Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Sarah Hurt conducted an unannounced visit today for the facility’s annual inspection. LPA met with Licensee, Paulette Holland, Continual Administrator's Certification expires 07/14/2027. There are currently 0 residents who reside at this home. LPA inspected the interior and the exterior of the facility including the common living spaces, resident bedrooms and bathrooms, activity rooms, medication storage, kitchen, garage and outdoor areas. Bedrooms were clean and in good repair. There is a locked storage for medications. Fire extinguisher is within the safety regulation period. First Aid kit is on site and complete. Toxins and cleaning supplies are locked and inaccessible. There is not any residents currently living at this facility. The licensee will contact Licensing when the first resident moves into the facility. A post licensing inspection will be completed once first resident is admitted to the facility. There were no deficiencies observed or cited during today's inspection per California Code of Regulations, Title 22. LPA requested the following documents: LIC 500 Personnel Report, LIC 308 Designation of Administrative Responsibility, LIC 610-E the Emergency Disaster Plan and copy of current Administrator’s Certificate to update the facility file. Feb 11, 2025Facility evaluation reportReport on file
Type of visit: Required - 1 Year
On 02/11/2025, Licensing Program Analyst (LPA) R Bruce arrived to conduct an annual inspection. LPA introduced self, stated the purpose of the visit, and met with Licensee Paulette Holland. There are currently no clients residing at the facility. A pre licensing inspection was completed on 2/28/24. No issues noted at this time. A post licensing inspection will be conducted when residents are placed. At this visit, LPA conducted a tour inside and outside of facility. Facility observed to be clean, odor free and at a comfortable temperature. Common areas were furnished well with adequate seating and lighting available. Resident rooms appeared clean and had required furnishings. Resident bathrooms were properly equipped with securely fastened grab bars in toilet and tub/shower areas, non-skid mats were observed. Kitchen toured, appeared clean. Exterior tour conducted, all exits open and free of obstructions. Fire extinguisher serviced in December 2024. Smoke detectors and carbon monoxide detectors observed operational during today’s inspection. All cleaning supplies will be locked as well as medication when residents are present. There are no staff or client files to review. Upon acquiring a resident, CCL will conduct post licensing inspection. Identification of the applicant and administrator was verified through interview questions based on photo ID and other identifying personal information. During COMP II, applicant and administrator confirmed the understanding of the California Code Title 22 Regulations. Signed LIC 809 with copy of photo ID have been obtained.
