Allegation investigated: Staff are physically, psychologically and mentally abusing resident in care.

At 11:30am, Licensing Program Analyst (LPA), Angela Panushkina conducted an unannounced visit in response to the above-mentioned allegation. LPA met with the Administrator and explained the reason for the visit. At 11:35am, LPA requested resident and staff roster. At 11:40am, LPA requested copies of pertinent information which include, but not limited to Admission Agreement, Physician's Report, Appraisal Needs and Services Plan, Staff Training, Abuse Policy, relevant to the investigation. At approximately 11:45am, LPA conducted a physical plant tour, to ensure health and safety of the residents are protected. Between 11:50am - 1:00pm, LPA conducted an interview with the Administrator, two (2) staff and one (1) resident. Continue on LIC9099-C Unsubstantiated Allegation: Staff are physically, psychologically and mentally abusing resident in care. It was alleged that the facility staff pulled R1’s hair and said, “you deserve it.” To investigate this allegation LPA conducted an interview with the Administrator, who denied any knowledge of abuse and informed LPA that they have not witnessed or heard any reports of any staff abusing residents. During the physical plant tour, LPA did not observe any signs of abuse, and R1 was clean and well taken care of. Two (2) staff interviewed informed LPA they have never physically or verbally abused R1 or witnessed other staff abusing R1. During the interview with R1, R1 expressed no concern regarding this allegation. R1 also verified that staff have never physically, psychologically, mentally or verbally harmed them and that all staff are very kind, gentle and treat them with dignity and respect. Based on interviews, record review and LPA's observation there is not enough information to verify the allegation. Therefore, the allegation is deemed Unsubstantiated at this time. No deficiency issued during today's visit. At 9:30am, Licensing Program Analyst (LPA), Angela Panushkina conducted an announced Pre-Licensing visit to the above facility and met with the Administrator, Khachatur Vahramyan, and explained the reason for the visit. An application was submitted to Community Care Licensing Division (CCLD) for a Residential Care Facility for the Elderly (RCFE) to serve age range of 60 and over. Fire Clearance approved on 07/31/2025 for five (5) Non-ambulatory and one (1) Bedridden resident in bedrooms #3 or #4. Dementia and Bedridden Plans are submitted. Emergency numbers are posted and readily available for review. The facility is a single-story home consisting of four (4) bedrooms, designated for residents’ use only, and two (2) bathrooms. The facility also has a staff room located by the kitchen. LPA observed that all bedrooms are furnished with beds, dressers and required bedding and linen. The bedrooms have sufficient closet space and have sufficient lighting. Facility maintains a temperature of 74°F At 9:40am LPA observed all bathrooms are clean and in good repair. All bathrooms have a working toilet, wash basin and a shower. Properly supplied with toilet paper, soap and paper towels. At 9:45am, the hot water temperature measured at 118.7°F. All trash cans in bathrooms had fitted lids to protect them from cross contamination. At 9:50am, LPA observed a Fire Extinguisher which was last purchased on 07/29/2025. Dishes, cups, and flatware are stored in the kitchen cupboards, inspected and in good condition. Knives were observed locked in the kitchen drawer. Adequate supplies of two (2) days perishable and seven (7) days nonperishable foods to accommodate a maximum capacity of six (6) residents were available. Continue on LIC809-C Refrigerator, oven, microwave, dishwasher and washer/dryer were observed to be in good/working condition. The first-aid kit has been inspected which has at least the following: tweezers, scissors, antiseptic, bandages, gauze, thermometer; including a current First Aid manual. Resident and staff files will be kept in a locked, metal cabinet located by the kitchen. At 10:10am, LPA toured the outside area of the facility. LPA observed appropriate outdoor furniture, with a covered shaded area for residents. The outdoor area was free of visible immediate hazards. There is a swimming pool that is fenced to keep residents out of the swimming pool area. The fence is approximately 5 feet high throughout the parameters. You will need a key to unlock the padlock to gain entry to the swimming pool as it is kept locked at all times. LPA discussed the importance of maintaining care and supervision to meet the needs of residents. The residence is equipped with central heating and air conditioning. Cleaning supplies and toxins are locked under the kitchen sink and only accessible to staff. Smoke detectors and carbon monoxide were tested at 10:00am and observed to be functional. Component III was conducted at 10:20am. Pursuant to Title 22, Division 6 Chapter 2, the facility's physical environment appears to be compliant and ready for licensure. Identification of the applicant and administrator was verified through interview questions based on photo ID and other identifying personal information. During COMP II, applicant and administrator confirmed that they have read and understand community care facility licensing laws included in the Health and Safety Codes and the California Code of Regulations Title 22.
