On 11/25/2025, at 10:30 AM, Licensing Program Analyst (LPA) James Sampair arrived unannounced to conduct a Plan of Correction (POC) visit. Upon arrival, the LPA stated the purpose of the visit to Caregiver Jenny Lagana and spoke with Licensee Tina Camaclang by phone about the purpose of the visit. On 9/26/2025, LPAs Sampair and Christy conducted a Required 1-Year inspection of the facility during which 1 deficiency was cited for not sending in an Unusual Incident Report (LIC 624) for the incident on 09/17/2025 concerning R1. The Licensee attested to clearing the deficiency over the phone on or before the 10/6/2025 due date, so the LPA cleared the deficiency. No citation was issued during this visit. On 11/25/2025, at 10:30 AM, Licensing Program Analyst (LPA) James Sampair arrived unannounced for this Case Management - Deficiencies visit. Upon entry, the LPA explained the purpose of the visit to Caregiver Jenny Lagana and spoke with Licensee Tina Camaclang by phone about the purpose of the visit. The purpose of the visit concerned the partial obstruction of the front gate that is access into the facility. The Licensee had moved the car that was partially obstructing the gate on 9/26/2025 when the LPA requested that it be moved. No citation was issued during this visit. Type of visit: Required - 1 Year

On 09/26/2025 at 8:30AM, Licensing Program Analysts (LPAs) James Sampair and Andrew Christy arrived unannounced to conduct the Required Annual Inspection. Upon entry, LPAs met with Administrator Tina Camaclang and explained the purpose of the visit. The facility currently houses five (5) residents. The LPAs inspected the interior and exterior of the facility. The inspection of the physical plant included the kitchen, dining area, restrooms, community living spaces, resident rooms, storage areas, and the grounds of the facility. More than the required minimum of 7 days of nonperishable and 2 days of perishable foods were appropriately stored. The hot water temperature in the bathroom was measured to 105.1 degrees Fahrenheit. The room temperature of the living room was 71.2 degrees Fahrenheit. Fire Extinguisher was last maintained 06/05/2025. The carbon monoxide and smoke detectors were fully operational. The LPAs observed required postings in the facility, including the Residential Care Facility for the Elderly Complaint Poster, Ombudsman and Personal Rights posters, and the Theft and Loss Policy. Five (5) resident files and five (5) files were reviewed. The Emergency Disaster Plan was last updated 09/06/2025. The Quarterly Emergency Drill was last conducted 07/06/2025. Type of visit: Case Management - Health Checks

On 9/19/2025, at 12:15 PM, Licensing Program Analyst (LPA) James Sampair arrived unannounced for this Case Management - Health Checks visit. Upon entry into the facility, the LPA stated the purpose of the visit to Licensee / Administrator (ADM) Albertina Camaclang. LPA and ADM checked all of the perishable and nonperishable foods in the facility. One of the perishable and 55 of the nonperishable foods were expired and thrown out. A Type-A citation was issued for this deficiency (refer to LIC 809-D). After throwing out the expired foods, the facility still had more than the required minimum of 2 days of perishable and 7 days of nonperishable food for the residents. Deficiencies are cited from Title 22 California Code of Regulations (refer to LIC 809-D). Failure to submit proof of correction may result in additional civil penalty. Upon entry in the facility, the LPA stated the purpose of the visit to Licensee Tina Camaclang. The complaint alleges that staff did not check on resident according to care plan. On 11/06/2024 at 9:00 AM, Licensing Program Analyst (LPA) James Sampair conducted an unannounced visit to deliver the finding on the allegation above. Upon arriving, the LPA explained the purpose of the visit to Supervisor Gina Longoria. The complaint alleges that a staff member pinched residents in care. The LPA conducted interviews of the Licensee, 1 resident, and 3 staff members. No data collected from those interviews indicated that a staff member from this facility has pinched any residents in this facility. Although the allegation may have happened, or is valid, there is not a preponderance of evidence to prove it; therefore, the allegation is UNSUBSTANTIATED. On 10/15/2024 at 9:00 AM, Licensing Program Analysts (LPAs) James Sampair and David Doidge arrived unannounced to conduct a Case Management - Annual Continuation inspection to complete the Required Annual Inspection that was partially completed on 9/30/2024. Upon entry, LPAs stated the purpose of the visit to Caregiver Gina Longoria. Licensee Tina Camaclang arrived at approximately 10:00 AM. The LPAs completed a review of the facility records. The LPAs reviewed the records of 5 residents and 5 staff members. The LPAs interviewed 4 staff members. Administrator will provide LPA copies of the following documents by 10/22/2024: 1. Upon entry, LPAs stated the purpose of the visit to Caregiver Maribel Zumel. Administrator Tina Camaclang arrived at 1:05 PM. The LPAs inspected the interior and exterior of the facility. The inspection of the physical plant included the kitchen, dining area, restrooms, community living spaces, resident rooms, storage areas, and the grounds of the facility. More than the required minimum of 7 days of nonperishable and 2 days of perishable foods were appropriately stored. The hot water temperature in the bathroom was 109 degrees at 12:25 PM. The room temperature of the living room was 72 degrees at 12:30 PM. Fire Extinguisher date was 09/11/2023. The carbon monoxide and smoke detectors were fully operational. The LPAs observed required postings in the facility, including the Residential Care Facility for the Elderly Complaint Poster, Ombudsman and Personal Rights posters, and the Theft and Loss Policy. An administrator is on site more than the minimum of 20 hours a week to oversee the proper business operations. One type A and one type B citations were issued during the inspection.
