Licensing Program Analyst LPA Samer Haddadin conducted an unannounced visit for the purpose of completing annual inspection. LPA was greeted and granted entry by staff member, Rani Abbas. LPA and staff member began the tour and observed the following: The facility is a one-story resident with a detached two-door garage. The facility has a total of four bedrooms, three full bathrooms with no clients in care at the time of the visit. NO residents were observed in the facility. LPA checked all four bedrooms and observed all to have the required furnishing: clean linens, chair night stand room for storage and enough lightning. Hard wired smoke detectors, carbon monoxide and audible exit alarms were tested to be operational. Fire extinguisher was checked and observed it to be full charge with last inspection date of Dec.1st,2025. Bathrooms were observed to be in good repair and provided with grab bars and hot water was measured between 115.6-116.6 degrees Fahrenheit. Facility did meet the minimum two-day supply of perishable and seven-day supply of non-perishable food stock requirements. Emergency Food and water was also available and stored in the garage For the exterior portion, facility had a shaded area with char for residents ; grounds were free of tripping hazards and ample space for activities. Facility has a 2-car garage and is kept locked and used for storage; the garage also had an operational washer and dryer. kitchen was in good repair with cleaning supplies and sharp items locked in cabinet. LPA observed a locked cabinet in hallway which will be used for meds. An informal office meeting was held on this day. Present during the meeting was Licensing Program Analyst (LPA) Samer Haddadin, Licensing Program Manager (LPM) Alisa Ortiz, Licensees Abdirahman Qumane and Rani Abbas. The purpose of today's meeting was to discuss LPA Haddadin's attempted annual visit on March 03, 2026 with Licensees. At the time of the attempted visit LPA Haddadin became aware that property was being occupied by an unidentified individual. At the time of inspection, Licensee reported there were no residents in care and LPA was unable to obtain access to the property. During today's visit, Licensee informed the department that perspective caregivers occupied the property during LPA's visit with the agreement of them being fingerprinted, however, that did not occur. Licensee also stated that he had attempted to contact LPA Haddadin, however, did not leave a voicemail. Furthermore, the individuals were removed from the property. During today's visit LPM discussed adherence to criminal background requirements prior to presence in facility; reporting requirements; expectation of maintaining control of property and LPA inspection authority; and Responsibility of Licensee. Licensee agreed to adhere to the above items and will notify the department once first resident has been admitted. LPA and staff member began the tour and observed the following: The facility is a one-story resident with a detached two-door garage. LPA checked bedroom one and observed two separate beds; both beds had linens which; LPA checked the mattresses and found light brown stains on both. Room one had enough lighting and closet for storage. LPA did not observe any chairs. Bedroom two had to beds, light stand and closet for storage. LPA did not observe any chairs. LPA Checked bedroom three and observed that it had two beds; LPA observed both mattresses on the bed had saints; Room three had enough lighting and space for storage. Bedroom three did not have a chair. Bedroom four had two beds which also had two stained mattresses; it had enough lighting and storage and no chair. Hard wired smoke detectors, carbon monoxide and audible exit alarms were tested to be operational. Fire extinguisher was observed with a tag without inspection date. Bathrooms were observed to be in good repair and provided with grab bars and hot water was measured at 115.6 degrees Fahrenheit. Facility did not meet the minimum two-day supply of perishable and seven-day supply of non-perishable food stock requirements. For the exterior portion, facility had outside a shaded area and no chairs; grounds were free of tripping hazards and ample space for activities. Facility has a 2-car garage and is kept locked and used for storage; the garage also had an operational washer and dryer. kitchen was in good repair with cleaning supplies and sharp items locked on cabinet. LPA observed a locked cabinet in hallway which will be used for meds. Based on today’s visit and observation made at this facility, LPA will schedule a pre-licensing.
