On 3/28/2026, Licensing Program Analyst (LPA) M. Medina conducted an Annual/Required visit. LPA contacted Licensee/Administrator, Sheila Dillard who arrived to conduct facility inspection with LPA Facility was licensed in March 2023 and has had no residents in care. Facility tour conducted, facility observed to be fully furnished with required furnishings to receive residents. All common areas are furnished with adequate seating to accommodate residents. Facility has all utilities available. Resident bathrooms toured and observed fixtures to be operational, shower has grab bars, non-skid and shower chair available. Water temperature measured at 105 degrees F. Kitchen toured, LPA observed sharps to be locked and secured in hallway closet. First aid kit present with all required items. Smoke detectors and carbon monoxide observed to be operational during today's inspection. Fire extinguisher present with a purchase date of 10/30/2025. Outside of facility toured. Pool is surrounded by gate that is locked, secured, and inaccessible to residents. All exits open free of obstruction, no hazards observed. No deficiencies cited during today's visit. Allegation investigated: Staff are locking the exterior gate to the facility restricting access in case of an emergency

Licensing Program Analyst (LPA) Daiquiri Boyd conducted the complaint investigation visit to the facility. During the course of this complaint investigation LPA interviewed Licensee and obtained and/or reviewed facility records. It was found that there were no residents residing in the facility and it is currently unoccupied, Licesee stated tha lock was placed for security of the premises. It was determined based on observation and records review that the above allegation is SUBSTANTIATED. Based on LPAs observations and interviews which were conducted and record review(s), the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. 87203 - Fire Safety - All facilities shall be maintained in conformity with the regulations adopted by the State Fire Marshal for the protection of life and property against fire and panic. This requirement was not met as evidenced by the facility had a padlock on the front gate of the facility, which is not permitted by their approved Fire Clearance; which poses an immediate risk to the health, safety, or personal rights risk to the residents in care.the state’s words, verbatim · CDSS document, Feb 13, 2026

Licensing Program Analyst (LPA) Rachel Bruce arrived unannounced to conduct the Annual Inspection. LPA met with and explained the purpose of the visit with Licensee Sheila Dillard. There are no residents living in the facility at this time. During this visit, LPA toured the facility inside & out. Resident bedrooms are found to be in good repair, contained required furnishings and lighting. The resident bathrooms were clean and in good repair with faucets delivering hot water at 110 degrees. Kitchen faucet temperature measured at 117. LPA observed required hygiene items and grab bars. Towels, extra bedding, and linens were stored and available for use. The kitchen was clean, with necessary items and appliances. LPA observed required food supply and paper product storage. The pool in backyard is currently empty with appropriate fencing in place. Knives/sharps, cleaning/disinfecting supplies and chemicals are locked and stored separate from food. Medications will be centrally stored in a locked closet. First aid kits contained required items. There are visitation areas available inside and out. Doors and passageways are unobstructed throughout the facility. LPA observed a self-releasing gate and windows with screens in good repair. Smoke and Carbon Monoxide detectors were tested during the visit. The Fire extinguishers were purchased 10/15/2024. Emergency Disaster Plan and Infection Control Plans are in place and appropriate.
