On August 24, 2026, Licensing Program Analyst (LPA) Brandon Lopez made an unannounced visit to the facility to conduct the required annual inspection. LPA was greeted and granted entry into the facility by care giving staff after explaining the purpose for the visit. Administrator (AD) Mareties Pontoy was notified via telephone and later arrived to assist with the inspection. LPA observed that Mareties Pontoy has a valid Administrator certificate which expires on August 10, 2028. The facility is a Residential Care Facility for the Elderly (RCFE) licensed for six non-ambulatory residents and has an approved hospice waiver for six residents. The facility is a single story home with four resident bedrooms, two of which are shared, three bathrooms, one of which is for staff use only, a living room, a dining room, a kitchen, a laundry room, a storage room, and detached two car garage. LPA, accompanied by the AD, conducted a tour of the interior portions of the facility. On today's visit, LPA observed five resident in care and two care giving staff present. LPA observed the See Something, Say Something poster, (PUB 475) mounted on the wall in the living room. LPA inspected all five resident bedrooms and observed them to be free of hazards. LPA observed resident bedrooms to have the required furnishings of a bed, a chair, a chest of drawers, and a lamp. LPA observed resident beds to have clean linens and blankets. LPA inspected the two resident bathrooms and observed them to be clean. Bathrooms were equipped with grab bars and non-skid floor mats. Faucets and toilets were operational. Hot water temperature measured 118.7 and 119.8 degrees Fahrenheit. LPA observed the facility has a two day perishable and a seven day non-perishable food supply on hand. LPA observed kitchen appliances to be clean and operational. LPA observed kitchen knives and sharps to be stored in a locked kitchen cabinet. LPA observed fire extinguishers to be mounted on the wall in the living room and in the resident hallway. CONTINUED ON LIC809-C Fire extinguishers were observed to be charged and serviced on February 12, 2026. LPA observed the centrally stored medication to be kept in a locked cabinet located in the living room. LPA observed the facility has a first aid kit stored inside the same cabinet and it had all the required components. LPA observed chemicals and toxins to be stored in locked cabinet in the laundry room. LPA observed additional linens to be stored in the laundry room. LPA observed the storage room to be kept free of any hazards. LPA tested the wired smoke detectors and the individual carbon monoxide detectors which tested operational. LPA observed the facility conducted their most recent emergency disaster drill on July 14, 2026. LPA, accompanied by the AD, conducted a tour of the exterior portions of the facility. LPA observed the exterior to be free of any obstruction or hazards. LPA observed a shaded outdoor seating area with furniture for resident use. LPA inspected the detached two car garage and observed it to be kept locked and inaccessible to residents in care. LPA observed the garage to be used for storage. LPA observed additional chemicals and toxins to be stored in the garage. LPA also observed the facility has a three day emergency food and water supply stored in the garage. LPA observed the perimeter gate of the facility can be opened in an evacuation. There are no bodies of water on the premises. LPA reviewed all five resident files. All the required documentation were present and current in the resident file reviewed. LPA reviewed the residents' medication and medication records. LPA reviewed four staff files. All staff are background cleared and associated to the facility. Based on the observations made during today's visit, no deficiencies are being cited per Title 22 of the California Code of Regulations. Licensing Program Analyst (LPA) Samer Haddadin conducted an unannounced annual inspection at the facility. Upon arrival, LPA met with staff member Jessica Hernandez, explained the purpose of the visit, and conducted a tour of the interior and exterior areas of the facility. Licensee Mareties Pontoy arrived shortly thereafter.The facility is a one-story residence consisting of four resident bedrooms, three resident bathrooms, a kitchen and dining area, living room, backyard, laundry room, storage room, and a detached two-car garage. Resident bedrooms were observed to be furnished with beds, chairs, clean linens, and provided sufficient storage space. Rooms were free of obstructions and tripping hazards.All manual smoke detectors, carbon monoxide detectors, and exit alarms were tested and found to be operational. Bathrooms were in good condition and equipped with grab bars for resident safety. However, hot water temperatures measured between 130.9°F and 128.6°F, which exceeds the regulatory limit. A deficiency is being cited in relation to this observation.The facility maintained the required minimum of a two-day supply of perishable food and a seven-day supply of non-perishable food. Kitchen appliances were clean and in working condition. All chemicals, knives, and sharp objects were observed to be locked and inaccessible to residents in care.Fire extinguishers were fully charged with indicators in the green zone; however, the inspection tag showed an outdated service date of February 17, 2025. Medications were stored securely in a locked cart located in the staff room.LPA reviewed two resident files and corresponding medications, with no discrepancies noted. All staff and resident records contained the required documentation. The most recent quarterly fire drill was conducted on May 14, 2025, meeting regulatory expectations.The exterior of the facility was observed to be well maintained, with outdoor furniture in good repair and walkways free of tripping hazards.Based on the observations made during this inspection, deficiencies are being cited in accordance with Title 22, Division 6 of the California Code of Regulations. Licensing Program Analyst (LPA) Dwayne Mason Jr. arrived at the facility unannounced for the purpose of conducting a required annual inspection. LPA was greeted and granted entry into facility by Dante Pontoy, Caregiver. Administrator Mareties Pontoy and Licensee Theresa Bernabe joined the inspection approximately 15 minutes after LPA's arrival. The facility is a one-story home with four resident bedrooms, three resident bathrooms, kitchen/dining room, living room, backyard, laundry room, storage room and detached 2-car garage. All resident rooms had required elements, including bed, chair, closet space and ample lighting. Facility has extra linens and supplies for residents in the garage. Restrooms are stocked with soap and paper towels. LPA measured water in resident bathrooms to be between 105 and 120 degrees Fahrenheit. LPA noted Fire Extinguishers were last serviced on 02/20/2024 according to the service tag on them. LPA observed the smoke/carbon monoxide detectors to be operational. Facility keeps emergency water and emergency disaster supplies in the garage. LPA observed hazardous items such as knives, chemicals and cleaners to be locked up in cabinets. Knives are locked up separate from toxic chemicals. Medication for each resident is kept locked in a cabinet in the kitchen. The backyard has a shaded sitting/lounging area. Exit gate is unlocked. LPA observed exit gate to be unobstructed. LPA observed a sample menu and activity calendar in the facility. LPA reviewed three of the six resident files and three staff files. LPA also reviewed medication for three out of six residents. LPA interviewed two residents and one staff. Based on today's inspection, no deficiencies are being issued.
