On June 17, 2026, Licensing Program Analyst (LPA) Brandon Lopez made an unannounced visit to the facility to conduct the required annual inspection. LPA was greeted and granted entry into the facility by staff after explaining the purpose for the visit. Administrator (AD) Anna Mallari was unable to assist with today's inspection, however, House Manager (HM) Adrian Ngo was present for today's visit. LPA observed that Anna Mallari has a valid Administrator certificate which expires on December 26, 2027. The facility is a Residential Care Facility for the Elderly (RCFE) licensed for six residents, of which five can be non-ambulatory and one can be bedridden in bedroom #5, and has a hospice waiver for two. The facility is a single story home which consist of four resident bedrooms, two of which are shared, two shared resident bathrooms, a living room, a dining room, a kitchen, and an attached two car garage. LPA, accompanied by the HM, conducted a tour of the interior portions of the facility. On today's visit, LPA observed six residents in care and two care giving staff present. LPA observed the See Something, Say Something poster (PUB 475) mounted on the wall by the entryway of the facility. LPA inspected the four resident bedrooms and observed them to be clear of any hazards. LPA observed resident bedrooms to have the required furnishings of a bed, a chair, a chest of drawers, and a lamp. LPA observed resident beds to have clean linens and blankets. LPA observed additional linens to be stored in a hallway cabinets. LPA inspected the two shared resident bathrooms and observed them to be clean. Resident bathrooms were equipped with grab bars and non-skid floor mats. Faucets and toilets were operational. Hot water temperature measured between 110.6 and 111.3 degrees Fahrenheit. LPA observed the facility has a two day perishable and a seven day non-perishable food supply in the kitchen. LPA observed kitchen knives and sharps to be stored in a locked kitchen cabinet. CONTINUED ON LIC809-C LPA observed a fire extinguishers to be mounted on the wall in the kitchen and it was observed to be charged and serviced as of August 11, 2025. LPA tested the wired smoke detectors and individual carbon monoxide detector which tested operational. LPA observed the facility conducted their most recent emergency disaster drill on May 16, 2026. LPA observed that there is a first aid kit stored in the kitchen which has all the required components. LPA observed the centrally stored medication to be kept in a locked cabinet located in the living room. LPA observed the facility has a three day emergency food and water supply to be stored in a hallway closet. LPA observed the door leading to the attached two car garage to be kept locked and inaccessible to residents in care. LPA observed the garage to be used for storage and laundry. LPA observed chemicals and toxins to be stored in a locked closet located in the garage. LPA, accompanied by the HM, conducted a tour of the exterior portions of the facility. LPA observed the exterior to be clear of any obstructions or hazards. LPA observed a shaded outdoor seating area with furniture for resident use. LPA observed the perimeter gate of the facility to be self-latching and can be opened in an evacuation. There are no bodies of water on the premises. LPA reviewed all six resident files. All the required documentation were present and current in the resident files reviewed. LPA reviewed the residents' medication and medication administration records. LPA reviewed the Personal and Incidental (P&I) expense monies for the six residents. LPA reviewed three staff files. All staff are background cleared and associated to the facility. Based on the observations made during today's visit, no deficiencies are being cited per Title 22 of the California Code of Regulations. Allegation investigated: Staff spoke to resident in an inappropriate manner. Staff did not provide resident's money when requested.

Licensing Program Analyst (LPA) Hanna Gough made an unannounced visit to the facility to conduct an investigation into the above mentioned complaint allegations. LPA was greeted and granted entry by staff. LPA met with Administrator (AD) Anna Mallari and discussed the purpose of the visit. The investigation into staff spoke to resident in an inappropriate manner and staff did not provide residents money when requested revealed the following: It was alleged that on January 7, 2026, staff spoke inappropriately to Resident #1 (R1) when they were requesting their money before getting on the bus and did not receive their requested amount of $20. LPA reviewed a physicians report for R1 dated March 10, 2025, stating that R1 is able to follow instructions and can communicate their needs. R1 is not able to manage their own cash resources. Continue on LIC9099C Unfounded LPA reviewed a cash resources log (LIC405) for R1 stating that they used $11.30 of their money on January 7, 2026. This transaction was signed by facility staff only. LPA reviewed an Individual Program Plan for R1 dated October 28, 2025, stating that facility staff will assist R1 with their money. LPA interviewed 4 of 4 staff and it was revealed that the incident reported never occurred at the facility. 4 of 4 staff informed LPA that when residents inquire about money, it is provided to them upon request. 4 of 4 staff denied speaking to residents in an inappropriate manner. LPA interviewed 6 of 6 residents in care and 5 of 6 residents denied the allegations. 1 of 6 residents was unable to confirm or deny the allegations. LPA reviewed 3 of 4 staff having updated training on topics such as behavioral issues, recognizing and reporting abuse and resident rights. 