Licensing Program Analysts (LPA) B. Miranda conducted an unannounced visit today for the facility’s annual inspection. LPA introduced themselves and was allowed entrance into the facility. LPA met with owner Vikramjit Sidhu and Administrator Suhyun Ra. Facility is licensed for 6 residents and has a current census of 5. There are 2 residents on hospice and 2 with home health. Water temperature was checked in the kitchen which read at 113.5 degrees Fahrenheit and the common bathroom which read at 114.4 degrees Fahrenheit. Fire Extinguisher was serviced December 22, 2025 and is within the safety regulation period. Smoke and carbon monoxide detectors were tested and in working order. Suhyun Ra Administrator's Certification expires June 30, 2027. Staff files were reviewed, and LPA observed 2 caregivers in the home without proper clearance. Resident files were reviewed. Facility was not able to provide a log for disaster drills. First aid kit on site. Toxins and cleaning supplies are locked and inaccessible. There is a locked storage for medications. LPA inspected the interior and the exterior of the facility including the common living space, resident bedrooms, bathrooms, medication storage, kitchen, garage and outdoor areas. Bedrooms were clean, properly furnished, with adequate lighting, and in good repair. Food supply is adequate for 2-day perishable and 7-day nonperishable. Deficiencies observed were cited during today's inspection per California Code of Regulations, Title 22. LPA is requesting the following documents: LIC 500 Personnel Report, LIC 308 Designation of Administrative Responsibility, LIC 610-E the Emergency Disaster Plan and copy of current Administrator’s Certificate to update the facility file. Listed documents shall be sent to Licensing by May 27, 2026. Type of visit: Post Licensing

On 12/10//2025, Licensing Program Analyst (LPA) M, Medina conducted an unannounced Post Licensing Inspection. LPA arrived, introduced self, and allowed entrance. Administrator, Suhyn Ra was present. The facility is a 4 bedroom and 3 bathroom home, there are currently four (4) residents in home. All residents were home at time of visit. Facility tour began in residents bedrooms. All bedrooms observed to have required furnishings and adequate lighting. Resident bathroom toured, bathroom fixtures observed operational, shower has a non-skid surface, and water temperature measured 111 degrees F. Facility living room and dining room observed to have adequate seating available for residents in care. Kitchen toured, facility observed to have a 2-day supply of perishable and a 7-day supply of non-perishable food available. All knives observed to be locked and secured. Fire extinguisher present with a service date of 12/16/2024, smoke detectors and carbon monoxide detectors present and observed operational during post licensing inspections. All chemicals observed to be locked and secured in hall closet. Staff files reviewed during inspection. Outside of facility toured. Facility has a shaded area with seating available. Type of visit: Prelicensing

On 5/23/2025, Licensing Program Analyst (LPA) M. Medina conducted an announced Pre-licensing inspection. LPA introduced self, stated the purpose of the visit, and was allowed entrance into the facility. LPA met with Licensees Vikramjit Sidhu and Jeffrey Ra and Administrator, Suhyun Ra. LPA toured the facility with Licensees and Administrator. The facility is 4 bedrooms and 3 bathrooms home. Fire clearance was granted for 6 residents, 4 ambulatory and up to 2 bedridden residents. Common areas were furnished and had adequate seating and lighting available. All bedrooms were observed to have required furnishings. Bathrooms toured, fixtures observed operational. Grab bars available and skid-resistant surfaces in showers observed. Kitchen was toured and observed to have pots, pans, plates, cups and utensils. Facility has 7-day supply of non-perishable food available as of date of inspection. Knives observed to be locked and secured. Medication will be locked and secured in hall closet. Cleaning supplies and chemicals will be locked and secured in hall closet. Fire extinguisher present with a service date of 12/16/2024. Smoke detectors and carbon monoxide detectors were observed to be operational during this inspection. Water temperature measured at 109 degrees F. Facility has a working telephone of (661) 615-3054. . Outside of facility toured. Facility has a covered patio with seating available. Exits open free of obstruction. . Gate observed to be self-latching. Component III conducted during pre-licensing inspection. LPA found that applicant has met all pre-licensing requirements. LPA will submit documentation to CAB in Sacramento for final review prior to license being issued. Exit interview conducted.
