On 07/31/26, Licensing Program Analyst (LPA) Aziz Faizi made an unannounced visit to the facility to conduct a required annual inspection. LPA was greeted and granted entry by caregiver Ahley Moran who was informed of the purpose of the visit. The facility is a one-story home with five (5) bedrooms and three (3) bathrooms, including a garage. There are no pools or known firearms on the premises. LPA toured the facility's exterior and observed outdoor pathways were free of obstructions. Outdoor shaded seating area is available for the clients in care. The physical plant, including floors, windows, and doors, was clean and well maintained. Fixtures and furniture were in good repair. LPA toured the kitchen and observed the facility has a two-day supply of perishable foods and more than a seven-day supply of non-perishable foods, which are stored in a safe and healthy manner. LPA observed knives and sharp instruments secured in locked laundry cabinets. Laundry equipment was in good working condition. Both the smoke detector and carbon monoxide detector were operational, and the hot water temperature was 116.1°F. Fire extinguishers equipped throughout the building are maintained in accordance with the department's safety requirements expiring on 06/05/2027. Adequate staff were present to supervise clients during the visit. The administrator holds a current administrator’s certificate expiring on 01/26/2028. Continued with 809C LPA reviewed files for three (3) staff members, confirming criminal clearances, updated training, and CPR/First Aid certification. Three (3) resident files were reviewed and contained all required documentation. All resident medications were securely locked in cabinets. LPA observed medications were checked and listed ensuring accuracy and proper documentation by the facility and staff. LPA reviewed the facility’s emergency and disaster plan, including documentation of the last fire/earthquake drill conducted on 07/04/2026, which met department requirements. All facility exits were clear of obstructions. No deficiencies were cited during the visit. On 07/31/25, Licensing Program Analyst (LPA), Javina George made an unannounced visit to the facility to conduct the required annual inspection. LPA met with Administrator, Kevin Calhoun initially, who then left the facilty. LPA conducled the inspection with caregiver Clariza Bawiin. At the time there was At the time of the visit there were 3 staff and 5 residents present. The facility has an approved hospice waiver for 4. There are currently 3 residents receiving hospice services. There is one (1) resident receiving home health services. Below are the observations made during today's inspection. The facility was observed to be clean, clutter and odor free. The food supply was adequate as there was a 2 day supply of perishable and a 7 day supply of non perishable food items. The smoke and carbon monoxide detectors were tested and were found to be operable. The facility was observed to have the required postings. A copy of the facility's liability insurance was obtained. The hot water temperature was tested and 106.1. The facility is conducting emergency disaster drills on a quarterly basis with the last drill being conducted on 4/14/25, a drill will be conducted, by the end of day today. There are no guns, ammunition or bodies of water on the premises. The medications, and chemicals were observed to be locked and inaccessible to residents in care.A records review all (5) resident and (3) staff files. The files were found to have the required documentation (admissions agreement, medical assessment and resident appraisal). Staff were observed to have obtained criminal record clearance, and Cardio Pulmonary Resuscitation (CPR) First Aid training. The administrator Kevin Calhoun possesses a valid administrator certification that expires on 1/26/26. No citations were issued during today's inspection. On 07/03/24, Licensing Program Analyst (LPA), Javina George made an unannounced visit to the facility to conduct the required annual inspection. LPA met with Caregiver Clarizza Bawiin and Administrator, Kevin Calhoun via telephone as he was unable to come to the facility. LPA explained the purpose of the visit. At the time of the visit there were 2 staff and 5 residents present. The home is licensed to serve age range 60 and over, 6 non-ambulatory of which 6 may be bedridden. The facility has an approved hospice waiver for 4. There are currently 3 residents receiving hospice services. The facility was observed to be clean, clutter and odor free. Outdoor and indoor passageways are free of obstruction. The facility is a single story consisting of (5) bedrooms, and (3) bathrooms, kitchen, living room, backyard, and garage. The restrooms were equipped with grab bars and non skid mats. The facility food supply met the requirements as there is a 7 day supply of non perishables, and a 2 day supply of perishable food items. The fire extinguisher was observed to be fully charged, with tag in tact. Carbon monoxide and smoke detectors were tested and were observed to be operable. The facility was observed to have the required postings such as Emergency disaster plans, personal rights, and complaint procedures were posted in a prominent area. LPA conducted a records review of 5 resident and 4 staff files. The files were found to have the required documentation, staff were observed to have obtained criminal record clearance. Per the records review 2 of 2 staff did not possess a valid CPR certification. A citation will be issued in accordance with the California Code of Regulations (Title 22, Division 6, Chapter 8), on the attached 809D.
