Allegation investigated: Facility increaseed rent without proper notice

Licensing Program Analyst (LPA) Samer Haddadin conducted an unannounced visit to the facility to investigate the above-mentioned allegation. Upon arrival, LPA Haddadin was granted entry and met with Administrator (AD) Troy Byington, to whom the purpose of the visit was explained. During the course of the investigation, LPA Haddadin obtained and reviewed facility records, interviewed current residents, and interviewed staff. It was alleged that “Facility increased rent without proper notice” to Resident 1 (R1) and Resident 2 (R2). LPA Haddadin interviewed R1, who confirmed that the facility had provided a ninety-day notice of the rent increase. LPA Haddadin was unable to interview R2 due to a current medical condition that did not allow R2 to provide a reliable statement. In addition, LPA Haddadin interviewed three current residents, all of whom denied the allegation. Unfounded LPA Haddadin also interviewed five staff members; four denied the allegation, and one was unsure. During record review, LPA Haddadin confirmed that the residents interviewed were mailed a ninety-day notice of the rent increase on September 24, 2025, with an effective date of January 1, 2026. Copies of the notices were maintained in the facility’s records. Therefore, based on the preponderance of evidence gathered through interviews and record review, the allegation, “Facility increased rent without proper notice,” was found to be unfounded. This means the allegation was determined to be false, could not have happened, and/or is without a reasonable basis. No deficiencies were cited during today’s visit. Allegation investigated: Staff did not prevent resident from having bed bugs

Licensing Program Analyst (LPA) Samer Haddadin conducted an unannounced visit to the facility to investigate the above-mentioned allegation. Upon arrival, LPA Haddadin was granted entry and met with Administrator (AD) Troy Boyington, to whom the purpose of the visit was explained. During the course of the investigation, LPA Haddadin obtained and reviewed the facility’s pest control records and verified the facility utilizes a pest control company that provides services to the facility. The most recent service date on file was November 7, 2025. LPA Haddadin also conducted a physical inspection of six randomly selected resident rooms and did not observe any bedbugs or any evidence consistent with bedbug activity. Additionally, the facility’s maintenance records reflected ongoing daily, weekly, and monthly scheduled cleaning. LPA Haddadin conducted six interviews, consisting of three staff interviews and three resident interviews. All individuals interviewed denied the allegation and reported that resident rooms are cleaned according to an established schedule and upon a resident’s request. Based on record review, observations, and interviews, the preponderance of evidence standard has not been met; therefore, the allegation, “Staff did not prevent resident from having bed bugs,” is unsubstantiated. Licensing Program Analyst (LPA) Samer Haddadin conducted an announced pre-licensing inspection visit (CHOW): Change Of Ownership. Upon arrival, LPA Haddadin was greeted by Administrator Troy Byington, who granted entry. Together, they toured the interior and exterior areas of the three-story facility, which includes a delayed-egress memory care unit, commercial kitchen, large dining room, medication room, multiple common areas and storage rooms, and two courtyards—one central and one dedicated to memory care—both providing shaded seating and ample space for resident activities. The facility consists of 115 resident rooms, and the census at the time of inspection was 108 residents. Resident rooms observed were furnished with a bed, chair, and clean linens, provided adequate storage space, and were free of tripping hazards. Outdoor areas, including both courtyards, were well maintained with furnishings in good repair, clear pathways, and grounds that appeared safe and inviting. All safety equipment was up to date. The fire alarm system is maintained by Fire Safety Systems, with the most recent inspection completed on March 11, 2025. Fire extinguishers were serviced by Fire Master on June 2, 2025. Hot water temperature was measured at 107.9°F, meeting regulatory requirements. The facility maintains an adequate food supply, consisting of at least a two-day supply of perishables and a seven-day supply of non-perishables. The kitchen was observed to be clean, organized, and in good repair, with all major appliances functioning properly. Cleaning supplies were securely stored in the third-floor maintenance room, ensuring they remained inaccessible to residents. A review of resident files and medication logs revealed no discrepancies. Staff personnel files were complete and contained all required documentation. Records also confirmed that the facility conducted its annual emergency drill on June 18, 2025.No deficiencies were observed during this inspection.
