 109.6°F. There is sufficient supply of clean linen available in stock at the cabinet. Medications: LPA observed medication in the kitchen cabinet to be locked and inaccessible to residents. Medications are listed on the centrally stored medication and destruction record. Complete first aid kit is readily available. Client records: Client records are reviewed. Client records appear to be complete and updated. Staff records: LPA conducted a complete file review of staff records. Staff records appeared to be complete and updated. Disaster drill was last conducted on 07/16/24.
