On 11/05/2025 at approximately 12:30PM, Licensing Program Analyst (LPA) Ali Deniz arrived unannounced to conduct 1-Year Required visit of this licensed Residential Care Facility for The Elderly (RCFE). LPA was greeted by Staff, Lourdes Valdez. Administrator, Marivie Fabie arrived shortly later at approximately at 12:40PM. The facility currently approved for capacity of 6 residents and has an approved fire clearance for 6 non-ambulatory residents. Facility has an approved hospice waiver for 2 individuals. Upon arrival, LPA was informed that there were 4 residents in care and 2 staff members on-site. Facility is a 1 story building with 6 Resident bedrooms, 2 bathrooms, and common spaces. At approximately 1PM, LPA reviewed the Facility's Staff Roster and found that all staff on-site were background cleared and associated to the facility per regulation. At approximately 01:35PM, LPA and Licensee toured the building and grounds which was found to be clean and in good repair. LPA observed all walkways and exits to be unobstructed. All notices that are required to be posted have been posted and are in a highly visible area. LPA observed activity supplies for resident use. The amount of fresh and nonperishable foods is within regulation. Facility kitchen, refrigerators and freezers were clean, and food was stored properly. However, the LPA advised the administrator to include either an expiration date or the date the food was packed when individually packaging food items. Staff were in the process of cleaning up kitchen at the time of this inspection. Toxins are stored in the garage. Water temperature measured 110.6 degrees F and 112.4 degrees F which is within regulation between 105- and 120-degrees F at faucets accessible to residents. Continued on LIC809-C... Continued from LIC809... Fire Extinguishers found to be last charged on 09/02/2025 at the time of visit. Carbon Monoxide and smoke detectors were present and in order. There was enough lighting in all common areas, resident rooms, and hallways. Medication is centrally stored and secure/locked in the kitchen cabinet. At approximately 2:35PM, LPA reviewed 2 resident records and found 2 of 2 residents have current care plans, signed admission agreements, and physician's report on file. Medication records are thorough and contained physician's orders for each resident. At approximately 2:50PM, LPA reviewed 2 staff records. 2 of 2 records did contain documentation of completed training records as required. Evidence of current first aid and CPR training were current. LPA was presented with proof of current CPR & 1st Aid certification. Administrator Certificate is for Marivie Fabie #7006408740 expires 11/29/2026. LPA reviewed the facility emergency disaster plan. Facility has supplies enough to operate for more than 72 hours in an emergency. Facility conducted and documented a disaster drill on 10/20/2025. Licensing Program Analyst (LPA) Canela arrived unannounced, to conduct an Annual Required 1 YR inspection and was greeted by care staff, Lourdes Valdez, Administrator, Marivie Fabie was called and arrived a few minutes later. LPA toured the facility and made the following observations: the facility was a comfortable temperature. Extra hygiene products and linens were available and required bath mats and grab bars were observed. Water temperature in residents' bathrooms measured at 113 degrees F which is within regulation. Cleaning products and other toxins are locked and located under the sink in kitchen. Resident rooms were furnished per regulation. Facility has at least two days of perishable and one week of non-perishable foods. Medications were centrally stored and locked. Fire extinguishers were last inspected 10/23/24. Smoke detectors located throughout the facility and carbon monoxide detector were tested and functional. Last Disaster Drill conducted on 9/28/24. Fire Dept. conducted annual inspection on 11/5/2024. Exit doors have auditory alert system that were functional at time of visit. Required postings were observed. LPA initiated file review of 3 Residents and 2 Staff records. Type of visit: Case Management - Incident

Licensing Program Analyst (LPA) Araceli Canela arrived announced to conduct a Case Management Inspection and to go over a recent SOC341 that was received regarding resident R1, who is still living in the home. LPA met with care staff, Melody Andaluz and toured the home. Administrator, Marivie Fabie arrived a few minutes later. LPA Canela and Administrator discussed R1's current situation. LPA provided the following information: Health & Safety 1569.655 : Increase in fee rates for elderly residents; 60 days’ written notice stating amount of and reasons for increase; application of section. LPA also went over eviction procedures and requirements, Regulation 87224 Eviction Procedures. LPA consulted regarding the side yard gate being able to self close and latch. Check all exit sliding doors that they open easily. LPA requested facility to submit a current LIC999 facility sketch (floor plan/yard) to identify all rooms use. LPA/Administrator toured the facility and made the following observations: the facility was a comfortable temperature and free from obstructions. Extra hygiene products and linens were available and required bath mats and grab bars were observed. Water temperature in residents' bathrooms measured 118 degrees F which is within regulation. Cleaning products and other toxins are locked and located under the sink in kitchen. Resident rooms were furnished per regulation. Facility has at least two days of perishable and one week of non-perishable foods. Medications were centrally stored and locked. Fire extinguishers were last inspected 7/22/23. Smoke detectors located throughout the facility and carbon monoxide detector were tested and functional. Last Disaster Drill conducted on 9/1/2023. Fire Dept. conducted annual inspection on 10/16/2023. Exit doors have auditory alert system that were functional at time of visit. Required postings were observed. LPA initiated file review of 5 Residents and 2 Staff records. Administrator Certificate for Marivie Fabie, 6021180740, expires on 11/29/2024. Staff have required First Aid and CPR certificates.
