Type of visit: Required - 1 Year
On 06/09/2026, at 09:45 AM, Licensing Program Analyst (LPA) James Sampair arrived unannounced to conduct this Required Annual inspection. Upon entry, the LPA stated the purpose of the visit to Maintenance Director (MD) Jason Fern. The Executive Director (ED) Kelli Greene arrived at approximately 09:15 AM The LPA toured the exterior of the facility by himself, and the interior with the MD and the ED. The tour of the facility included, but was not limited to, two (2) residents’ rooms, bathrooms, the central kitchen, and common areas. The LPA observed adequate lighting for the comfort and safety of residents throughout the community and in individual's rooms. The walkways in both the interior and exterior of the facility are free from obstructions. There are no bodies of water. The temperature of the refrigerators was below 40 degrees Fahrenheit and the freezers below 0 degrees Fahrenheit. At 12:10 PM, the front hallway temperature was measured at 72.1 degrees Fahrenheit. At 10:10 AM, the maximum hot water temperature was measured in the Bistro at 112.6 degrees Fahrenheit. The residents’ bathrooms are equipped with grab bars and slip-resistant mats. There is more than a 7-day supply minimum of nonperishable foods and a 2-day supply of perishable foods. The LPA observed the required postings in the facility. The smoke and carbon monoxide detectors were last tested on 01/08/2026 and found to be in full operating condition. The fire extinguishers were last serviced on 01/08/2026. The Emergency Disaster Plan was reviewed by the ED on 02/09/2026. Emergency and disaster drills are conducted monthly; the most recent was conducted on 05/27/2026. The first aid kit was observed to be complete and staff dispense medications directly from the original container to residents. The liability insurance expires on 06/01/2027. Type of visit: Required - 1 Year
On 5/22/2025, at 1:00 PM, Licensing Program Analyst (LPA) James Sampair arrived unannounced to conduct this Required 1 Year inspection. The LPA informed Executive Director (ED) Kelli Greene of the purpose of this visit. The LPA and ED inspected the inside and outside of the facility, which included the kitchen, dining room, common areas, bedrooms, and outside common areas. An adequate amount of food supplies were observed, more than the required minimum of 2 days of perishable and 7 days of non-perishable food. The central storage for medications was locked. The cleaning supplies and dangerous objects were not accessible to residents. The Facility has working smoke and carbon monoxide detectors. The staff of the facility conduct disaster / emergency and fire drills on a monthly basis. The fire extinguishers were serviced 12/26/2024. The indoor temperature was 73 degrees Fahrenheit and the maximum hot water temperature was 114 degrees Fahrenheit, both within the acceptable range. The LPA reviewed facility records. No citation was issued during this inspection. Required 1 Year inspection incomplete. LPA will return unannounced at a later date to complete inspection. Allegation investigated: Staff does not ensure residents' rooms are clean.
On March 26, 2025, at 1:00 PM, Licensing Program Analyst (LPA) James Sampair arrived unannounced to investigate the allegation above. Upon entry into the facility, the LPA identified himself and stated the purpose of the visit to Executive Director (ED) Monique Bindra. The complaint alleges that staff does not ensure residents' rooms are clean. The LPA interviewed the ED. She stated that they had an unexpected staff shortage. Staff were moved around and staff from other facilities were brought in to ensure rooms were deep cleaned at least once a week. The data collected does not confirm the allegation. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove it; therefore, the allegation is UNSUBSTANTIATED. Allegation investigated: Staff did not prevent a resident from sustaining a fracture while in care. Facility is not adequately staffed to meet the needs of the residents in care. Staff are not serving nutritious meals to residents.
