Licensing Program Analyst (LPA) Hanna Gough made an unannounced visit to the facility to conduct a case management visit. LPA was greeted and granted entry by staff. LPA met with Administrator (AD) Marife Gironella and discussed the purpose of the visit. LPA toured the facility and checked on Residents in care. LPA observed residents napping and watching television. LPA discussed the fire clearance that was submitted to the Regional Office in January of 2026 with AD that is pending. AD informed LPA that during their inspection they were informed of changes and facility updates that needed to be made in order to pass the fire inspection and be granted a new fire clearance. AD informed LPA that the changes have been made and that they are now working on the documents that need to be sent to the Regional Office. AD informed LPA that they will send all documents for a new fire clearance request along with an LIC200 to the Regional Office by close of business May 22, 2026. Licensing Program Analyst (LPA) Hanna Gough made an unannounced visit to the facility to conduct the required annual inspection. LPA was greeted and granted entry by staff. LPA met with Administrator (AD) Marife Gironella and discussed the purpose of the visit. The facility currently has six residents in care. The facility is a one story home with four resident bedrooms, living room, dining room, kitchen, laundry room, sun room and attached two car garage. The facility appears clean, safe and sanitary. LPA observed the facility requested a new fire clearance from the Department for the sun room on January 5, 2026 and is awaiting an appointment to be made for an inspection to get a new fire clearance. LPA observed the room not to be used for residents until it has been cleared. LPA observed the resident rooms to have the required components and furnishings. LPA observed a clean supply of linens in the hall cabinets by the resident bathroom. LPA observed the resident bathrooms to have toilet paper, paper towels and non slip mats in the shower. The water was tested between 112.4 and 112.6 degrees Fahrenheit. LPA observed the kitchen to be clean and free of vermin. LPA observed a two day perishable and seven day non perishable food supply on hand. The appliances were found to be operational. LPA observed the knives and sharps to be locked under the kitchen sink and made inaccessible. The centrally stored medications were observed to be in the locked closet located by the front door. LPA observed the emergency supplies to be in the closet by the front door. LPA observed toxins and chemicals to be stored in the locked laundry room closet. An outdoor shaded seating area was observed for resident use. No debris or obstructions were found. LPA observed the garage to be used as extra storage for the facility with a fridge for extra food. LPA observed a locked garage cabinet full of toxins and chemicals and made inaccessible to residents. AD tested the carbon monoxide and smoke detectors and they were found to be operational. Continue on LIC809C LPA reviewed staff files and 2 of 2 staff need updated 20 hours of annual training pertaining to the regulatory topics of dementia, postural supports, hospice and restricted health conditions. LPA reviewed resident files and no discrepancies were observed. LPA reviewed resident medications and no discrepancies were observed. P&I was reviewed with AD and no discrepancies were observed. The last fire drill was conducted on January 5, 2026. All staff present are background cleared and associated to the facility. Based on todays inspection a technical violation is being noted per Title 22 Division 6 of the California Code of Regulations. Allegation investigated: Staff forces residents to sleep early.

Licensing Program Analyst (LPA) Hanna Gough arrived at the facility to investigate the above mentioned complaint allegation. LPA was greeted and granted entry by staff. LPA met with Administrator (AD) Marife Gironella and discussed the purpose of the investigation. It is alleged that residents are put in bed for the night between the hours of 4:00PM and 4:30PM and that they are in bed until morning. The investigation into the facility allegation of staff forces residents to sleep early revealed the following: LPA reviewed progress notes for five of six residents in care that between October 1, 2025, and October 20, 2025, three of six residents when noted to be put in bed for the night was between the hours of 7:00PM and 8:30PM. One of six residents when noted to be put in bed for the night was consistently at 8:30PM and one of six residents when noted to be put in bed for the night was between 8:30PM and 10:00PM. LPA observed medication lists for six of six residents for the month of October. LPA observed four of six residents have 8:00PM medications and that they were all taken daily. Continue on LIC9099C Unfounded LPA conducted interviews with staff and residents. Three of six residents informed LPA that they go to bed when they want and that they are not forced to go to bed at any time of the day. One of six residents informed LPA that they like to take a nap in the afternoon, eat dinner and then go back to bed soon after dinner. Four of four staff informed LPA that residents are not forced to take naps in the afternoon and they are not forced to go to bed at a specific time. Four of four staff informed LPA that the majority of the residents are ready for bed around 8:00PM. Three of four staff informed LPA that if the resident does not want to go to bed or take a nap, they offer them to stay and continue doing what they are doing or offer another activity. Based on the evidence gathered, the Department finds that the allegation staff forces residents to sleep early is deemed UNFOUNDED. The allegation is false, could not have happened, and/or is without a reasonable basis. Therefore the department dismisses the allegation. On February 27th, 2025 Licensing Program Analyst (LPA) William Vanegas made an unannounced inspection for the purposes of an annual inspection. Upon arrival LPA Vanegas was greeted and granted entry by Care Giver (CG) Karen Santos and expalined the purpose of the visit. LPA Vanegas began a tour of the facility and observed the following. This is a one storied home with four bedrooms, and two bathrooms. There is also an attached two car garage. LPA Vanegas observed the kitchen area to be clean and free of any mildew or debris. LPA Vanegas observed there to be a two-day supply of perishable food and a seven day supply of non-perishable food. LPA Vanegas observed there to be a sufficient amount of emergency water. LPA Vanegas observed restrooms to be clean and free of mildew and debris. Water faucets and toilets tested to be operational. LPA Vanegas observed bathrooms to have all required furnishing such as grab bars, shower chair, and slip resistant floor matts. Water tested between 112.2 and 112.6 degrees. LPA Vanegas observed resident bedrooms to have all required furnishings such as a lamp, chest drawers, enough closet space, a bed, and clean linens in good repair; meaning no strains or tears. Rooms are big enough to lounge freely and not have any obstructions blocking entry ways. LPA Vanegas observed fire extinguishers to be up to date, and fully charged. LPA Vanegas observed all smoke and carbon monoxide detectors to be in good repair, and all tested operational. LPA Vanegas observed first aid kit to have all required equipment such as adhesive tape, scissors, tweezers, a thermometer, and bandages. CONTINUED ON LIC809C LPA Vanegas observed the outside of the facility to be clean and in good repair. LPA Vanegas observed backyard to be big enough to participate in outdoor activities upon resident request and an outdoor shaded sitting area. LPA Vanegas observed back yard exit routes to be free of any obstructions and side doors to be self latching and unlocked. LPA Vanegas reviewed emergency disaster plan and infection control plan to be up to date, and to be filled out correctly; containing all required components. LPA Vanegas reviewed six resident files, and five staff files. All files (Staff and Resident) contained all required documentation, and all staff training was up to date and documented correctly. LPA Vanegas reviewed P&I with Administrator (AD) Divina Alipio and all balances were accounted for and documented correctly. LPA Vanegas reviewed medications with AD, and all medications are being documented correctly. Per LPA Vanegas review medications are being administered per physicians orders. Based on observations made during today's visit no deficiencies will be cited per title 22 Chapter six of the California Code of Regulations.
