Licensing Program Analyst (LPA) Samer Haddadin conducted an unannounced visit to the facility for the purpose of completing the annual required inspection. LPA arrived at the facility, was greeted, and was granted entry by Administrator (AD) Victoria Paul. LPA toured the interior and exterior of the facility and observed the following: The facility is a one-story home consisting of five bedrooms and three bathrooms, with an attached garage currently being used for storage. At the time of the visit, the facility had four residents in care.LPA and AD tested the smoke detectors and carbon monoxide detectors in common areas and resident bedrooms; all were operational. Resident bedrooms contained the required furniture, bed linens, and adequate closet and drawer space to accommodate residents comfortably.LPA observed fire extinguishers mounted in the kitchen and garage areas. The last inspection date was September 18, 2025. Upon review of records, the emergency drill log indicated the most recent drill was conducted on October 13, 2025.LPA observed that sharps and knives were locked in the kitchen and inaccessible to residents in care. Restroom toilets and water faucets were operational. Grab bars were secure, and showers were observed to be free of mold and mildew. Water temperature measured between 109.1 degrees Fahrenheit and 109.6 degrees Fahrenheit.LPA and AD toured the backyard and observed a shaded seating area available for residents’ use. LPA confirmed emergency exits were free of obstacles. LPA reviewed two resident files and two staff files and observed they contained the required documentation.{***CONTINUE 809C***) Based on observations and record review conducted during today’s inspection, no deficiencies were cited pursuant to Title 22, Division 6, of the California Code of Regulations. Allegation investigated: Staff stole residents storage bin

On this date Licensing Program Analyst (LPA) Samer Haddadin conducted an unannounced 10-day complaint investigation at the facility. The purpose of the visit was to present findings regarding a reported allegation. Upon arrival, LPA Haddadin met with Administrator (AD) Victoria Paul, who provided full access to the facility. The investigation involved a tour of the premises, interviews with staff members, an interview with the responsible party for Resident 1 (R1), and a thorough review of all records pertaining to the alleged incident involving R1. Allegation: The investigation addressed the following allegation received by Community Care Licensing (CCL) on June 6, 2025: "Staff stole a resident's storage bin." Background: Resident 1 (R1) moved into the facility on May 6, 2025, with the assistance of their sibling. They brought personal belongings, including one black and yellow plastic storage bin. Investigation and Findings: A review of R1's signed inventory intake sheet confirmed that all listed items are present and accounted for. {****CONTINUE ON 9099C***} Unfounded According to the Administrator and four staff members who were present during the move-in process, the item in question was a black plastic storage bin, approximately 60 inches long and 20 inches deep. They stated that this bin contained items that were not included on the facility's official inventory sheet. The LPA conducted interviews with four staff members; all four denied the allegation. Furthermore, the LPA reviewed security camera footage which showed a care staff member wheeling the storage bin out of the facility and placing it back into the sibling’s vehicle on the day of move-in. Conclusion: Based on the preponderance of evidence gathered through staff interviews, a review of facility records, and security footage, the allegation that "Staff stole a resident's storage bin" is Unfounded. An "Unfounded" determination means the allegation was determined to be false, could not have happened, or is without a reasonable basis. Closing: No deficiencies were cited during this visit. Upon review of record, emergency drill log indicated that the last drill was conducted on October 23rd, 2023. LPAs and AD toured the backyard of the facility and observed a shaded seating area for residents’ enjoyment. LPA checked emergency exits to be free of obstacles. LPAs reviewed two residents’ and two staff files and observed they all had the required documentation. Based on the observations made during today’s inspection, deficiencies are being cited per Title 22 Division 6 of the California Code of Regulations. Type of visit: Prelicensing

Licensing Program Analyst (LPA) Sean Haddad conducted this announced inspection for the purpose of conducting a pre-licensing inspection. LPA met with Applicant (AP) Victoria Paul, discussed the purpose of the inspection, and toured the facility. Facility is to operate a Residential Care Facility for the Elderly. Application was submitted to Community Care Licensing on 09/14/2023. This is an initial application with no residents in care. During the inspection, LPA and AP observed the following: Structure: this is a one-story home. Facility is a 5-bedroom, 3-bathroom, 1 story house with an attached garage that is being used for storage. There is a back yard with a patio cover for the residents. Facility telephone number is (714) 833-5851. Resident Bedrooms: the 3 resident bedrooms are spacious and will easily accommodate the residents’ furnishings. Lamps, chairs, linens, and storage for each resident bedroom inspected. Staff Bedrooms: LPA inspected the 2 staff bedrooms. Bathrooms were clean, faucets and toilets were operational. Water temperature: tested between 110 and 117.5 F degrees. Linens & Hygiene Supplies: new linens and fully stocked linen closets were observed. Emergency Phone Numbers, Exit Plan, and Menu: reviewed. Food Service: 7 days nonperishable food supply reviewed and AP stated they will obtain 2 days perishable food supply prior to accepting residents. Carbon Monoxide, Smoke Detectors, Fire Extinguisher: inspected. Appliances: stove burners, microwave, washer, and dryer inspected. Knives: observed locked in the kitchen. Toxins: observed locked in the garage. Medication cabinet is locked. First-Aid Kit and Activity Supplies: observed and available. Resident & Staff Files: this is an initial inspection and LPA observed storage space for resident and staff files. Fire clearance was approved by Anaheim Fire Department on 09/26/23. Backyard: backyard exit gate is operational and unlocked. Backyard has shaded area for outdoor activities and sufficient seating for residents. Component III was completed with AP during today’s inspection. AP will obtain liability insurance once the application is approved. During the inspection, LPA explained the process of this application and about the post licensing inspection once the facility is licensed. AP was informed today that the facility is ready for licensure and final approval will be processed by the CAB supervisor in Sacramento.
