On 08/24/2026, Licensing Program Analysts K.Kaur arrived at the facility unannounced to conduct annual inspection. LPA was greeted by staff and allowed entry into the facility. Administrator Evangeline Schissler was contacted and arrived a short while later. LPA toured the facility inside and out with staff. Entryway observed with required postings. LPA observed the residence to be a single story, 4 bedroom/ 2 bathroom home. There is a dining room and living room observed to have adequate furnishings and lighting.Fire extinguisher did not have a receipt or service date tag. Smoke alarms and Carbon Monoxide detectors were tested and observed to be operational. Night lights in the hallways by the bathrooms were observed. Linens and towels are kept in the hallway closet. Hallway closet observed with unlocked chemicals and medications. Hot water temperature in hallway bathroom measured at 103.5 degrees F and Master Bathroom water measured at 103.1. Grab bars were observed in the bathrooms next to the toilets and in showers. Non-slip mats observed. A locked cabinet in the kitchen is designated to store medications. Sharps/knives were observed in a locked drawer in the kitchen. First aid kit is located in the locked medication cabinet in the kitchen. Washer and Dryer observed in the laundry room with additional storage space available in upper cabinets. LPA observed cleaning supplies and chemicals unlocked in the garage. LPA toured the outside of the residence and observed an outdoor Umbrella cover for seating area. Adequate outdoor seating for activities. A self-latching gate found to be working properly. LPA observed unlocked chemicals in unlocked shed in the backyard. Due to time constraints, and documents being unavailable, LPA will return at a later date to continue annual inspection and issue citations. Exit interview conducted. Allegation investigated: Facility staff will not allow Ombudsman into the facility

On 02/12/2026, Licensing Program Analyst (LPA) V Gorban arrived unannounced to commence complaint investigation. LPA introduced self, stated the purpose of the visit to staff Rodello and was granted entry. Administrator was notified of Licensing visit and as able to arrive shortly. During the course of the investigation, LPA conducted a facility tour and conducted records review and interviews. This agency has investigated the complaint allegation: Facility staff will not allow Ombudsman into the facility. Based on records reviews and interviews facility had no personnel and residents on site during ombudsman visits. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegations are unsubstantiated. On 05/23/25, Licensing Program Analysts L. Salazar and M. Garza arrived at the facility unannounced to conduct the required annual inspection. LPAs were greeted by Administrator, stated the purpose of the visit and were allowed entry into the facility. Administrator on record is Evangeline Schissler, #6023393740 Expiration 01/13/25. Administrator stated they have submitted the renewal prior to the expiration date, however, LPA did not observe the name on the active or pending certification list online. LPA contacted the Administrator certification bureau at the time of visit. There are no residents in care at the time of visit. Facility was licensed one year ago but has been unable to secure staffing. LPAs toured the facility inside and out with Administrator. LPAs entered through the front door observed the required postings in the entry. LPA observed the residence to be a single story, 4 bedroom/ 2 bathroom home. There is a dining room and living room observed to have adequate furnishings and lighting. LPA observed fire extinguisher to be charged with a service date of 07/02/2024. Smoke detectors were tested and observed to be operational. Carbon Monoxide detectors were observed. Night lights in the hallways by the bathrooms were observed. LPA observed a adequate supply of towels. A supply of paper towels were observed in the bathrooms. Hot water temperature in bathroom measured at 114 degrees F. Grab bars were observed in the bathrooms next to the toilets and showers. Cleaning supplies and chemicals are locked in the garage. A locked cabinet in the kitchen is designated to store medications. Sharps/knives were observed in a locked drawer in the kitchen. Appliances observed to be in working order. First aid kit is located in the locked medication cabinet in the kitchen. Washer and Dryer observed in the laundry room with additional storage space available in upper cabinets. LPA toured the outside of the residence and observed a covered patio with seating area. Adequate outdoor seating for activities. A self-latching gate found to be working properly. There are no bodies of water on the premises. Exit interview conducted. No deficiencies cited on today's visit, there is no potential or immediate safety risks as there are no residents in care at this time. Licensee intends to receive more residents in 6 months. On 06/01/24, Licensing Program Analyst (LPA) L. Salazar arrived at the facility