1 of 4 staff have an active Administrators certificate as well as updated resident rights training. Based on the evidence gathered, the Department finds that the allegation is unfounded. A finding that the complaint is unfounded means that the allegation is false, could not have happened, and/or is without a reasonable basis. Licensing Program Analyst (LPA) Hanna Gough arrived at the facility for the purpose of conducting a plan of correction visit to verify the corrected citation. The citation was issued during the required annual inspection that was conducted on May 23rd, 2025. LPA was greeted and granted entry by staff. LPA met with House Manager Paulo Faeldo and explained the purpose of the visit. LPA toured the facility and observed that the wall in the covered patio had been taken down. LPA observed building permits for the demolition of the non-permitted interior wall in the patio. Based on today’s observations the deficiency cited under 87305(a) of Title 22 Division 6 of the California Code of Regulations has been cleared. Licensing Program Analyst (LPA) Hanna Gough arrived at the facility to conduct the required annual inspection. LPA was greeted and granted entry by staff. LPA met with House Manager (HM) Paulo Faeldo and explained the purpose of the visit. The facility currently has five residents of which four are away at day program. The facility is a one story home with four client bedrooms, kitchen, living room, two bathrooms, activities room, backyard and an attached two car garage. LPA observed the activities room to have a wall down the middle of it. After file review LPA observed that the activities room on the facility sketch is just one room with no wall. LPA observed the kitchen to be free of debris and vermin. LPA observed the two day perishable and seven day non-perishable food supply on hand. LPA observed the sharps to be in a locked drawer by the kitchen sink making them inaccessible to residents in care. LPA observed all the appliances to be operational. LPA observed the toxins and chemicals to be stored in the garage in a locked closet, making them inaccessible to residents in care. LPA observed the centrally stored medication to be locked in a closet by the front door, making them inaccessible to residents in care. LPA observed the emergency food and water supply to be located in a closet in the hallway. LPA observed all resident bedrooms had the required components and furnishings. LPA observed the bathrooms to have toilet paper, paper towels, and non-slip flooring in the showers. LPA tested the water between 109.4-109.9 degrees Fahrenheit in both bathrooms. LPA observed the backyard to be free of obstructions. LPA observed a shaded seating area for resident use. LPA tested the fire and carbon monoxide detectors and they were found to be operational. LPA observed a fire extinguisher located in the kitchen to be charged and have a service date of May 15, 2025. Continue on LIC 809-C LPA reviewed resident files, no discrepancies were observed. LPA reviewed staff files and one of two staff does not have the required annual training. LPA reviewed P & I with HM and no discrepancies were observed. LPA observed that the last fire drill conducted was on January 12, 2025. LPA reviewed resident medication and no discrepancies were observed. Based on today’s inspection deficiencies are being cited per Title 22 Division 6 of the California Code of Regulations. Licensing Program Analyst (LPA) Dwayne Mason Jr. arrived at the facility unannounced for the purpose of conducting a required annual inspection. LPA was greeted and granted entry into facility by staff. LPA met with Direct Service Providers (DSPs) Paulo Faeldo, Celia Salalac and Adian Ngo. The facility is a one-story home with four resident bedrooms, two bathrooms, kitchen, dining room, living room, activity room, backyard and attached 2-car garage. Facility appears clean, safe and sanitary. LPA noted residents were away at day program. All resident rooms had required elements, including bed, chair, closet space and ample lighting. Facility has extra linens and hygiene supplies for clients in the hallway closet. Restrooms are stocked with soap and paper towels and have hand washing postings. Hot water measured between 105 and 120 degrees F in both bathrooms. LPA observed facility has emergency food and water supply. LPA observed the fire extinguisher was last serviced on May 14, 2024. LPA observed hazardous items such as knives, chemicals and cleaners to be locked up in the kitchen and the garage. Knives are locked up separate from toxic chemicals. Medication for each client is kept locked in a closet near the entryway. The backyard has a shaded sitting/lounging area. Exit gate is unlocked and self latching. LPA observed exit gate to be unobstructed. LPA reviewed three resident files and three staff files. LPA also reviewed medication and P&I for three clients. LPA interviewed one staff. Resident were away from the facility at the time of the inspection and unable to be interviewed. Facility has board games, exercise equipment, electronics, music and more for resident use. This unannounced inspection is being conducted by Licensing Program Analyst (LPA) Sean Haddad for the purpose of conducting a Required – 1 Year Inspection. LPA met with Staff #1 (S1) Shirgil Regio and discussed the purpose of the inspection. Administrator (AD) Anna Mallari was not present during the inspection. LPA reviewed Infection Control requirements. At about 1:15PM, LPA and S1 conducted a tour of the inside and outside of the facility, common areas, resident rooms, kitchen, and garage and observed the following: Structure: this is a one-story home. Facility is a 4-bedroom, 2-bathroom, one-story house with attached garage that is being used for storage. LPA observed 2 staff and 4 residents present at the facility. Resident Bedrooms: the 4 resident bedrooms are spacious and will easily accommodate the residents’ furnishings. Furniture for each resident bedroom inspected. Staff Bedrooms: there are no staff bedrooms. Bathrooms: the bathrooms were clean, faucets and toilets were operational. Water temperature: tested at 109.9 degrees F in the common client bathroom and 111.5 in the bedroom client bathroom. LPA inspected all rooms in the facility. Linens & Hygiene Supplies: new linens and fully stocked linen closets were observed. Emergency Phone Numbers, Exit Plan & Menu: reviewed. Food Service: LPA observed the facility has a 2-day supply of perishables and a 7-day supply of non-perishable food is available as required by regulations. Carbon Monoxide, Smoke Detectors, Fire Extinguisher: observed and tested, including the wired smoke detectors/carbon monoxide detectors. Appliances: stove burners, microwave, washer, and dryer inspected. Knives: observed locked in the kitchen. Toxins: observed locked in the garage. Medication cabinet: observed to be locked. First-Aid Kit and Activity Supplies: observed and available. LPA discussed licensing fees with S1. At about 2:15PM, LPA reviewed 6 resident files and 4 staff files, interviewed 4 residents and 2 staff, inspected medications for 6 residents, and inspected resident money and ledgers for 6 residents. There were no deficiencies observed in the areas inspected. Based on the observations made during today’s inspection, no deficiencies are being cited per Title 22 Division 6 of the California Code of Regulations.
