On today's date Licensing Program Analyst (LPA) William Vanegas conducted an unannounced visit for the purposes of conducting an annual inspection. Upon arrival LPA was greeted and granted entry to the facility by Administrator (AD) Tina Menendez. LPA explained the purpose of the visit, and began a tour of the facility accompanied by AD. LPA observed the following. AD has a valid Administrator certificate valid from March 23, 2026 through March 22, 2027. This is a one storied home with five bedrooms, five and a half bathrooms five of which are private resident bathrooms, and one of which is a guest/staff half bathroom, and a detached two car garage. LPA observed kitchen area to be clean and free of any mildew and debris. LPA observed there to be a refrigerator, dishwasher, gas stove, microwave, washer, and dryer. All appeared to be in good repair and tested operational. LPA observed there to be a two day supply of perishable food and a seven day supply of non-perishable food. LPA observed a sufficient amount of emergency water on hand. LPA observed all resident rooms to be clean and free of any hazards or debris. LPA observed all resident rooms to have all required furnishings including a bed, clean linens in good repair; meaning no strains or tares, a reading lamp, a chair, a chest drawers, and enough storage space for resident to store their personal belongings. LPA Observed resident bathrooms to be clean and free of any mildew or debris. Water faucets, and toilets tested to be operational, bathrooms had all required furnishings such as grab bars, slip resistant floor matts, and a shower chair. Hot water temperature tested between 105.2 and 108.9 degrees Fahrenheit. CONTINUED ON LIC809C LPA observed all fire extinguishers to be fully charged, and up to date. LPA observed all smoke detectors and carbon monoxide detectors to be in good repair and tested operational. LPA observed first aid kit to have all required items such as a thermometer, bandages, adhesive tape, tweezers, scissors, and a first aid manual. LPA toured the outside of the facility with AD and observed the following. There is an outdoor shaded sitting area, the backyard is large enough to participate in outdoor activities upon resident request, no hazards or obstructions were observed along the exit routes. Side gates are locked and self latching. LPA reviewed three staff files and five resident files, all files (staff and resident) had all required documents. Staff training was up to date and documented correctly. LPA reviewed medications with AD and observed no discrepancies. LPA observed medications documented correctly, and per LPA review all medications are being administered per physicians orders. Based on today's observations during the inspection. Per title 22 chapter 8 division 6 no deficiencies will be issued on today's date. On February 24th, 2025 Licensing Program Analyst (LPA) William Vanegas conducted an unannounced inspection for the purposes of an annual inspection. Upon arrival LPA Vanegas was greeted and granted entry by Care Giver (CG) Mayra Bermudez. LPA Vanegas explained the purpose of the visit and began a tour of the home, and observed the following. This is a one storied home with five bedrooms with five and a half bathrooms. Five of which are used for resident use and the half bathroom is used for guest and employees. The facility also has a detached single car garage. LPA Vanegas observed kitchen area to be clean and free of any mildew and debris. LPA Vanegas observed a two day supply of perishable food and a seven day supply of non-perishable food. LPA Vanegas observed there to be a dishwasher, gas stove, microwave, washer and dryer; that all appeared to be in good repair, and tested to be operational. LPA Vanegas observed resident bathrooms to be clean and free of any mildew or debris. Water faucets and toilets tested to be operational. Bathrooms had all required furnishings such as a shower chair, grab bars, and slip resistant matts. Water tested between 109.3 and 113.5 degrees. LPA Vanegas observed all smoke detectors and carbon monoxide detectors to be operational and in good repair. LPA Vanegas observed fire extinguisher to be fully charged and up to date. LPA Vanegas observed all resident rooms to be clean and free of any debris. All resident rooms had required furnishings such as chest drawers, a lamp, screened windows, a bed, and clean linens in good repair meaning no strains or tears. LPA Vanegas observed outside of the facility to be clean and free of any obstructions along the emergency exits. Backyard is clean and provides a shaded sitting area, and is big enough to participate in outdoor activities. CONTINUED ON LIC809C LPA Vanegas reviewed four staff files and four resident files all files (Staff and Resident) Had all required documentation, and staff training was all up to date and documented. LPA Vanegas observed sharps, toxins, and medications all to be locked, and inaccessible to residents in care. LPA Vanegas observed medications to be documented and administered per physicians orders. Based on observations made during today's visit no deficiencies will be cited per title 22 chapter six of the California Code Of Regulations. On today's date, Licensing Program Analyst (LPA) LPA Rosie Quiroz conducted an unannounced visit for the purpose of conducting a required Annual inspection. LPA was greeted and granted entry into the facility by Caregiver 1 (CG1). LPA Quiroz met with Administrator (AD) Tina Menendez and House Manager (CM) Christine Lezsak and discussed purpose of today's visit. This is a a Residential Care Facility for the Elderly, licensed to provide services to age range 60 and over, approved for capacity of (6) six resident which (5) five are Non-Ambulatory and (1) one Ambulatory; Bedroom 5 approved for Ambulatory only; Rooms 1-4 approved for Non-Ambulatory; Hospice waiver approved for 2 hospice residents. There are currently four (4) residents in care at this time, of which (1) one is receiving hospice care services. There are no active COVID-19 cases in the facility at this time. AD Tina Menendez has an Administrator certificate with expiration date of March 22, 2024. AD Menendez indicated currently in process of renewing Administrator certificate, and agreed to submit copy of renewal to Community Care Licensing upon completion. LPA Quiroz along with (AD) Menendez and (HM) Lezsak toured the interior and exterior of the facility. During today's inspection tour, LPA Quiroz observed 2 of 4 Residents in the living room area watching television with staff supervision and 2 of 4 residents in their bedrooms resting with staff conducting frequent round checks as needed. LPA Quiroz interacted and interviewed with Caregivers and residents during today's visit. LPA Quiroz inspected resident's bedrooms and bathrooms. The water temperature in resident's bathrooms were recorded to be within normal limits. LPA Quiroz inspected resident's bedrooms and appeared to be clean. Facility temperature in resident's bedrooms and throughout the facility was recorded to be within normal limits. LPA Quiroz observed the emergency and disaster and evacuation plan. Facility has supply of emergency food, water and PPE in the garage area readily available for staff and residents in care. 4 of 4 Fire extinguishers observed last serviced on March 1,2024. LPA Quiroz observed functional and operational washer and dryer in the laundry room area. Medications, sharps and disinfectants were observed to be locked and secured. CONTINUED ON LIC 809-C PAGE... CONTINUED...LPA Quiroz toured the outside of the facility and observed seating and shaded area in the backyard and front porch area for residents and visitor's enjoyment. CONTINUED...LPA Quiroz reviewed 4 of 4 resident records and 7 of 7 personnel files. No citations issued during today's visit. During today's visit, LPA Quiroz provided Consultation on Title 22 and Infection control.