for an announced pre licensing inspection. LPA was greeted by Licensee/Administrator and was allowed entry into the facility. LPA toured the facility inside and out with licensee. LPA entered through the front door observed the required postings in the entry. LPA observed the residence to be a single story, 4 bedroom/ 2 bathroom home. There is a dining room and living room observed to have adequate furnishings and lighting. All bedrooms were observed to have beds, however, 4 out of 4 bedrooms have night stands, personal lighting and chest drawers to accommodate 2 residents per room. LPA observed fire extinguisher to be new and charged. Smoke detectors were tested and observed to be operational. Carbon Monoxide detectors were observed. Night lights in the hallways by the bathrooms were observed. LPA observed a adequate supply of towels. Hand washing signs were observed in the bathrooms. A supply of paper towels were observed in the bathrooms. Hot water temperature in bathroom measured at 114 degrees F. Non-Skid mats were observed in the shower. Grab bars were observed in the bathrooms next to the toilets and showers. Kitchen observed to have supply of dishes, plates, pots and pans. Food storage and preparation areas are clear and appropriate for food preparation. Cleaning supplies and chemicals are locked in the garage. A locked cabinet in the kitchen is designated to store medications. Sharps/knives were observed in a locked drawer in the kitchen. Appliances observed to be in working order. LPA observed a 7 day supply of non-perishable food. First aid kit was observed. Washer and Dryer observed in the laundry room with additional storage space available in upper cabinets. Doors and passageways are clear and free from obstruction throughout the home. LPA toured the outside of the residence and observed a cover patio with seating area. Adequate outdoor seating for activities. A self-latching gate found to be working properly. There are no bodies of water on the premises. Exit interview conducted. On 05/02/24, Licensing Program Analyst (LPA) L. Salazar arrived at the facility for an announced pre licensing inspection. LPA was greeted by Licensee/Administrator and was allowed entry into the facility. LPA toured the facility inside and out with licensee. LPA entered through the front door and did not observe the required postings in the entry. LPA observed the residence to be a single story, 4 bedroom/ 2 bathroom home. Deadbolt locks were observed on 3 out of the emergency exits and will need to be removed. There is a dining room and living room observed to have adequate furnishings and lighting. All bedrooms were observed to have beds, however, 4 out of 4 bedrooms did not have night stands, personal lighting and chest drawers to accommodate 2 residents per room. LPA observed fire extinguisher to be new and charged, however, no service or purchase date was provided on the extinguisher. Smoke detectors were tested and observed to be operational. Carbon Monoxide detectors were not observed. Emergency lighting /flashlights and night lights in the hallways by the bathrooms were not observed. Licensee will purchase. LPA observed a adequate supply of towels. There were no hand washing signs observed. A supply of paper towels were observed in the bathrooms. Hot water temperature in bathroom measured at 128.4 degrees F. Non-Skid mats were not present in the shower. Grab bars were observed in the bathrooms next to the toilets and showers. Kitchen observed to have supply of dishes, plates, pots and pans. Microwave and underneath cabinet next to stove needs to be degreased/cleaned. Food storage and preparation areas are clear and appropriate for food preparation. Cleaning supplies and chemicals are locked in the garage. A locked cabinet in the kitchen is designated to store medications. Sharps/knives were observed in a locked drawer in the kitchen. Appliances observed to be in working order. There are no bodies of water on the premises. The following observed will need to be brought into compliance: Chest of drawers for each resident Night stand for each resident Obtain first aid kit Microwave and underneath cabinet next to stove needs to be degreased/cleaned Turn down the water heater so temperature is under 120 degrees F Obtain telephone line service at the facility and provide phone number to CCL Proof of Fire Extinguisher in compliance Purchase Carbon monoxide detectors Posting of Fire Clearance, PUB 475, Resident rights, Visiting Policy Provide paper towels for bathrooms Purchase adequate food supply for 7 days Install night lights in hallway Patio needs covering for seating area. Deadbolt locks need to be removed on 3 out of 3 emergency exits Weather stripping on door exiting to backyard needs a section replaced Update the emergency disaster plan (LIC 610D) to reflect specific steps of what the facility will do in the event of an emergency disaster. Identification of the applicant / administrator was verified through interview questions based on photo ID and other identifying personal information.
