Type of visit: Required - 1 Year

On September 3, 2025, Licensing Program Analysts (LPAs) Brandon Lopez and Garlli Tat made an unannounced visit to conduct the required annual inspection. LPAs were greeted and granted entry by care giving staff after explaining the purpose for the visit. Licensees (LI) Maria Emeterio and Jay Parungao were notified via telephone and later arrived to assist with the inspection. LPAs observed that Maria Emeterio has a valid Administrator certificate which expires on August 15, 2026. The facility is a Residential Care Facility for the Elderly (RCFE) licensed for six non-ambulatory residents, of which one may be bedridden, and has a hospice waiver for five. The facility is a single story home with six private resident bedrooms, one staff bedroom, three and a half resident bathrooms, a living room, a dining room, a kitchen, a sitting area, a beauty shop, and an attached two car garage. LPAs accompanied by the LI, conducted a tour of the interior portion of the facility. On today's visit, LPAs observed four residents in care and two care giving staff present. LPAs observed the See Something, Say Something poster (PUB 475) mounted on the wall by the dining room. LPAs inspected the six resident bedrooms, and they were observed to be free of any hazards. LPAs observed the resident bedrooms had the required furnishings of a bed, a chair, a chest of drawers, and a lamp. All resident beds had clean linens and blankets. LPAs observed additional linens are stored in the hallway closet. LPAs inspected the three and a half resident bathrooms. Resident bathrooms are clean. Bathrooms are equipped with grab bars and non-skid floor mats. Faucets and toilets were operational. Hot water temperature measured between 106.9 and 120 degrees Fahrenheit. LPAs observed the staff bedroom to be kept locked and inaccessible to residents in care. LPAs observed the kitchen has a two day perishable and seven day nonperishable food supply on hand. LPA observed kitchen appliances to be clean and operational. LPAs observed the four burner gas stove lights unassisted. LPAs observed kitchen knives are stored in a locked kitchen cabinet. CONTINUED ON LIC809-C LPAs observed a fire extinguisher mounted on the wall in the kitchen and it was observed to be charged and purchased on August 12, 2025. LPAs tested the wired smoke detectors and carbon monoxide detectors which tested operational. LPAs observed the facility conducted their last emergency disaster drill on June 30, 2025. The centrally stored medication is kept in a locked cabinet by the kitchen. The First Aid kit is stored in the locked cabinet and it was observed to have all the required components. LPAs observed the beauty store to be free of any hazards. LPAs observed the facility has a three day emergency food and water supply stored in the beauty store. LPAs observed the door leading to the attached two car garage is kept locked and inaccessible to resident in care. The garage is used for storage and laundry. LPAs observed chemicals and toxins to be stored in the garage. LPAs and LI conducted a tour of the exterior portion of the facility. LPAs observed the exterior portion to be clear of obstructions and hazards. LPAs observed a shaded outdoor seating area with furniture for resident use. The perimeter gate is self-latching and can be opened in an evacuation. There are no bodies of water on the premises. LPAs reviewed all four resident files. All the required documentation were present and current in the resident files reviewed. LPAs reviewed residents’ medication and medication records. LPAs reviewed two staff files. All staff are background cleared and associated to the facility. Based on today's observations, there are no deficiencies being cited per Title 22 of the California Code of Regulations. Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Rose Ruppert made an unannounced visit to the facility today to conduct an Annual Required Evaluation. LPA was greeted and granted entry by Staff #1 at 8:00 AM. During today’s visit, LPA met with "Cherry" Maria Rosario Emeterio, Administrato and Jay Parungaor. The facility is a single story, seven bedroom residential home with an approved fire clearance of six non-ambulatory of which one may be bedridden in bedroom seven. The facility has a hospice waiver for five residents.and currently has a census of five residents in care. During today’s visit, LPA toured the facility and inspected the physical plant, including but not limited to testing all smoke detectors, testing hot water temperature in three of three resident bathrooms, and testing auditory devices on all exits. The hot water temperature measured between 107.9 and 120.3 degrees Fahrenheit and all smoke and carbon monoxide detectors were operational. The fire extinguisher is charged and was serviced this year The facility’s last fire drill was conducted on September 8, 2024. LPA inspected the facility food supply and observed the facility retained a minimum of two days perishable and seven days non-perishable food on hand. LPA observed medication storage and reviewed the centrally stored medications. Per review medications are being given as prescribed. The First Aid Kit has all of the required elements and includes a First Aid Manual book. LPA walked the exterior of the facility and observed a covered patio area and a self-latching gate. LPA reviewed three of three staff training and fingerprint records and a complete review of resident records. LPA interviewed alert residents regarding their quality of care and spoke to staff present regarding care provided. LPA confirmed that administrator has a current administrator certificate on the pending renewal list which will expire on August 12, 2026. (Continued on LIC 809-C) (Continued from LIC 809) Based on the observations made during today’s visit, the facility appears to be in compliance with Title 22 Division 6 of the California Code of Regulations, no deficiencies cited on this date.
