Type of visit: Prelicensing
Licensing Program Analyst (LPA) Katie Brown arrived at the facility to conduct a follow up Pre-Licensing Inspection. LPA met with Licensee/Administrator (AD) Bhinder Singh (Certificate #7010851740 Exp. 10/23/26). This is a Change of Ownership Pre-Licensing - Census is 6 residents. This visit was conducted today to confirm the completion of the outstanding maintenance & repair needed after the initial inspection 7/10/26. LPA observed the walls, doors, baseboards requested areas have been painted and repaired as needed. The Comp III was completed 7/10/26. This applicant has met all pre-licensing requirements. LPA will submit documentation to CAB in Sacramento for final review prior to license being issued. Jul 10, 2026Facility evaluation reportReport on file
Type of visit: Prelicensing
Licensing Program Analyst (LPA) Katie Brown arrived at the facility to conduct a Pre-Licensing Inspection. LPA met with Licensee/Administrator (AD) Bhinder Singh (Certificate #7010851740 Exp. 10/23/26). This is a Change of Ownership Pre-Licensing - Census is 6 residents. LPA began the tour by entering through the front door of the 5-bedroom, 2-bathroom home with 1 identified as a staff room. This facility has fire clearance for 5 non-ambulatory and 1 bedridden residents. Required postings were observed hung in the entry way. Common areas throughout the home were well lit with functioning ceiling fans with clear walkways and flooring intact. LPA observed supply of cleaning, disinfecting and paper products. LPA observed resident rooms to be furnished as required with available extra supply of linens and towels. The bathrooms are clean and found to be in good repair with faucets delivering hot water at 110 degrees F. Smoke and Carbon Monoxide detectors were tested and found to be working order. Cleaning supplies, chemicals, sharps and knives are stored and locked as required. The Fire Extinguishers were serviced by Valley Fire on 2/6/26. The kitchen was observed to have a complete supply of dishes, plates, utensils, and cooking items. Appliances were found to be in working order. LPA observed the required food supply, including separate emergency food, water and supplies. Resident medications are stored in a locking hall closet. The First aid kits contained the required items. Doors and passageways are unobstructed throughout the home. There is a fire door that is required to be closed at all times per fire clearance. See page 2 Page 2 Outside of the facility was toured. The self-releasing gate was found to be working properly. Walkways in front and backyards are clear. There is a small shed which was observed to store furniture and household item. The facility land line phone number is (559) 554-3094. Emergency Disaster and Infection Control Plans were reviewed as well as Personnel and Resident Files. LPA conducted the COMP III during this visit with AD. An exit interview was conducted, and a copy of this report was provided to AD. The following requirement needs to be met prior to licensure: 1.Maintenance and repair is needed on walls, doors and baseboards in the back hallway, bedroom doorways and bathroom wall. AD will contact LPA Brown when the above has been resolved to schedule a subsequent visit.