On 8/6/2024 at 2:55 PM, Licensing Program Analyst (LPA) J. Sampair arrived unannounced to complete the investigation of the allegations above. The LPA informed Executive Director (ED) Monique Bindra of the reason for the visit. The Department's investigation included, but was not limited to, interviews with staff, residents, and the Reporting Party (RP). The Department obtained and reviewed records pertaining to Resident R1, which included facility records, medical records, Emergency Medical Services (EMS) report, and 911 audio call recording. R1 was admitted to the facility on 3/31/2021. Continued on LIC 9099-C Unsubstantiated Continued from LIC 9099 The complaint alleges that staff did not prevent Resident R1 from sustaining a fracture while in care. Review of the records and interviews of the ED and staff revealed that R1 was independent and only needed assistance using the restroom at night. R1 was not known to wander during the day or at night. When R1 fell outside on the patio and sustained a right leg fracture on 12/23/2023, it was the first time R1 tried to leave the facility at night. The complaint alleges that Facility is not adequately staffed to meet the needs of the residents in care. Review of the records and interviews of the ED and staff revealed that R1 was a fall risk and was placed on two-hour checks to make sure that she was in her room and safe. Interviews with overnight staff S1, S2, and S3 revealed that they responded to the door alarm in R1’s room within 5 minutes, in accordance with the facility’s policy. The complaint alleges that staff are not serving nutritious meals to residents. Review of the restaurant menu and interviews of 2 residents, R4 and R5, revealed that the facility staff are serving nutritious meals to the residents. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED. Allegation investigated: Staff are not providing residents adequate food service
On 7/12/2024 at 4:30 PM, Licensing Program Analyst (LPA) A Gomez arrived unannounced to conduct an initial 10-day complaint investigation and deliver findings in regard to the allegation above. LPA met with Maintenance Director, Kaila Homolka (MD) and explained the purpose of the visit. Executive Director was unavailable and approved MD to sign. During the visit LPA obtained copies of restaurant menu, bistro menu, and weekly specials menu. LPA toured kitchen and observed food of good quality and variety. LPA also interviewed 2 residents who all stated that they are satisfied with the food service. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED. Type of visit: Required - 1 Year
On 5/1/2024 at 12:30 PM, Licensing Program Analyst (LPA) J. Sampair arrived unannounced to conduct the Required Annual Inspection of the facility. Upon arrival, LPA stated the purpose of the visit to Executive Director (ED) Monique Bindra. The LPA inspected the physical plant with the ED which included the kitchen, dining room, restrooms, community living spaces, resident rooms, storage, and the exterior of the facility. In the kitchen, more than the required minimum of 7 days of nonperishable and 2 days of perishable foods were appropriately stored. examined the emergency food and water supplies. Fire extinguishers were last serviced on 1/3/2024 and the most recent inspection of the fire suppression systems were completed on 1/3/2024 and 1/23/2024 and the fire alarm on 2/22/2024. The LPA verified the completion of required staff training and the completion of quarterly emergency/disaster training for staff during every shift. The LPA observed postings in the facility that included a complaint poster, Ombudsman and Personal Rights posters, Theft and Loss Policy, Rights to Resident Council, and Rights to Family Council. An administrator is on site more than the minimum of 20 hours a week to oversee the proper business operations. The LPA reviewed 5 resident and 5 staff files and interviewed 5 staff and 5 residents. Type of visit: Case Management - Health Checks
On 1/8/2024 at 11:00 AM, Licensing Program Analyst (LPA) J. Sampair arrived unannounced to conduct a health and safety check of the facility. Upon arrival, LPA stated the purpose of the visit to Executive Director (ED) Monique Bindra. LPA toured facility including but not limited to the common areas, kitchen, trash disposal, parking, and outdoor areas. Facility temperature was maintained at 73.7 degrees F in the Bistro. Hot water temperature was measured at 112.8 degrees F in bathroom sink. 7-days of non-perishable and 2-days of perishable food supplies were sufficient. Resident medications were kept locked in the Medication Office. First-aid kit was complete. Fire extinguisher was observed to be full and last serviced on 1/3/2024. Allegation investigated: The facility license is not posted for public viewing
On 12/13/2023 at 03:50 PM, Licensing Program Analyst (LPA) J. Sampair arrived unannounced to conduct the initial 10-day complaint inspection of the facility pertaining to the allegation above. Upon arrival, LPA stated the purpose of the visit to Administrator Monique Bindra. The complainant alleged that the facility license is not posted for public viewing. Shortly after entering the facility, the LPA confirmed that the license was still posted in the location where it had been when he last visited the facility on 07/28/2023. The allegation is false, could not have happened, and/or is without a reasonable basis, therefore, the allegation is UNFOUNDED.
