Type of visit: Required - 1 Year

On 11/17/25, Licensing Program Analyst (LPA) Felisa Shirley conducted an unannounced annual required visit. LPA met with the Administrator, Leia Joaquin and explained the purpose of today’s visit. The purpose of today’s visit was to conduct the required annual inspection, using the new Care Tool. This facility is licensed to serve 6 adults ages 60 and above, of which 1 maybe bedridden, 2 maybe non-ambulatory, and 2 maybe on hospice. The census is currently 5. Americare Assisted Living is a one-story structure located in a residential neighborhood. The facility consists of (7) bedrooms (2) bathrooms, living room, kitchen, dining area, shaded patio, and an attached garage which housed the washer and dryer. Facility maintains all required posting throughout the facility. LPA conducted a records review of (5) resident records, and (4) staff records. All client & staff records were complete. LPA Shirley and Leia walked through throughout the facility and all common areas such as the living room, dining room, and outdoor decks were all observed. Medications were centrally stored and properly locked, first aid kit was checked and fully stocked. Bathrooms were found to be within Title 22 regulations and were clean and operational. The water temperature was adjusted to meet temperature requirements. LPA observed that there is ample seating and space for all clients. All walkways were clean, and clear of obstructions and hazards. All areas have ample lighting. Smoke/ Carbon Monoxide detectors were operable. The last fire/emergency drill was conducted on 10/4/25. There are (3) charged fire extinguishers last serviced 3/25. Exterior areas are clean and clear of obstructions and hazards and there are no bodies of water present. No deficiencies were cited during this inspection visit. Type of visit: Required - 1 Year

On 11/8/2024, the Department of Social Services (DSS) – Community Care Licensing Division (CCLD) staff conducted an unannounced Required – 1 Year Inspection to the above-named facility and met with Leia Joaquin Licensee/Administrator. The purpose of the visit was explained to the Licensee/Administrator. This facility is licensed to serve 6 adults ages 60 and above, of which 1 maybe bedridden, 2 maybe non-ambulatory, and 2 maybe on hospice. A total of 6 residents are currently residing in this facility. The Annual Licensing Fees are current. The facility is a one-story house located in a residential street. The home consists of 6 resident bedrooms, 1 staff bedroom, 2 full bathrooms, 1 kitchen, 1 living room area, 1 dining room area with an office area, 1 attached garage with a laundry area, and 1 backyard patio area with shaded seating. Outside grounds were toured and no bodies of water were observed. The patio furniture is under a shaded area and accessible to residents. Walkways around the home were clear of hazards. There are no security bars or weapons on the premises. The kitchen area has supplies of nonperishable foods for a minimum of one week and fresh perishable foods for a minimum of two days. Knives and toxins were kept in locked storage cabinet. Medications were safe, locked, and inaccessible. All medications observed were labeled and maintained in compliance with label instructions and State and Federal law. Documents are posted as mandated. Last fire drill was conducted on 11/02/2024. Smoke and carbon monoxide detectors were in compliance and operational. There is a fire extinguisher in the kitchen area, and it was last serviced on 3/26/2024. There is a landline telephone and a video-conferencing device in the office area. 6 out of 6 resident’s bedrooms were checked. Mattresses were in good condition, adequate lighting, plenty of dresser and closet space observed. Walls and floors were clean and in good condition. Comforters, bed linen, bath towels and mattress protectors were adequately stocked. Bathroom toilets and water faucets worked properly, grab bars were secure, and a non-skid mat was in place. Adequate lighting and toiletries accessible to residents Hot water temperature was adjusted to meet temperature requirements. This facility provides residents with hygiene products such as feminine napkins, nonmedicated soap, toilet paper, toothbrush, toothpaste, and comb. 5 staff records were reviewed, 5 out of 5 staff records had required documentation. 5 resident records were reviewed and, 5 out of 5 resident records had required documentation. A technical violation is being recorded under CCR 87303(a) for not being able to walk through the garage due to too many things on the floor. The Licensee has agreed to organize their garage by 11/26/2024. Residents do not go inside the garage. No deficiencies are being cited based on LPA observation and record review in accordance with the California Code of Regulations, Title 22. Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Felisa Shirley conducted an unannounced Annual required and infection control visit to the above facility. LPA was met by Leia Joaquin, Administrator and the purpose of today’s visit was explained. There are currently (5) residents in placement. The facility is a single-story structure located in a residential neighborhood. It consists (7) bedrooms, (2) full bathrooms, ramp along side north side and back of facility, shaded back yard, and a front yard with an attached garage that houses the washer and dryer. LPA Shirley and Leia walked through the kitchen and all appliances were in good working order. Knives were locked and stored under the sink in the kitchen and inaccessible to residents. LPA observed a 3-day supply of perishable and a 7-day supply of nonperishable foods. The water temperature measured 118.9 degrees Fahrenheit. All bathrooms were checked, sufficient liquid soap and paper towels were observed. Toilets and water faucets worked properly. The walk-in shower was free of mildew and mold. LPA Shirley and Leia walked through all common areas. In the living room, kitchen, dining room there is ample seating and space for all residents. All rooms and walkways were clean, and clear of obstructions and hazards. All areas have ample lighting. All rooms, hallway, and living room have working smoke detectors. There is a charged fire extinguisher in the kitchen and hallway. LPA Shirley and Leia toured the entire facility inside and out. Documents are posted as mandated. Bedrooms 1-5 are occupied by residents and contain the mandated furniture. Bedroom 7 is a staff bedroom. The (2) bathrooms have grab bars and are clean and operational. First aid kit is fully stocked with manual. No firearms are stored at facility and no bodies of water present. Medications are stored, locked and inaccessible to residents. (1) Resident file along with medications are current. (1) Staff file is current. The facility is in good repair. PPE's will last for 30 days plus. According to the California Code of Regulations (Title 22, Division 6, Chapter 8), LPA did not observe any deficiencies, therefore no citations were issued at this time.
