On 10/07/2025 at 09:30 AM, Licensing Program Analyst (LPA) David Doidge unannounced to conduct a 1-Year Annual Required Inspection. LPA met with Caregiver Javier Pimentel Guerrero explained the purpose of the visit. Administrator Timea Yost was called by phone and arrived 9:50 AM. The LPA toured the facility inside and out with Administrator Timea Yost. LPA inspected the kitchen, dining area, living room, bedrooms, bathrooms, front, side and back yards. LPA observed lighting in all rooms are adequate for the comfort and safety of the residents. Residents’ bathrooms are equipped with grab bars and non-skid mats. Hallway temperature was maintained at 70 degrees Fahrenheit. The hot water temperature was measured in a shared bathroom at 109.2 degrees Fahrenheit. There is a minimum of one week supply of nonperishable and 2-day of perishable foods. Centrally stored medications, sharps and toxic are locked and inaccessible to residents in care. Smoke detectors and carbon monoxide detectors were in operating condition. Fire extinguisher was last serviced on 06/02/2025. Emergency Disaster Plan was last posted on 08/30/2025. Emergency disaster and fire drills are conducted quarterly; last conducted on 08/01/2025. First aid kit was observed to be complete. LPA reviewed four (4) residents records and five (5) staff records; all were complete. At 10:20 Am, LPA found medication in an unlocked kitchen drawer. The deficiency was observed (see LIC 809D) and cited from the California Code of Regulations, Title 22. Failure to correct the deficiencies may result in civil penalties. On 08/15/24 at 01:20 pm Licensing Program Analysts (LPA) J. Clancy-Czuleger arrived unannounced to do an annual inspection. LPA meet with House Manager Telesha Clarke and explained the purpose of the visit. LPA inspected the facility inside out. There is no body of water. Physical plant is consistent with the facility sketch received by Central Application Bureau (CAB) and approved by the fire department. LPA inspected the living room, dining area, kitchen, bedrooms, hallways, bathrooms, side and backyards. Bedrooms were observed appropriately furnished with adequate lighting and drawers. Facility has sufficient towels, extra bed sheets and comforters. Equipment and supplies for residents' personal hygiene are available and on site. Dinner and silver wares were observed sufficient for residents' use. Food supplies checked and observed good for seven days of non-perishables. Facility was observed equipped with refrigerator, microwave, dishwasher, washer and dryer. Cabinet for knives, cleaning supplies, and central storage for medications were observed with locks. Activity supplies were available. Outdoor activity space was observed furnished with tables, chairs and shade. The facility has a mitigation plan. Fire extinguishers were observed fully charge and tags showed serviced 03/21/2024. At 1:32 pm LPA reviewed 4 residents records. At 1:45 pm, LPA reviewed 3 staff records and 5 of 5 were fingerprint cleared and associated to the facility. No deficiencies cited during visit. Allegation investigated: Resident sustained unstageable pressure injuries due to staff neglect

On 9/28/2023 at 2:15 PM, Licensing Program Analysts (LPAs) J. Clancy-Czuleger arrived unannounced to conduct a complaint visit. LPA explained the purpose of the visit with Administrator Telesha Clarke. During the complaint visit LPA obtained information, reviewed records, collected documents, and interviewed staff. It was alleged a resident sustained an unstageable pressure injury while in care. During an interview with S1 they stated that they did have a resident who had a pressure injury during that time frame. This resident was on hospice when the ulcer developed and was being addressed in the hospice care plan that the facility was following. Although the allegations may have happened or are valid, there are not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are UNSUBSTANTIATED.
