On 12/15/2025 Licensing Program Analyst (LPA) B. Miranda arrived to the facility unannounced to conduct an annual inspection. LPA met and spoke with House Lead Felicha Sanchez. Licensee Paulette Holland was contacted and arrived shortly after. Upon arrival LPA observed residents eating breakfast in the dining area at the kitchen table. Facility is licensed for 6 residents and have a current census of 5. The facility has 3 bedrooms and 3 bathrooms. LPA observed the facility to be clean and odor free. LPA tested the water temperature in the kitchen which read at 114.2. LPA observed a fire extinguisher in the kitchen which was last serviced 12/6/2024 and has current charge. Carbon monoxide detectors and smoke detectors were tested and are in working order. LPA reviewed disaster plan which is completed but has not been marked as recently being reviewed or updated. LPA did not observe an infection control plan at the facility and Licensee Paulette Holland confirmed there was not currently an infection plan at the facility. LPA reviewed all resident files. R1 is missing a care plan for their oxygen and there is no verification of staff training, residents do not have a physician's report on file within the past 12 months. LPA reviewed a sample of employee files, S1's file did not have current training in file. LPA observed a TV, multiple walkers, and wheelchairs in the outside patio area. LPA observed scissors and screw drivers in unlocked drawers in the kitchen. Due to time constraints LPA did not cite and will return to cite at a later date. Type of visit: Case Management - Incident

On April 24, 2025, Licensing Program Analyst (LPA) Rachel Bruce conducted an unannounced case management visit at the facility. LPA met with House Manager and also conducted a phone conference with Administrator Paulette Holland to discuss the purpose of the visit. LPA explained there were two recent incident reports that were lacking a bit of information see explanation below: 1- Resident 1 - date of occurrence 4/6/2025. R1 was found in bathroom after a fall. Staff helped her up did a quick exam to check for injuries only noticed a small amount of redness on elbow. R1 was offered an ice pack and there was no need for further care. LPA explained that the IR should have included information as to why R1 fell and what will happen as a result . For instance was this the first time, is she a fall risk, was the family notified, will we closely monitor. Although the staff did the right thing, the report needed further description. 2. Resident 2- date of occurrence 4/5/2025- R2 was found in bed shaking with his eyes rolled back. Staff called house manager and 911. R2 was taken to Clovis community Hospital. The report was lacking information regarding what was the diagnosis, any change in care plan, any follow up needed. Administrator and house manager understood the reports could have included more information and will ensure that going forward there are more complete. LPA requested to have staff continue to send reports to the incident fax line and to also send them directly to LPA for review through email. Type of visit: Required - 1 Year

On October 11, 2024, Licensing Program Analyst (LPA) R. Bruce arrived unannounced to conduct a Required 1 year Annual Inspection. LPA introduced self, stated the purpose of the visit and requested to meet with Administrator. Paulette Holland was available, left for a portion of the visit and returned at conclusion of inspection. Visitor log-in/temperature check, masks, and disinfection station were observed upon entry. Facility has one entrance/exit point. Facility appeared clean and obstruction of fire clearance issues. LPA observed the Fire extinguisher located in the kitchen with a service date February, 2023. All bathrooms observed with trash cans that have lids, and securely fastened grab bars. Showers have non-skid mats. All resident’s rooms toured and observed to be adequately furnished and lit. LPA observed 3 shared resident’s bedrooms. The exterior tour was conducted. Side gate was self-closing and self-latching. There is extra equipment, paint and misc. items that are stored in a shed which is locked. Staff records were reviewed and all required documentation was present. Three out of six resident's records were reviewed and found to be in compliance. LPA checked residents’ locked medications. Food supply was checked and appeared to be an adequate supply. Water temperatures in kitchen and bathrooms tested and within range per regulations. LPA is requesting the following documents be submitted to the Fresno CCL office by November 1, 2024. Current copy of Administrator Certificate, Designation of Facility Responsibility (LIC308), Administrator Organization (LIC 309), Emergency and Disaster Plan, Personnel Report (LIC500), Register of Facility Clients/Residents for LIC9020. An exit interview was conducted with Licensee.
