Type of visit: Required - 1 Year

On 12/29/2025, Licensing Program Analyst (LPA) M. Medina conducted an Annual/Required visit. LPA contacted Licensee/Administrator, Sheila Dillard who arrived to conduct facility inspection with LPA Facility was licensed in January 2024 and has had no residents in care. Facility tour conducted, facility observed to be fully furnished with required furnishings to receive residents. All common areas are furnished with adequate seating to accommodate residents. Facility has all utilities available. Resident bathrooms toured and observed fixtures to be operational, shower has grab bars, non-skid and shower chair available. Water temperature measured at 106 degrees F. Kitchen toured, LPA observed a 7-day supply of non-perishable food. Medications and sharps will be locked and secured in hallway closet. First aid kit present with all required items. Smoke detectors and carbon monoxide observed to be operational during today's inspection. Fire extinguisher present with a purchase date of 10/25/2025. Outside of facility toured. Pool is surrounded by gate that is locked, secured, and inaccessible to residents. All exits open free of obstruction, no hazards observed. On 1/23/2025, Licensing Program Analyst (LPA) M. Medina conducted an Annual/Required visit. LPA met with Licensee/Administrator, Sheila Dillard to conduct facility inspection. Facility was licensed in January 2024 and has had no residents in care. Facility tour conducted, facility observed to be clean, odor free and a comfortable temperature. Residents bedrooms are fully furnished with required accommodations. Adequate seating and lighting observed in both the living room, and dining room. Client bathrooms toured, fixtures observed operational, shower has grab bars, non-skid and shower chair available. Water temperature measured at 115 degrees F. Kitchen toured, LPA observed a 7-day supply of non-perishable food. Medications and sharps will be locked and secured in hallway closet. First aid kit present with all required items. Smoke detectors and carbon monoxide observed to be operational during today's inspection. Fire extinguisher present with a purchase date of 10/15/2024. Outside of facility toured. Pool is surrounded by gate that is locked, secured, and inaccessible to residents. All exits open free of obstruction, no hazards observed. No deficiencies cited during today's visit. Type of visit: Prelicensing

On 1/16/23, Licensing Program Analyst (LPA) M. Medina conducted an announced Pre-licensing inspection. LPA introduced self, stated the purpose of the visit, and was granted entry into the facility. LPA met with Licensees, Sheila Dillard. LPA toured the facility with Licensee. The facility is 4 bedrooms and 3 bathrooms home. Fire clearance was granted for 6 non-ambulatory residents. Common areas were furnished and had adequate seating and lighting available. All bedrooms were observed to have required furnishings. Kitchen was toured and observed to have dishes, plate, and utensils. Facility has 7-day supply of non-perishable food available as of date of inspection. Resident bathrooms have grab bars in the shower and toilet areas, and a non-skid mat available. Knives and medication will be locked and secured in hall closet. Cleaning supplies and chemicals will be locked and secured in laundry room. Fire extinguisher present with a purchase date of 9/01/23 . Smoke detectors and carbon monoxide detectors were observed to be operational during this inspection. Water temperature measured at 110 degrees F. Facility has a working telephone of (661) 213-3926. Outside of facility toured. Exits open free of obstruction. Pool is surrounded by a locked and secured gate and inaccessible to residents. Component III conducted during pre-licensing inspection. LPA found that applicant has met all pre-licensing requirements. LPA will submit documentation to CAB in Sacramento for final review prior to license being issued. Exit interview conducted.
