At approximately 9:00AM, Licensing Program Analyst (LPA) Chris Arnhold arrived at this facility unannounced to conduct a Post Licensing inspection. LPA met with Administrator Renato Jamat Jr. and explained the purpose of the visit. Administrator certificate is current. Facility has a Hospice waiver for 2 residents. At approximately 9:30AM, LPA toured the facility to ensure the health and safety of residents in care. The facility was observed to be at a comfortable temperature. Areas toured include but are not limited to resident rooms, common areas, bathrooms, kitchen, storage areas and back yard. In the areas toured no immediate health, safety, or personal rights violations were observed. Fire extinguishers were fully charged. Smoke detectors are all operational. Carbon Monoxide Detector was present. The common areas, bathrooms and kitchen were clean and in good repair. All bedrooms had required furniture, bedding, and lighting. Food appears to be stored and prepared properly. Refrigerators and freezers were maintained at the proper temperature. Facility has required supply of perishable and non-perishable food. Emergency water was present to ensure facility can be self-sufficient for 72 hours. Emergency lighting devices were present. First aid kit was present. No pools/bodies of water are on the premises. Facility has been conducting Emergency drills every 3 months. At approximately 9:45AM, LPA reviewed 5 resident files. 2 of 5 records did not have signed admission agreements and did not contain completed appraisals. Admission agreements were signed during visit. Technical violation issued. Documentation of a physician visit was present. LPA discussed with Administrator that an After Visit Summary regarding an annual wellness visit could be used in place of the LIC602A. Medication records were organized and contained orders for each medication. Medications were secured and not accessible. Continued on LIC809-C… At approximately 10:30AM, LPA reviewed 8 staff files. Staff files reviewed did not contain evidence of completed initial or annual training. First Aid/CPR certification was current. All employees requiring background checks are cleared. Updated copies of the following documents were requested for facility file and are to be submitted to CCL within 30 days of this visit: LIC500- Personnel Report Deficiencies are cited from the California Code of Regulations (CCRs), and/or the Health and Safety Code. Failure to correct the cited deficiency(ies), on or before the Plan of Correction (POC) due date, may result in a civil penalty assessment. 02/11/2026, Licensing Program Analyst (LPA) Loera arrived to conduct a Pre-Licensing inspection and was greeted by Applicant, Izah Singerman. This is a change of ownership (CHOW) with four residents in care. This CHOW is for Applicant Caringhands Carehome, LLC. Facility received an approved fire clearance dated 11/14/2025 that allows for a total capacity of five non ambulatory and one bedridden. LPA conducted a tour and inspection of the indoor and outdoor portions of the facility. Facility was found to be clean and comfortable temperature with bedroom doors free from obstruction. LPA observed fire extinguishers to be last charged February 2026. Toxins and sharps are kept out of resident access and were found to be secured. Additional supply of hygiene products available to residents. Water temperature in three out of three faucets accessible by residents were found to be within regulation between 105 & 120 degrees F. Bathrooms had appropriate grab bars and slip resistant mats. Auditory devices on all exits were found to be working. Medications are centrally stored and secured in locked cabinet located in the utility room. Facility has all appropriate public documentation and personal rights information posted in common spaces within the facility. The facility has live-in staff. Facility has a storage shed in the backyard that is locked and inaccessible to residents. Facility's carbon monoxide/smoke detectors located throughout the facility are hardwired and were tested and operational. Component III was reviewed with Applicant. No Deficiencies given during visit. Pre-Licensing completed. License will be granted upon completion of a final review and approval from the Licensing Program Manager. LPA will submit Pre-Licensing Application Report to the Application Unit Analyst in Sacramento. Application Unit Analyst will notify Applicant of Status. Exit interview conducted.
