On October 9, 2025, at 8:00 AM, Licensing Program Analyst (LPA) Edward Kim conducted an unannounced required 1-Year annual visit using the CARE Inspection Tool. Upon arrival at the facility, LPA Kim met with Administrator (ADMIN) Georgeta Firuta and explained the purpose of the visit. The facility is licensed to operate for six (6) non-ambulatory and have a hospice waiver for six (6) residents. The facility is a single-story building located in a residential neighborhood. It consists of the following: five (5) resident bedrooms, three (3) staff bedrooms, two (2) bathrooms, office space, living area, dining area, kitchen, attached 2-car garage, and an outside covered patio area. LPA Kim toured indoor and outdoor of the physical plant with ADMIN Firuta. There are no obstructions or bodies of water on the premises. There is a fountain in the front yard being used as a decoration peace. It does not contain any water in it nor is it connected to a water source. All rooms were inspected. Beds and bedding supplies were in good condition, adequate lighting was provided, storage for each resident’s personal belongings was observed. Bed linens, comforters, and bath towels were adequately stocked at the time of visit. All bedrooms were inspected: Resident Room 1, Resident Room 2, Resident Room 3, Resident Room 4, and Resident Room 5. Bathrooms were found to be within Title 22 regulations and were clean and operational. The water temperature measured between 107.4 degrees F and 109.4 degrees F. A comfortable temperature of 76 degrees F was maintained in the facility. LPA Kim observed the facility to be sanitary and appropriately furnished at the time of visit. Storage areas for personal hygiene, cleaning supplies, toxins, and sharps objects were stored and not accessible to residents. Evaluation Report Continues on LIC 809-C During the visit, LPA Kim observed the facility's infection control practices, plan of operation, and screening protocols for visitors, staff, and residents. LPA observed the facility has a 30-day supply of Personal Protective Equipment (PPE). The kitchen was inspected and there is a two-day supply of perishable and seven-day supply of non-perishable food available and maintained properly. Emergency food, water, and supplies are stored in the kitchen cabinets. The facility has one (1) fire extinguisher that is charged and mounted in the living area. All smoke/carbon monoxide detectors were operable and in working order. The facility conducts Fire/Safety Drill quarterly and was last conducted on September 2, 2025. A working telephone (714-844-2115) remains available. First Aid Kit contained all the necessary elements. LPA conducted an audit of resident files (R1-R4), staff files (S1-S4) and medication and medication administration record that were all in order and complete. LPA Kim conducted 4 resident interviews and 1 staff interview. No deficiencies were cited during the inspection visit. Licensing Program Analyst (LPA) Lydia Martinez made an unannounced visit for the purpose of conducting a Required 1 Year inspection. LPA met with Caregivers Yocelin Verastica Meza and Francis Montiel Mata and purpose of visit was shared. Administrator (AD) Georgeta Firuta arrived shortly after. AD's Certificate expired on 08/21/2024. AD stated she has submitted required documentation to renew certificate. LPA Martinez, along with staff toured the inside and outside of the facility. Facility currently has four residents, of which one resident is in the hospital. There are no residents receiving Hospice care. LPA observed residents were having lunch. Residents were observed clean, content and well taken care of. LPA observed the facility to be clean and in good repair. The home is maintained at a comfortable temperature. Lighting is sufficient for safety and comfort. Backyard has a covered patio for outdoor activities and sufficient seating for residents and visitors. Bedrooms were observed to be spacious and easily accommodate furnishings such as lamps, chair, dresser and a bed. Bathrooms were observed to be clean and have a supply of soap and paper towels. Hot water temperature was within regulatory requirements. Linen and hygiene supplies were stocked. Emergency Phone Numbers and Exit Plan were reviewed. Food prep area is clean and organized. Food supply meets the requirement of one (1) week supply of non-perishable and two (2) day supply of perishables. Smoke detectors and carbon monoxide detectors were found to be operational. Fire Extinguishers were charged and mounted. Stove burners, dishwasher, microwave, washer, and dryer are operational. Chemicals and sharps are made inaccessible to the residents. Medication was observed locked in a kitchen cabinet. Medications reviewed appear to have been dispensed accurately. LPA observed the Emergency Disaster Plan posted. LPA reviewed 4 resident files and 2 staff files; and interviewed residents and staff. No documentation of Emergency Drills conducted. Based on observations made during today's visit, no deficiencies are being cited. LIC9102 Advisories were issued.
